Recent studies have challenged the concept that 50% of colon cancers are detectable by digital examination and two-thirds are within reach of the rigid sigmoidoscope. This is an important consideration because of the potential for failure of an otherwise appropriate screening method when evaluating a patient for carcinoma of the colon. BACKGROUND: Appendiceal goblet cell adenocarcinoma (GCA) is a rare malignant tumor originating from the appendiceal mucosa, with an insidious onset. Its biological behavior lies between that of a carcinoid tumor and an adenocarcinoma, and it has a relatively favorable prognosis but a high risk of long-term recurrence and metastasis. The coexistence of primary cecal tubular adenocarcinoma and appendiceal GCA is extremely rare, and poses challenges in diagnosis and treatment.

A comparative study was performed between plain abdominal films of 110 patients with neoplasms of the large bowel and 52 normal subjects for control. In neoplasms of the right colon the failed spontaneous visualization of caecum and ascendent colon or the direct recognization of lume's deformation allow us to suspect such a lesions in about 90% of the cases. Neoplasms of the other tracts of the large bowel may be suspected only when plain abdominal film demonstrates direct evidence of an obstruction: in about 25% of the cases.

A case of carcinoid tumor of the last ileal loop is presented; the first clinical sign was exophthalmos due to bilateral orbital metastases. The case is believed to be the third recorded one of carcinoid with metastases to the orbit; other metastatic lesions were located in myocardium, liver, spleen, kidney, ovary and trachea. Porgressive hirsutism was present in association with the somewhat atypical clinical picture.

Neuroendocrine tumors (NET) develop from the diffuse endocrine system. These are rare tumors that can affect diverse organs. We present here the case of a 42-year-old female patient in whom a NET of the breast was discovered that was likely not of mammary origin. The main challenge was finding the primary tumor using immunohistochemistry and specific medical imaging modalities for NET. We report a case of primary appendiceal cancer that metastasized to the ureter. The patient was a 51-year-old woman who complained of persistent left back pain and right lower abdominal pain. After a detailed examination she was diagnosed to have cancer of the appendix, multiple liver metastases, and left hydronephrosis. A 26-year-old lactating mother presented with a 3-week history of abdominal pain, constipation, and vomiting. She denied any history of alcohol abuse or other gastrointestinal problems. Contrast-enhanced CT identified a small-bowel obstruction caused by a cecum cancer (Fig. 1A). Therefore, she underwent right hemicolectomy and ileocolic anastomosis. Post-operatively, she gradually developed drowsiness, fainting, and a rapid heart rate at 130 bpm. However, blood tests were all normal.
