Lipid Secretion by Parasitic Cells of Coccidioides Contributes to Disseminated Disease.

Coccidioides is a soil-borne fungal pathogen and causative agent of a human respiratory disease (coccidioidomycosis) endemic to semi-desert regions of southwestern United States, Mexico, Central and South America. Aerosolized arthroconidia inhaled by the mammalian host first undergo conversion to large parasitic cells (spherules, 80-100 μm diameter) followed by endosporulation, a process by which the contents of spherules give rise to multiple endospores. The latter are released upon rupture of the maternal spherules and establish new foci of lung infection. A novel feature of spherule maturat…

Earthquake-Ridden Area in USA Contains Coccidioides, the Valley Fever Pathogen.

Early July 2019, two major earthquakes occurred in the Mojave Desert of California near the city of Ridgecrest and the community of Trona and generated a large dust plume that lingered for days. The earthquakes hit an area endemic for Coccidioides, a soil-borne fungal pathogen that can become airborne when soil is disturbed and typically manifests as a pulmonary disease when inhaled. This study is the first to confirm the presence of Coccidioides in soils near Trona using a nested polymerase chain reaction (PCR) approach. First responders to earthquake events, the public, and physicians in the…

Laboratory exposure to Coccidioides: lessons learnt in a non-endemic country.

Coccidioides is a primary pathogenic fungus, which infects humans through highly infectious arthroconidia, causing substantial morbidity including life-threatening disseminated infections. Due to the low infectious dose, laboratory personnel might become infected during diagnostic procedures. Accordingly, coccidioidomycosis is reported as the most frequent laboratory-acquired systemic mycosis worldwide. This risk is aggravated in non-endemic countries, where the diagnosis may not be suspected. We report on an inadvertent exposure of 44 persons to Coccidioides posadasii in a clinical microbiolo…

Rational Design of T Lymphocyte Epitope-Based Vaccines Against Coccidioides Infection.

Coccidioidomycosis is a potentially life-threatening mycosis endemic to the Southwestern USA and some arid regions of Central and South America. A vaccine against Coccidioides infection would benefit over 30-million people who reside in or visit the endemic regions. Vaccine candidates against systemic fungal infections come in many forms. Live attenuated vaccines are derived from disease-causing pathogens and generally stimulate excellent protective immunity. Since attenuated vaccines contain living microbes, there is a degree of unpredictability raising concerns regarding safety and stability…

Detection of Coccidioides immitis in Kern County, California, by multiplex PCR.

Coccidioides immitis is a fungal human pathogen endemic to semiarid soils in southern California and Baja California (Mexico). Results of culture-dependent detection of C. immitis in the past indicated a spotty distribution and unreliable prediction of C. immitis growth sites and accumulation sites. In this project we investigated bulk soil samples for the presence of the pathogen in nonagricultural loamy soils at nine different locations around Bakersfield, Kern County, California, for almost 2 y (2008-2009). To detect the pathogen we used a multiplex PCR method with optimized soil handling a…

Coccidioides immitis fungemia: clinical features and survival in 33 adult patients.

Coccidioides immitis is a fungus endemic to the southwestern United States. Susceptible hosts, including blacks, Hispanics, Filipinos, Native Americans, and those with compromised immunity, may develop disseminated disease, including fungemia. We retrospectively reviewed the records of all patients (n = 33) with Coccidioides immitis fungemia (CIF) at a 550-bed public hospital in Phoenix, Arizona, from 1990 to 2002. This is the largest reported series of CIF. The purpose of the study was to review the incidence, signs, symptoms, and outcomes of CIF. Twenty-nine patients had human immunodeficien…

Phenotypic characterization and ecological features of Coccidioides spp. from Northeast Brazil.

This study extends phenotypic and ecological knowledge of Coccidioides spp., by describing its recovery from soils of Ceará State (Northeast Brazil) and analyzing the in vitro features of the growth of its vegetative phase. Following a human coccidioidomycosis case, Coccidioides spp. strains were isolated from 3 of 14 soil samples collected in an armadillo's burrow. Mycological analysis showed colonies with glabrous, velvety or cottony texture and an increasing quantity of arthroconidia. The overall growth rates of the strains were slower in 8% NaCl medium, maximum growth rate was obtained at …

Induction of tumor necrosis factor alpha by spherules of Coccidioides immitis.

The cytokine tumor necrosis factor alpha (TNF-alpha) functions as an immunomodulatory protein and as a mediator of cachexia. We report that viable or Formalin-killed spherules of Coccidioides immitis induced the secretion of TNF-alpha by peritoneal-exudate cells from BALB/c mice. The identification of the cytokine as TNF-alpha was based on its lytic activity against the TNF-alpha-sensitive LS murine fibrosarcoma cell line but not the TNF-alpha-resistant LR cell line, its neutralization by rabbit anti-TNF-alpha, and its secretion by peritoneal cells having characteristics of macrophages. The in…

Scanning electron microscope studies on the parasitic cycle of Coccidioides immitis.

The process involved in the in vivo conversion of the arthroconidia of Coccidioides immitis into endosporulating spherules was studied with the aid of a scanning electron microscope. By the fifth and sixth day after inoculation of laboratory mice, complete conversion had occurred in their kidneys, lungs, and spleens. The progressive stages of cleavage that occurred in the enlarged arthroconidia were initiated by invagination at several points of the cytoplasmic membrane that covers the inner surface of the developing spherule's cytoplasm was divided into progressive smaller segments. These seg…

Inbred mouse strains differ in resistance to lethal Coccidioides immitis infection.

Inbred strains of mice were infected intraperitoneally with Coccidioides immitis, and the mean lethal dose was determined after 28 days. DBA/2N mice had a mean lethal dose of greater than 10(5) arthroconidia, whereas BALB/cAnN, C57BL/6N, and C57L/J mice had a mean lethal dose of less than or equal to 10(3). Since both BALB/c and DBA/2 mice are the H-2d haplotype, resistance is not primarily determined by the major histocompatibility locus. Resistance was the dominant phenotype. The pattern of C. immitis-resistant strains does not correspond to the strain distribution of the lsh gene or to the …

Induction of cellular immunity to Coccidioides immitis after sensitization with dinitrochlorobenzene.

A patient had progressive disseminated coccidioidomycosis and depressed cellular immunity to Coccidioides immitis. He developed a large nasal coccidioidal lesion that was unresponsive to conventional therapy. Application of dinitrochlorobenzene (DNCB) to the nasal lesion was temporally associated with resolution of the lesion and stabilization of the systemic disease. In addition, DNCB application was followed by signs of cellular immunity to C. immitis. These included development of delayed cutaneous hypersensitivity to coccidioidin, as well as lymphokine production and lymphoproliferative re…

Tempest from Tehachapi takes toll or Coccidioides conveyed aloft and afar.

New cases of acute primary coccidioidomycosis in large numbers resulted from a windstorm that blew through Kern County, California, on December 20, 1977. In most of these cases clinically apparent infections developed in early and mid-January 1978 and occurred not only in persons exposed directly to the dust raised by the windstorm but also among those in many areas to the north and west of Kern County. The exposure to the dustborne Coccidioides immitis was brief because of the arrival of drenching rains, but the continued heavy rainfall increased the potential for large numbers of cases of co…

Coccidioidal meningitis and neurosyphilis co-infection in a non-HIV patient.

Coccidioides sp. and Treponema pallidum can both cause infections of the central nervous system if untreated. We describe a case of an immunocompetent patient living in an endemic region for Coccidioides who presented with headaches and diplopia and was found to have co-infection of coccidioidal meningitis and neurosyphilis. We highlight the importance of evaluation for CNS co-infections as they may be underdiagnosed, especially in endemic areas for coccidioidomycosis.

Peripheral blood eosinophilia as a clue to the diagnosis of an occult Coccidioides infection.

A marked peripheral blood eosinophilia is an uncommon finding in a complete blood count (CBC). According to Wardlaw and Kay (Eosinophils and … of profound peripheral blood eosinophilia. The utility of Coccidioides polymerase chain reaction testing in the clinical setting.

Coccidioidomycosis, the fungal infection caused by dimorphic Coccidioides sp., is typically diagnosed by histopathologic identification of spherules in affected secretions and tissues or by culture. These tests are reliable but time-intensive, delaying diagnosis and treatment. To evaluate a polymerase chain reaction (PCR) test developed to detect Coccidioides sp. in clinical specimens, we conducted a retrospective chart review of all patients (N = 145) who underwent Coccidioides PCR at our institution between April 27, 2007, and May 6, 2008, abstracting clinical, microbiologic, serologic, radi…

Coccidioidal meningitis: update on epidemiology, clinical features, diagnosis, and management.

Coccidioidomycosis is a fungal infection endemic to geographically restricted areas of the southwestern United States, and its incidence is increasing within endemic areas. Coccidioidal meningitis, an infrequent but very severe form of coccidioidomycosis, is associated with significant mortality and morbidity. The diagnosis depends on the demonstration of coccidioidal antibody or positive culture, histopathology, or cytology in the presence of typical abnormalities of the cerebrospinal fluid. Central nervous system complications of this infection may include hydrocephalus, vasculitis, infarcti
