Parents have traditionally relied on health care professionals for advice and treatment for most orthopaedic conditions, including clubfoot. In a retrospective study we analysed the effect of caudal epidural anesthesia combined with general anesthesia on requirement for intraoperative narcotics and postoperative analgesics in children undergoing surgical release of congenital clubfoot. In 20 children (mean age at day of surgery 4,7 months) general anesthesia was supplemented by caudal epidural anesthesia, controls (20 children, mean age 4,3 months) received general anesthesia only. A procedure for soft tissue release through double incisions in clubfoot is described. It has been practiced in 287 cases of severe, neglected, resistant, and relapsed clubfeet in the last 7 years. Generally, it has been found to be a useful approach without risking any skin necrosis.

Between 1977 and 1989, 28 clubfeet were operated on, with follow-up ranging from 2 to 13 1/2 years and averaging 79 months. Open biopsies were carried out on the muscles of 23 clubfoot patients, ages 9 months to 4 years. Electron microscopic analyses of the so-called "clubfoot muscles" and the peroneal muscles were performed. The changes found were not present in every area of the muscles, but were surrounded by fields of normal structure. Fatty degeneration with fibrosis was observed as the consequence of immobilization. Tibiofibular torsion was measured in normal and clubfoot populations. A gradual increase, up to skeletal maturity, was demonstrated in normal legs. Lateral tibiofibular torsion was reduced in patients at the completion of clubfoot treatment, and it was concluded that derotation braces did not influence this result.

Fifty-four patients with congenital clubfoot (total: 82 club feet) were observed at the 2nd Department of Orthopaedic Surgery of Rome University from 1970 to 1980.
