Functional and morphological evaluation of the trapezius muscle after spinal accessory nerve transfer to brachial plexus nerves.

INTRODUCTION: The main innervation of the trapezius muscle is provided by the spinal accessory nerve. Several studies describe the contributions of cervical plexus roots to the trapezius muscle innervation, either directly or through connections with the spinal accessory nerve. There is no adequate understanding of how the trapezius muscle is affected after using the spinal accessory nerve in nerve transfer procedures with the usual technique, preserving at least 1 branch for the upper trapezius. METHODS: We evaluated 20 patients with sequelae of traumatic brachial plexus injury who underwent…

Diagnosis of obstetric brachial plexus injury in a 2-year-old girl using high‑frequency ultrasonography.

We describe an unusual case of infant obstetric brachial plexus injury located in the cervical (C)5-C6 brachial plexus nerve, which was preoperatively diagnosed using high-frequency ultrasonography (US) at 2 years of age. The girl was diagnosed with a right clavicular fracture because of shoulder dystocia. She had been showing movement limitations of her entire right upper limb after fracture healing and was then referred to our hospital at 2 years of age. High-frequency US showed that the roots of the right brachial plexus ran continuously, but the diameter of C6 was thinner on the affected s

Brachial plexus paralysis after thoracoscopic esophagectomy for esophageal cancer in the prone position: A thought-provoking case report of an unexpected complication.

INTRODUCTION: During prone esophagectomy, placement of a port in the third intercostal space for upper mediastinal dissection requires adequate axillary expansion. To facilitate this, the right arm is elevated cranially and simultaneously turned outward. Brachial plexus paralysis associated with esophagectomy in the prone position has not been documented. PRESENTATION OF CASE: A 58-year-old man diagnosed with middle intrathoracic esophageal cancer was referred to our department. Thoracoscopic esophagectomy in the prone position was performed following neoadjuvant chemotherapy. After surgery,

Development of the cerebrospinal fluid pathway in the normal and abnormal human embryos.

The subarachnoid space, the chorioid plexus and the arachnoid villi are microscopically studied in 60 normal human embryos and in 3 abnormal human embryos with rhombencephaloschisis and cervical myeloschisis. The subarachnoid space has been generally considered to be developed by outflow of cerebrospinal fluid (CSF) of the choroid-plexus origin from the IVth ventricle. This generally accepted concept does not meet with our findings: (1) cavity formation in the meninx primitiva is seen before appearance of the choroid plexus; (2) the primitive subarachnoid space is developed earlier in the prep

Sensory evaluation of the hands in children with brachial plexus birth injury.

The aim of this study was to examine sensory changes of the hand in brachial plexus birth injury (BPBI). Ninety-five patients (43 females, 52 males) comprising two age groups, 6 to 8 years (mean age 7y 6mo) and 12 to 14 years (mean age 13y 2mo), were included. Sixty-four had upper (cervical [C] 5-6), 19 upper and middle (C5-7), and 12 had total (C5-thoracic (Th)1) plexus injury. Obstetric plexus surgery had been performed on 14 patients at the age of 1 to 6 months. The sensibility of 32 palmar areas of both hands was tested with Semmes-Weinstein filaments (calibres 2.83, 3.61, 4.31, 4.56, and

Styloid-carotid artery syndrome treated surgically with Piezosurgery: a case report and literature review.

Styloid-carotid artery syndrome was first described by Eagle and is associated with cervical and facial pain caused by head movement resulting from mechanical compression of the carotid nerve plexus due to an elongated styloid process. The case of a 49-year-old man with persistent cervical pain, neurological symptoms and an elongated styloid process of 7.5 cm is reported here; this patient was successfully treated using Piezosurgery. In addition, a literature review is included.

Fascicular Injury in True Neurogenic Thoracic Outlet Syndrome Associated with Manual and Shockwave Therapies: A Case Report.

True neurogenic thoracic outlet syndrome (nTOS) is a rare condition resulting from brachial plexus compression, frequently associated with congenital anomalies such as cervical ribs or fibrous bands. We report a unique case of nTOS involving both a cervical rib and fibrous band, complicated by an incidental intraoperative finding of a fascicular rupture in the lower part of lower trunk. Notably, the patient had no history of trauma but had undergone multiple sessions of manual therapy and a single extracorporeal shockwave therapy to the cervical region, raising concerns about a potential iatro

Ultrasound measurement of the distance of the phrenic nerve from the brachial plexus at the classic interscalene point and upper trunk: A volunteer-based observational study.

BACKGROUND AND AIMS: The method of blocking the brachial plexus at the level of the upper trunk has been gaining popularity as a phrenic nerve-sparing alternative for interscalene block. We aimed to measure the distance of the phrenic nerve from the upper trunk and compare it with the distance between the phrenic nerve and the brachial plexus at the classic interscalene point by using ultrasound. METHOD: In this study, after ethical approval and trial registration, 100 brachial plexus of 50 volunteers were scanned from the emergence of the ventral rami and its course was traced to the supracl

A neck mass with brachial plexus injury: Pott's disease.

Tuberculous osteomyelitis of the spine is most commonly seen in lower thoracic and lumbar vertebrae. Cervical spine tuberculosis is a very rare condition, and it represents a very small part of all patients with Pott's disease. We present a case with thoraco-cervical Pott's disease, with left-sided neck mass and left arm and hand weakness and numbness. The patient had a paraspinal abscess under the sternocleidomastoid muscle that was compressing the brachial plexus.

Upper limb tension tests as tools in the diagnosis of nerve and plexus lesions. Anatomical and biomechanical aspects.

OBJECTIVE: To analyse the validity of nerve tension tests used in the diagnosis of nerve (root) and plexus lesions of the upper extremity. DESIGN: In six arms of embalmed human bodies, in situ measurements were performed to assess the effect of nerve tension tests on the median, ulnar and radial nerves and the cords of the brachial plexus. BACKGROUND: In clinical practice it is useful to have fast, easy and cheap tests for the diagnosis of nerve (root) lesions of the upper extremity, analogous to Lasègue's Straight Leg Raising test.Methods. The Upper Limb Tension Tests for the median, ulnar

Facial nerve communication with ansa cervicalis - An unusual anatomical variation.

Neural connections of the seventh cranial nerve with its neighboring nerves are common and well documented; however, communication with ansa cervicalis is as yet unknown. We present a case with such a connection found during cadaveric dissection, with hitherto unknown consequences. In this specimen, after giving the marginal mandibular and cervical branches, the cervicofacial division continued distally to communicate with the distal loop of ansa cervicalis. Presence of such connection may result in facial muscle paralysis on injury to the ansa or strap muscle paralysis on injury to the facial

Supraclavicular Resection of a Cervical Rib Causing Thoracic Outlet Syndrome: 2-Dimensional Operative Video.

Presence of a cervical rib results from overdevelopment of the seventh cervical vertebrae.1-3 The cervical rib along with scalene muscles can cause neurogenic thoracic outlet syndrome.4,5 Rib resection is typically done via anterior approach, using either supraclavicular or transaxillary route.6,7 We present an operative video detailing supraclavicular resection of a cervical rib causing neurogenic thoracic outlet syndrome with direct decompression of the lower trunk of the brachial plexus. The patient presented with severe symptoms including hand atrophy. We were able to directly visualize th

Hirayama disease: a rare cause of cervical myelopathy.

Hirayama disease (HD) is a rare cervical myelopathy involving lower cervical myotomes, causing asymmetric or unilateral distal upper extremity muscle atrophy. Diagnosis relies on clinical findings, nerve conduction studies, and cervical spine MRI in neutral and flexion positions. We present a 17-year-old male with painless, progressive weakness and atrophy of the left forearm and hand muscles, along with hyperhidrosis. Initial cervical spine MRI revealed loss of cervical lordosis, lower cervical hemicord atrophy, and increased T2 signal intensity in the anterior horn cells. Flexion MRI demonst

Neuralgic amyotrophy with hourglass-like constrictions: A case report.

BACKGROUND: Neuralgic amyotrophy (NA) is a rare disease with sudden upper limb pain followed by affected muscle weakness. The most commonly affected area in NA is the upper part of the brachial plexus, and the paraspinal muscles are rarely affected (1.5%), making these cases difficult to distinguish from cervical radiculopathy. CASE SUMMARY: A 76-year-old male presented to the emergency department with left hip pain post-fall. After undergoing left femoral neck fracture surgery, he experienced sudden left shoulder pain for 10 days with subsequent left arm weakness. Cervical spine computed tom

Bakody's Test Positive Thoracic Outlet Syndrome Caused by Anomalous Muscle: A Case Report.

Anomalous muscle causes thoracic outlet syndrome (TOS). A 40-year-old man presented with numbness of the left upper extremity, similar to cervical spondylotic radiculopathy. He presented with a positive Bakody's test. However, magnetic resonance imaging showed no significant changes in the cervical spine and revealed an anomalous muscle adjacent to the left brachial plexus. We diagnosed the muscle as the cause of TOS and performed a resection, which resulted in symptomatic improvement.

Intraspinal intradural variations of nerve roots.

PURPOSE: The aim of this work is to point out the intraspinal anatomical variations of nerve roots and their possible participation in radiculopathy. METHODS: The anatomical study was performed in 33 cadavers. There were 25 male cadavers aged 30-75 years and 8 female cadavers aged 45-77 years, with a mean age of 46.5 years to 24 h from death. All intradural rami communicantes between nerve roots were excised and examined histologically for the presence or absence of nervous tissue. The type of the plexus was defined by subtracting from the root C2. RESULTS: Findings of the normotype of plexu
