Integrating orthotic and prosthetic solutions into disaster risk management: a multimethod study protocol.

INTRODUCTION: Disasters often cause large numbers of injuries, including fractures, lacerations and amputations, which overwhelm emergency services. Orthotic and prosthetic solutions can provide timely primary care, reduce the need for surgery and prevent complications. A structured framework for preparedness and response is required to integrate these solutions into disaster risk management. This study aims to develop such a framework to enhance rehabilitation and reduce secondary injuries. METHODS AND ANALYSIS: This multimethod protocol comprises three phases. First, a systematic scoping re

Effect of a strength-based empowerment programme, using teach-back method, on caregiver burden and resilience among family caregivers of patients undergoing haemodialysis: a protocol for a clinical trial study.

INTRODUCTION: Family caregivers of patients undergoing haemodialysis experience significant psychological, social and physical challenges that contribute to caregiver burden and reduced resilience. Strengths-based empowerment programmes, combined with the teach-back method, may help reduce burden and enhance resilience. This protocol describes a randomised controlled trial designed to evaluate the effect of such an intervention among family caregivers of haemodialysis patients. METHODS AND ANALYSIS: This parallel-group randomised controlled trial will enrol 78 family caregivers of haemodialys

Study protocol of a phase 2, randomised, placebo-controlled, double-blind, adaptive, parallel group clinical study to evaluate the efficacy and safety of recombinant alpha-1-microglobulin in subjects at high risk for acute kidney injury following open-chest cardiac surgery (AKITA trial).

INTRODUCTION: Acute kidney injury (AKI) is a common complication after cardiac surgery (CS) and is associated with adverse short-term and long-term outcomes. Alpha-1-microglobulin (A1M) is a circulating glycoprotein with antioxidant, heme binding and mitochondrial-protective mechanisms. RMC-035 is a modified, more soluble, variant of A1M and has been proposed as a novel targeted therapeutic protein to prevent CS-associated AKI (CS-AKI). RMC-035 was considered safe and generally well tolerated when evaluated in four clinical phase 1 studies. METHODS AND ANALYSIS: This is a phase 2, randomised,

AI-Empowered Nuclear Medicine Education, Part 2: Practical AI Applications for Educators.

Nuclear medicine (NM) educators face a pivotal time of rapid scientific innovation and expansion of theranostic possibilities, rising volumes, staffing shortages, and a call for more high-quality training. In the midst of this expansion, AI technologies have disrupted nearly every aspect of society, including education. Although data on higher-level educational outcomes and patient-specific outcomes are limited, the rapid acceleration and sophistication of AI capabilities is undeniable and ubiquitous, causing many educators to express feelings of uncertainty, incompetence, distrust, and fear.

Relationship between vaginal and gut microbiome and pregnancy outcomes in eastern Ethiopia: a protocol for a longitudinal maternal-infant cohort study (the EthiOMICS study).

INTRODUCTION: Although evidence exists on the impact of microbiota on pregnancy outcomes in many high-resource settings, there is a lack of research in many low-resource settings like Ethiopia. This study aims to fill this gap by studying the gut and vaginal microbiota changes throughout pregnancy and assess how these changes relate to pregnancy outcomes among a cohort of pregnant women in eastern Ethiopia. METHODS AND ANALYSIS: Vaginal and stool samples will be collected using DNA/RNA Shield Collection kits three times starting at 12-22 weeks, 28-36 weeks and at birth (within 7 days). Postna

Prevalence and predisposing factors of retained drug needle fragments in people who inject drugs: a protocol for a preliminary cross-sectional multicentre observational study in Finland.

INTRODUCTION: Numerous published case reports have described retained drug needle fragments in soft tissue as causes of localised pain and infections in persons who inject drugs (PWIDs). Furthermore, there are case reports of PWIDs with lung and heart embolisations caused by needle emboli. Subcutaneously retained needles also pose a risk of needlestick injury to medical staff. There are no previous epidemiological attempts to evaluate how common X-ray-confirmed retained drug needle fragments are among community-dwelling PWIDs. Due to the unclear clinical relevance of needle fragment retentions

Complementary vaccination protocol with dendritic cells pulsed with autologous tumour lysate in patients with resected stage III or IV melanoma: protocol for a phase II randomised trial (ACDC Adjuvant Trial).

INTRODUCTION: Surgery is one of the treatments of choice for patients with a single metastasis from melanoma but is rarely curative. Such patients could potentially benefit from consolidation immunotherapy. Vaccination with dendritic cells (DCs) loaded with tumour antigens elicits a tumour-specific immune response. In our experience, patients who developed delayed type hypersensitivity (DTH) after DC vaccination showed a median overall survival (OS) of 22.9 monthsvs4.8 months for DTH-negative cases. A phase II randomised trial showed an advantage OS of a DC vaccine over a tumour cell-based vac

From Archetypes to Access: A Two-Axis Governance Framework and Decision-Support Tool for Rare Specimen Biobanking.

INTRODUCTION: Biobanks fulfill their scientific and ethical mandates when collected specimens are used effectively. Access policies govern researcher eligibility, allocation, oversight, and intellectual property (IP) considerations. Despite their importance, access policies vary substantially and lack a unifying practical framework. OBJECTIVES: We sought to identify commonalities and areas of under-specification across access policies in surgically oriented, pediatric, and/or national biobanks to describe real-world practices and develop a typology of rare biospecimen governance. METHODS: We

Randomised double-blind phase 3 clinical study testing impact of atorvastatin on prostate cancer progression after initiation of androgen deprivation therapy: study protocol.

INTRODUCTION: Blood cholesterol is likely a risk factor for prostate cancer prognosis and use of statins is associated with lowered risk of prostate cancer recurrence and progression. Furthermore, use of statins has been associated with prolonged time before development of castration resistance (CR) during androgen deprivation therapy (ADT) for prostate cancer. However, the efficacy of statins on delaying castration-resistance has not been tested in a randomised placebo-controlled setting. This study aims to test statins' efficacy compared to placebo in delaying development of CR during ADT tre

Healthcare systems and motivation.

Despite the fact that most American physicians, at least until around the 1970s, stood in the way of developing a universal healthcare syste … universal healthcare -- to the point of having included such language in their various professional codes of ethics (which, perversely enough, bioethicists as a group have failed to do). Contrary to the claims of our colleagues, Altom and Churchill, physicians seem to be genuinely frustrated as to what else they can do to change the current inequitable system.

Leveraging large language models for patient-ventilator asynchrony detection.

OBJECTIVES: The objective of this study is to evaluate whether large language models (LLMs) can achieve performance comparable to expert-developed deep neural networks in detecting flow starvation (FS) asynchronies during mechanical ventilation. METHODS: Popular LLMs (GPT-4, Claude-3.5, Gemini-1.5, DeepSeek-R1) were tested on a dataset of 6500 airway pressure cycles from 28 patients, classifying breaths into three FS categories. They were also tasked with generating executable code for one-dimensional convolutional neural network (CNN-1D) and Long Short-Term Memory networks. Model performance

Response Interruption and Redirection for Stereotypy: A Quality Review and Ethical Considerations.

Response interruption and redirection (RIRD) is a commonly evaluated intervention for reducing stereotypy, a core feature of autism spectrum disorder (ASD). Although RIRD has been recognized as an evidence-based practice, increasing emphasis on neurodiversity affirming and contextually grounded intervention has highlighted the need to reexamine both the methodological rigor of the literature and the conditions under which RIRD is appropriate. This systematic review evaluated the quality and ethical contextualization of published RIRD studies by applying the What Works Clearinghouse (WWC) singl

Mandatory reporting of child sexual exploitation online: A qualitative study of school staff perspectives in Hong Kong.

BACKGROUND: Child sexual exploitation online (CSEO) poses major challenges for child protection, as abuse can occur through hidden, fragmented, or ambiguous digital interactions. Although mandatory reporting (MR) of child abuse has been widely studied, its feasibility for CSEO in schools remains underexplored. OBJECTIVE: We examined school staff's perceptions of the feasibility of MR for suspected CSEO in Hong Kong, focusing on facilitators, barriers, and ethical dilemmas. METHODS: Using purposive sampling, we recruited 80 staff members from 20 primary and secondary schools, including princi

Epigenetic reprogramming as a therapeutic strategy for neurodegenerative diseases: A complex and novel approach.

Epigenetic reprogramming has emerged as a promising therapeutic strategy for neurodegenerative diseases by targeting reversible gene expression changes without altering the underlying DNA sequence. This review explores key epigenetic mechanisms, including DNA methylation, histone modifications, non-coding RNAs, and chromatin remodeling, that contribute to neurodegeneration in Alzheimer's disease (AD), Parkinson's disease (PD), and Huntington's disease (HD). Pharmacological interventions such as DNA methylation modulators, histone deacetylase (HDAC) inhibitors, and non-coding RNA therapies have
