Comparison of Population-Based Census versus Birth History for the Estimation of Under-5 Mortality in Niger.

The WHO guidelines on mass distribution of azithromycin for child survival recommend monitoring of mortality to evaluate effectiveness. Trials that contributed evidence to these guidelines used a population-based census to monitor vital status, requiring census workers to visit each household biannually (twice yearly). Birth history is an alternative to the census approach that may be more feasible because it decreases the time and labor needed for mortality monitoring. This study aimed to compare the population-based census (reference standard) and birth history (index test) approaches to est

[Maternal and child health care in Cuba: achievements and challengesCuidados de saúde materno-infantil em Cuba: conquistas e desafios].

In Cuba, maternal and child health care is based on the priority granted by the State, the implementation of the National Maternal and Child Health Program and the guarantee of equitable access to health services. This article describes the Cuban experience in this field, as well as its main achievements, challenges, and lessons learned. Among the most relevant results up to 2015 are the reduction of infant mortality rate and under-five mortality rate to 4.3 and 5.7 deaths per 1 000 live births, respectively; 5-year survival of 99.4%; more than 10 prenatal check-ups per delivery; 5.3% of low b

Artificial intelligence-powered models in predicting mortality in maternal, newborn, and children under five: a systematic review protocol.

BACKGROUND: Maternal, newborn, and under-five mortality remain significant global health challenges, with 287,000 maternal deaths reported in 2020 and 4.9 million under-five deaths in 2022. The use of artificial intelligence in prediction models has shown great potential in predicting these mortalities. Artificial intelligence (AI)-powered models are capable of processing complex datasets to identify non-linear relationships and patterns, potentially outperforming traditional statistical methods in predictive accuracy, thereby enabling early detections and interventions more specifically. As A

Systematic review: The model for end-stage liver disease--should it replace Child-Pugh's classification for assessing prognosis in cirrhosis?

BACKGROUND: Prognosis in cirrhotic patients has had a resurgence of interest because of liver transplantation and new therapies for complications of end-stage cirrhosis. The model for end-stage liver disease score is now used for allocation in liver transplantation waiting lists, replacing Child-Turcotte-Pugh score. However, there is debate as whether it is better in other settings of cirrhosis. AIM: To review studies comparing the accuracy of model for end-stage liver disease score vs. Child-Turcotte-Pugh score in non-transplant settings. RESULTS: Transjugular intrahepatic portosystemic shu

Chromobacterium violaceum infections in children: two case reports and literature review.

PURPOSE: Chromobacterium violaceum(C. violaceum) is a gram-negative bacterium that rarely infects humans, especially children. However, the mortality rate is high and there are no clear guidelines for treatment. The aim of this paper is to increase clinicians' awareness of diseases caused by C. violaceum infections in children, to diagnose and treat them in a timely manner, to improve patient survival and to reduce mortality. RESULTS: We analysed the latest paediatric-related English language literature over the last 10 years and summarised the latest mechanisms of injury, susceptibility fact

[Fertility and health in Mexico].

Fertility, health, and family planning are not independent factors, but rather involve a series of biological and social mechanisms in close interaction with one another. The impact that a high fertility rate has on health is reflected mainly in a rise in the rates of maternal and child mortality. Similarly, fertility has a greater negative effect upon the health of groups characterized by high reproductive risk, high parity, short intergenesic intervals, and unwanted pregnancies. On the other hand, family planning -and specifically the use of contraceptive methods-helps to achieve a lowering …

Emergency surgery for bleeding oesophageal varices.

PURPOSE OF STUDY--This case-series reviews patients who underwent emergency transgastric ligation or oesophageal transection for uncontrolled bleeding oesophageal varices despite initial conservative therapy with vasoactive drugs, balloon tamponade and injection sclerotherapy. The study seeks to identify factors determining outcome of surgery and the problems of transection surgery following endoscopic sclerotherapy. SELECTION OF STUDY SUBJECTS--One hundred and ninety-eight patients (median age 54.6 years) were treated for bleeding varices between 1981 and 1991. Of these, 36 (18%) required eme

Women, children and adolescents in conflict countries: an assessment of inequalities in intervention coverage and survival.

INTRODUCTION: Conflict adversely impacts health and health systems, yet its effect on health inequalities, particularly for women and children, has not been systematically studied. We examined wealth, education and urban/rural residence inequalities for child mortality and essential reproductive, maternal, newborn and child health interventions between conflict and non-conflict low-income and middle-income countries (LMICs). METHODS: We carried out a time-series multicountry ecological study using data for 137 LMICs between 1990 and 2017, as defined by the 2019 World Bank classification. The …

Child abuse: measuring a global problem.

Child abuse and neglect include four distinct conditions: physical abuse, neglect, emotional abuse and sexual abuse. The magnitude of the problem is not well defined. The lack of epidemiological data limits the extent to which sound public health and social welfare policies and intervention programmes could be developed, implemented and evaluated. In recent years two approaches have been developed to document the magnitude and nature of child abuse and neglect: regional or national case registers; and a screening instrument for suspected cases of child abuse and neglect (SCAN) for use in healt

The evolution of fetal and infant mortality review as a public health strategy.

Infant mortality review (IMR), the forerunner of fetal and infant mortality review (FIMR), emerged at the national level in the mid-1980s as a promising method to improve understanding of local factors contributing to infant mortality and to motivate community response. Building on federal efforts to enhance data capacity and early state and local infant mortality case review studies, the federal Maternal and Child Health Bureau (MCHB) initiated its IMR Program in 1988. Key actions taken to refine and diffuse the IMR/FIMR method include forging a public-private partnership between MCHB and the
