In an 8-year-old boy, the Stevens-Johnson syndrome (SJS) was associated with protracted intrahepatic cholestasis and ultimately fatal respiratory disease. No precipitating factors of SJS were identified, but a Mycoplasma pneumoniae infection was suspected. The appearance of jaundice on the seventh day of illness was preceded by the prescription of erythromycin ethyl succinate, but intrahepatic cholestasis has never before been associated with this derivative of erythromycin. It is suggested that there may exist a subtype of SIS with severe hepato-pulmonary pathology.

Two premature female infants with a new variant of pulmonary hyaline membrane disease had both received assisted ventilation with high oxygen concentrations. Postmortem examination showed straw-yellow hyaline membranes lining the terminal bronchioles and alveoli. Pneumonocytes and macrophages also contained straw-yellow pigments in the cytoplasm. These yellow membranes and pigments in pneumonocytes and in macrophages stained positively for bile and negatively for iron. Both infants had severe intrahepatic cholestasis. Cholestasis and pulmonary changes due to oxygen toxicity are believed to be

Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific disorder characterized by elevated maternal serum total bile acid (TBA) and increased risk of adverse fetal outcomes, yet current therapies and preventive strategies remain suboptimal. Here, we present a preclinical investigation of nanocellulose as a gut microbiota-targeted dietary intervention to restore bile acid homeostasis. We demonstrate that nanocellulose, particularly cellulose nanofibers (CNF), administered prophylactically, attenuated maternal TBA and improved offspring survival in a rat ICP model by enriching probio

Cholestasis secondary to protein serine kinase H1 (PSKH1) is a recently described entity, which leads to chronic liver disease in childhood or early teenage years with or without renal insufficiency. Mutations in the PSKH1 gene reduce PSKH1's kinase activity, leading to abnormally long and convoluted cilia in renal tubules and bile ducts, disrupting normal function. There are very few reports highlighting the outcomes of liver transplantation (LT) for PSKH1-related liver disease. There have been no reports of PSKH1-induced cholestasis reported from India till date. We describe the first report

INTRODUCTION: Despite the increasing body of evidence that autophagy implicate in intrahepatic cholestasis of pregnancy (ICP), we are still far from a mechanistic understanding of the autophagy in ICP pathogenesis.
METHODS: In this study, we performed untargeted lipidomics, scRNA-seq, and RNA-seq data to systematically demonstrate the lipid alterations, cells and gene expression closely related to autophagy in placenta. These results were also verified by tissue, cell, and animal experiments.
RESULTS: We confirmed that autophagy played a pivotal role in ICP, autophagy-related lipids includin

BACKGROUND AND AIMS: USP53 variants cause low-GGT progressive familial intrahepatic cholestasis (PFIC). The mechanism is not well understood. USP53, which encodes a ubiquitin-specific protease, has been proposed to be involved in blood-bile barrier impairment. However, Usp53 knockout mice did not show blood-bile barrier impairment. The aim of this study was to investigate the molecular mechanism underlying USP53 -PFIC.
APPROACH AND RESULTS: Immunohistochemistry of patient tissue, confocal immunofluorescence microscopy and surface protein biotinylation were performed to investigate the localiz

BACKGROUND/AIMS: A heterogeneous group of disorders caused by bile secretion and transport defects is progressive familial intrahepatic cholestasis (PFIC). PFIC has various subtypes with different presentations, laboratory findings, treatments, progression, and prognosis. Genetic analysis is the gold standard for diagnosis but is costly, time-consuming, and not readily available. In this study, immunohistochemistry (IHC) was evaluated as a tool for identifying subtypes of PFIC and differentiating them from other causes of pediatric cholestasis.
METHODS: The study included genetically confirme

BACKGROUND: Inflammatory Bowel Disease (IBD) requires maintenance of remission during pregnancy to avoid poor maternal and fetal outcomes. Intrahepatic cholestasis of pregnancy (ICP) could also increase these risks.
AIMS: To examine the prevalence of ICP in pregnancies with IBD and associations with thiopurine exposure.
METHODS: An electronic search of MEDLINE, EMBASE, and EMBASE Classic databases using search terms for IBD and ICP from inception to 10th September 2024 was performed. Studies involving pregnant adults with a confirmed diagnosis of IBD reporting ICP were included. Prevalence a

Few studies have reported an association between intrahepatic cholestasis of pregnancy (ICP) and preconception exposure to PM2.5 and sunlight duration, but there has been no in-depth analysis of the correlation between ICP and different constituents of PM2.5. Thus, we performed this retrospective analysis among 160,544 pregnant women who delivered between 2014 and 2020, to further estimate the impact of different constituents of PM2.5, as well as the duration of sunlight, on ICP via generalized linear models. During the three months prior to conception, the adjusted odds ratios (aORs) for ICP

BACKGROUND: Repeated episodes of jaundice and pruritus are common in a group of autosomal recessive liver diseases known as benign recurrent intrahepatic cholestasis. Benign recurrent intrahepatic cholestasis (BRIC) is divided into two types, type 1 and type 2, and is caused by mutations in the ATP8B1 and ABCB11 genes. Here, we report a rare case of BRIC type 2 mutation.
CASE PRESENTATION: A 45-year-old Chinese man had three frequent episodes of jaundice marked by extensive excoriation and severe pruritis, although he had no prior history of jaundice. Laboratory investigations showed no evide

Objective: To conduct clinical and genetic analysis in two cases of cholestatic liver disease to determine the specific etiology of cholestasis. Methods: Clinical data and the medical histories in family members of two cases were collected. The gene variation was detected by whole-exome sequencing technology. Sanger sequencing validation and bioinformatics analysis were performed on patients and their parents with suspected pathogenic mutations. Results: Whole-exome sequencing showed that the ABCB4 gene of case 1 (a male, 16 years old) had compound heterozygous mutations of c.646C > T from the

PURPOSE: Progressive familial intrahepatic cholestasis (PFIC) is a cohort of autosomal recessive syndromes which presents with jaundice, severe pruritus and liver derangement. Without treatments, patients progress to liver failure in early childhood. Biliary diversion strategies have been deployed to interrupt enterohepatic circulation to alleviate symptoms and delay progression to cirrhosis. Cholecystocolostomy has been the diversion method of choice at our institution and we aim to evaluate its long-term outcome.
METHODS: All patients with PFIC who underwent cholecystocolostomy between Augu

INTRODUCTION: Previous studies of fetal effects have suggested that intrahepatic cholestasis of pregnancy is associated with a higher rate of adverse neonatal outcomes including preterm birth, neonatal respiratory distress syndrome, meconium-stained amniotic fluid, neonatal intensive care unit admission, and stillbirth. The objective was to compare the neonatal and maternal consequences in pregnancies affected by intrahepatic cholestasis and normal pregnancies.
MATERIAL AND METHODS: This case-control study compares pregnancies affected by intrahepatic cholestasis (pruritus and bile acid ≥ 10

Background Progressive familial intrahepatic cholestasis type 3 (PFIC3) is an uncommon cholestatic liver disease caused by mutations in the ATP binding cassette subfamily B member 4 (ABCB4) gene. Although PFIC3 is frequently identified in childhood, ABCB4 disease-causing alleles have been described in adults affected by intrahepatic cholestasis of pregnancy, hormone-induced cholestasis, low-phospholipid-associated cholelithiasis syndrome or juvenile cholelithiasis, cholangiocarcinoma and in sporadic forms of primary biliary cirrhosis. Cholestanol is a biomarker which is elevated especially in

Multidrug resistance 3 (MDR3), encoded by the ATP-binding cassette, subfamily B, member 4 gene (ABCB4), localizes to the canalicular membrane of hepatocytes and translocates phosphatidylcholine from the inner leaflet to the outer leaflet of the canalicular membrane. Progressive familial intrahepatic cholestasis type 3 (PFIC3) is a rare hepatic disease caused by genetic mutations of ABCB4. In this study, we characterized 8 ABCB4 mutations found in PFIC3 patients, using in vitro molecular assays. First, we examined the transport activity of each mutant by measuring its ATPase activity using pacl

Hepatic cirrhosis is an important cause of morbidity and mortality. An unusual case of cirrhosis and portal hypertension in an 18-year-old patient secondary to Progressive Intrahepatic Cholestasis is discussed. The clinical and biochemical findings are discussed and a clinical approach to determining the underlying etiology of cirrhosis is outlined. Significant complications of portal hypertension include ascites, spontaneous bacterial peritonitis, hepatorenal syndrome, varices, and hepatic encephalopathy. A clinical approach to these complications of cirrhosis is presented. Progressive Famili

We report the case of a 68-year-old African woman who presented with jaundice, hepatomegaly and anasarca. Clinical investigation disclosed severe intrahepatic cholestasis, nephrotic syndrome, erythrocytosis and hypoglycemia. Diagnosis of systemic AL amyloidosis was established by percutaneous liver biopsy. Bone marrow biopsy showed 32% of myeloma cells. The patient started treatment with melphalan and prednisolone, but liver function deteriorated and she died in hepatic failure complicated by septic shock three weeks after the diagnosis. We present possible explanations for the unusual clinica

The multidrug resistance-associated proteins (Mrps) are a family of adenosine triphosphate-dependent transporters that facilitate the movement of various compounds, including bile acids, out of hepatocytes. The current study was conducted to determine whether induction of these transporters alters bile acid disposition as a means of hepatoprotection during bile acid-induced cholestasis. Lithocholic acid (LCA) was used to induce intrahepatic cholestasis. C57BL/6 mice were pretreated with corn oil (CO) or known transporter inducers, phenobarbital (PB), oltipraz (OPZ), or TCPOBOP (TC) for 3 days

OBJECTIVE: Intrahepatic cholestasis of pregnancy (ICP) has been associated with prematurity and fetal mortality. Recently, ICP has also been recognised as a risk factor for neonatal respiratory distress syndrome (RDS) in term or near-term neonates. Since fetal mortality is more frequent in pregnancies with an early ICP onset, we speculated that the time of exposure (ET) to maternal bile acids at the delivery (BAdeliv) could be involved in neonatal lung damage too. Study aim was to develop a scoring system to predict the RDS occurrence.
DESIGN: We conducted a retrospective analysis of 77 pregn
