Transudative chylothorax: an uncommon illness due to a common cause.

Chylothorax is defined as presence of chyle in the pleural space. It is commonly associated with cardiothoracic surgery, trauma, malignancy or some benign disorders. Transudative chylothorax is uncommon. A 52-year-old man presented with bilateral chylothorax with preceding history of blunt trauma to the chest. On further evaluation, he was diagnosed to have a transudative chylothorax due to cirrhosis of liver with coexisting chylous ascites and evidence of peritoneopleural communication. The patient was managed with diuretics followed by chemical pleurodesis with iodopovidone.

[Left-sided Chylothorax and Tumor of the Right Anterior Mediastinum].

An 82-year-old male patient presented with a left-sided spontaneous chylothorax. A computed tomographic scan of the chest revealed a cystic tumor in the right upper anterior mediastinum and nodules in the left mediastinum that we interpreted as enlarged lymph nodes. On suspicion of lymphoma and for therapy of chylothorax, we performed a left-sided thoracotomy. The putative lymph nodes proved to be chylomas. We interpret the right-sided mediastinal tumor as cystic lymphangioma causing the left-sided chylothorax at the junction of thoracic duct and venous system on the right side.

Constrictive pericarditis as a rare cause of chylothorax: a case report.

BACKGROUND: Chylothorax is a rare clinical condition that results from thoracic duct damage with leakage of chyle from the lymphatic system to the pleural space. Rarely, constrictive pericarditis has been associated with chylothorax, but to our knowledge only in relation to secondary causes such as tuberculosis, HIV, or malignancy. CASE SUMMARY: A previously healthy 63-year-old man presented with effusive-constrictive pericarditis, recurrent right-sided pleural effusion, and chylothorax. There was no history of co-morbidities, surgical illness, or cardiac procedures. No single aetiologic fact…

Traumatic chylothorax in a young child: Case report and management.

INTRODUCTION: A chylothorax is an uncommon feature of paediatric chest trauma. CASE REPORT: We report a case of traumatic chylothorax following blunt chest trauma in an eight year-old girl with polytrauma after being hit by a motor vehicle. She was initially found to have a bilateral frontal skull fracture extending into the left parietal area, pulmonary contusions, left posterior rib fractures, left clavicular fracture and a degloving injury of her left foot. On the fifth day of her admission she developed progressive dyspnoea with signs of a pleural effusion, which was confirmed radiologica…

Congenital Chylothorax of the Newborn: Diagnosis and Treatment in Three Pictures.

There is general agreement regarding the evident need for an international, multicenter trial including long-term follow-up to establish the correct criteria for diagnosing and managing congenital chylothorax. In an attempt to identify these criteria, which could then be used to draft a prospective multicenter trial, we propose three flow-charts showing three algorithms that could be used to: 1) obtain a definitive diagnosis of pleural chylous effusion; 2) specifically focus on chyle leakage evolution and etiology of chylothorax; and 3) focus on the management of congenital chylothorax. The ai…

Bilateral chylothorax as a complication of internal jugular vein cannulation.

Central venous catheterization is one of the most prevalent procedures in the Intensive Care Unit. Complications are reported in about 15% of the patients and usually comprise of infection, arterial puncture, malpositioning, pneumothorax, local hematoma, hemothorax, and so on. Chylothorax is a rare complication of this procedure. We present a 42-year-old lady, who developed bilateral massive chylothorax after cannulation of her left internal jugular vein (IJV), due to direct injury to the thoracic duct during the procedure. The patient was successfully managed with bilateral chest tube drainag…

Transudative chylothorax in a patient with liver cirrhosis: A rare association.

Chylothorax is an unusual type of pleural effusion which results from the accumulation of chyle in the pleural cavity. High triglyceride content and presence of chylomicrons in the chyle give this fluid a characteristic milky appearance. Chylothorax most commonly results from the obstruction of the thoracic duct by a malignant lesion or from its traumatic disruption. Liver cirrhosis is an uncommon and frequently underappreciated cause of chylothorax. Pleural effusion in chylothorax is typically described as a lymphocytic predominant, exudative type and it is exceedingly rare to encounter a tra…

Treatment of congenital chylothorax with octreotide in a hydropic preterm infant.

Congenital chylothorax is rare in preterm infants. While most cases respond to conservative treatment, a few require surgery. Treatment with intravenous octreotide has been reported to have varying success in preterm infants. A fetus was diagnosed with bilateral hydrothoraces at 29 weeks of gestation and repeated thoracocentesis was performed antenatally to allow growth of the lungs. She was delivered electively at 32 weeks by caesarean section. Hydrops fetalis was confirmed and chest tubes were inserted bilaterally soon after birth. Intravenous octreotide was commenced on day 4 of life, titra…

A case of chylothorax treated curatively with Sapylin, a streptococcus preparation.

Chylothorax is an uncommon disease where fatty fluid accumulates within the chest cavity. Conservative management, including repeated thoracentesis or pleurodesis, seems to be suitable to most cases. Herein, we present a case of efficacious pleurodesis by intrapleural injection of Sapylin, a streptococcus preparation, for the treatment of chylothorax. A 52-year-old non-smoking female farmer was diagnosed as idiopathic chylothorax after we ruled out possible causes including chest trauma, lymphoma, lung cancer, filariasis, tuberculosis, and etc. Two-time intra-thoracic injection of 3 Klinische …

[Idiopathic chylothorax: usefulness of aetiologic survey and the follow-up].

Chylothorax is an uncommon condition. It's characterized by the presence of chyle in the pleura. The most common causes are malignancy and trauma. When an underlying cause is excluded, the chylothorax is called idiopathic. We report a case of a 68 aged woman, admitted for abundant chylothorax. Thoraco-abdominal computed tomography, magnetic resonance imaging of the chest and thoracotomy showed no lesion of the lymphatic duct. Fibrothorax has developed after pleurodesis. Stable clinical and radiological condition was noticed during 3 years. We discuss the difficulty in the exclusion of a malign…

Thoracoscopic direct clipping of the thoracic duct for chylopericardium and chylothorax.

BACKGROUND: Chylothorax is a challenging clinical problem. Untreated, it carries a high mortality and morbidity. Traditional surgical management for cases refractory to conservative treatment is thoracic duct ligation through a right open thoracotomy. METHODS: We describe 4 patients treated successfully by video-assisted thoracic surgery, using ports and no thoracotomy, and precise ligation and division of the thoracic duct just above the diaphragm. A pericardial window was made in the patient with chylopericardium, as in the patient with end-stage renal disease. Pleurodesis was used in the p…

Use of omentum as a physiologic drain for treatment of chylothorax in a dog.

OBJECTIVES: To describe a novel technique for the surgical treatment of idiopathic chylothorax in a dog. ANIMALS: A 6-year-old, male Rhodesian Ridgeback, which presented with a history of reduced exercise tolerance and dyspnea. METHODS: Idiopathic chylothorax was diagnosed. Intermittent pleural drainage failed to resolve the problem. During surgery, extensive pleural fibrosis made it impossible to identify the thoracic duct. A dorsal omental pedicle was advanced through an incision in the diaphragm and into the cranial thoracic cavity to act as a physiological drain. RESULTS: The dog recove…

The use of pleuroperitoneal shunts in the management of persistent chylothorax in infants.

BACKGROUND/PURPOSE: The development of chylothorax is a serious and often life-threatening clinical entity that may cause profound respiratory, nutritional, and immunologic complications and has become increasingly common in recent years. Optimal management of this problem has not been well defined because medical therapy has a significant failure rate. Surgical treatment of complicated chylothorax has become a mainstay of care. METHODS: Over the last 36 months, seven infants had a pleuroperitoneal shunt placed for the management of refractory chylothorax. Ages ranged from 10 to 66 days with …

Accessory lung lobe torsion and chylothorax in an Afghan hound.

Torsion of the accessory lung lobe is extremely rare and has been reported only in one human. Accessory lung lobe torsion (LLT) and chylothorax occurred in a 3.5-year-old, male Afghan hound. An exploratory thoracotomy was used to remove the torsed lung lobe. When medical management of the chylous effusion failed, thoracic duct ligation was performed, resulting in resolution of the effusion. The dog remains clinically healthy 29 months postoperatively.

Chylothorax in a twin pregnancy of 34 weeks--sonographically diagnosed.

In a twin pregnancy of 34 gestational weeks sonographic follow-up revealed a rapidly spreading hydrothorax in one twin and slight ascites in the other. Analysis of aspirated fluid by intrauterine needle puncture suggested a chylothorax. Pathogenesis of chylothorax is discussed. Differential diagnosis of hydrothorax is presented. Intrauterine puncture is the only method of achieving prenatal diagnosis and therapy.

Neonatal chylothorax: a report and discussion of nine consecutive cases.

We here report the clinical findings and management of 9 consecutive cases of chylothorax, 5 of which occurred spontaneously. One cases followed cardiac surgery. Three cases occurred in low-birth-weight, premature infants concurrently with other symptoms of the superior vena cava syndrome secondary to central intravenous nutrition. To our knowledge, this is the first description of chylothorax as a possible complication of total parenteral nutrition in newborn babies. In our treatment of chylothorax in the newborn, we employed diagnostic thoracentesis followed by chest tube drainage and a medi…

Chylothorax and lymphangiomas of bone: unusual manifestations of lymphatic disease.

A patient with chylothorax, mediastinal lymphangiomatous malformation of the thoracic duct, and multiple lymphangiomas of bone is reported. If a patient has radiographic findings of increased pleural fluid with multiple lucent lesions of the skeleton, the possibility of a generalized abnormality in the lymphatic system should be considered. Early diagnosis and treatment can prevent a protracted course and wasting of the body due to the loss of essential body nutrients in the chylous fluid.

Chylothorax vs. Pseudochylothorax: Cholesterol Crystal Finding in Pleural Effusion.

Chylothorax and pseudochylothorax are both rare forms of pleural effusion characterized by a milky appearance of the pleural fluid. Despite their similarities, they have distinct etiologies, clinical implications, and medical management strategies. Pleural fluid analysis of cholesterol and triglyceride levels is crucial to differentiate between them, establish a proper diagnosis, and guide clinical evaluation and treatment. This case report describes an uncommon presentation of pseudochylothorax associated with long-standing rheumatoid arthritis (RA) in a patient with no prior history of pleur…
