[Secondary repair of the extensor pollicis longus].

Repair of extensor pollicis longus, even when it is imperfect, generally gives fairly good results. This tendon is exposed to injury particularly as it leaves the pulley around Lister's tubercle and especially after a slightly displaced Pouteau-Colles fracture. Very marked retraction of the proximal part of the tendon is almost constant after three weeks and repair by secondary suture is rarely possible. For this reason, a palmaris longus graft or transfer of the intrinsic extensor of the index finger is generally required. The analysis of the results of 23 cases shows that, in order to obtain

Traumatic rupture of the extensor pollicis longus tendon.

Rupture of the extensor pollicis longus tendon is rather rare, attributable in about half of all cases to rheumatoid arthritis affecting the wrist or to a Colles' fracture. In the remainder of cases, either a direct closed injury to the wrist or a hyperextension injury of the wrist is most often responsible. Far more unusual is a rotational injury which can cause complete avulsion of the EPL tendon at the musculotendinous junction. A delayed rupture is most probably caused by an avascular necrosis secondary to traumatic disruption of the mesotendon. Surgical correction of the injury is best ac…

Augmented external fixation of distal radius fractures: a biomechanical analysis.

An in vitro model of an unstable extra-articular distal radius fracture was created in 8 fresh-frozen cadaveric specimens and stabilized with an external fixator. Rotation and translation kinematics of the distal radial fracture fragment were measured in relation to the proximal radius during physiologic loading, using infrared light-emitting diodes and a 3-dimensional motion-sensing device. The effect of supplemental single and combination Kirschner wire (K-wire) fixation on fracture fragment stability was assessed. Fixation of supplemental K-wires to the fixator frame via a custom-developed

[Treatment of extra-articular malunited fractures of the distal radius using osteotomy with interposition of a graft].

Malunited fractures of the distal end of the radius are corrected by an osteotomy transversely oriented at the level of the angulation (15 cases reported). After correction of the deformation (length and angulation) compact iliac bone graft is inserted. 5 malunited fractures with a volar deformation were corrected by a palmar surgical approach and AO-T-plate fixation. 8 cases with a dorsal displacement were corrected by a dorso-lateral surgical approach and fixed with K-wires (7 cases) and without any internal fixation (1 case). 2 cases with dorsal displacement were corrected by a palmar appro

[Method of functional treatment of distal radius fractures].

PURPOSE OF THE STUDY To present the method of functional treatment of distal radius fractures, and to evaluate its results in a group of 57 patients. MATERIAL The method described here was used to treat a Colles fracture in 57 patients between 2001 and 2005. The average age of patients at the time of injury was 62 years (range, 19 to 82). The minimal interval between the end of treatment and the evaluation of the patient group was one year. METHODS The presented method is based on the principles of functional treatment of wrist fractures, as advocated by Sarmiento et al. When the patient has b

[External fixation of unstable wrist fractures].

The results of external fixation of 66 unstable distal radial fractures were evaluated. In the majority, the radiological results were excellent or good. There was a clear correlation between an excellent or good clinical score and the subsequent radiological result. Upper limb dystrophy which caused a considerable morbidity was found in every third patient. Unstable distal radial fractures with severe initial displacement predispose to reflex dystrophy. External fixation obviously cannot prevent this severe complication, it is even possible that tension on soft tissues, capsules and ligaments

A hypothesis: the causes of hip fractures.

Neither age-related osteoporosis nor the increasing incidence of falls with age sufficiently explain the exponential increase in the incidence of hip fracture with aging. We propose that four conditions must be satisfied in order for a fall to cause a hip fracture: (a) the faller must be oriented to impact near the hip; (b) protective responses must fail; (c) local soft tissues must absorb less energy than necessary to prevent fracture, and (d) the residual energy of the fall applied to the proximal femur must exceed its strength. All of these events become more likely with aging and lead to a

[Py-Desmanet pinning in distal radius fractures].

UNLABELLED: Internal fixation by pinning is one of the most used methods of surgical treatment in fractures of the distal extremity of the r … s fractures (3 women and 2 men) about 56 years of average age, all with good-quality bone, all with Colles' fracture pattern (2-4 cm above the radio-carpal articulation, no articular involvement, posterior displacement of the distal fragment). The reduction of the fractures was achieved by closed manipulation and controlled intraoperatively with the C-arm. We pref …

Load-displacement behavior in a distal radial fracture model. The effect of simulated healing on motion.

External fixation of fractures of the distal end of the radius neutralizes external forces and maintains axial alignment during healing. As far as we know, there have been no biomechanical studies of the effects of early removal of the fixator in a partially healed fracture model. The purpose of the present study was to observe the load-displacement behavior of a distal radial fracture model in which we had simulated partial healing by injection of butyl-rubber caulk and augmented this simulated healing with Kirschner-wire fixation. Sixteen fresh-frozen hand-wrist-forearm specimens from cadave

Augmentation of distal radius fracture fixation with coralline hydroxyapatite bone graft substitute.

We implanted coralline hydroxyapatite bone graft as a substitute for autogenous bone graft to support the reduced articular surface of 21 consecutive patients with distal radius fractures treated with external fixation and K-wires. The purpose of this single-cohort retrospective study was to report the outcomes of treatment with this material, complications associated with its use, and its efficacy in supporting the articular surface reduction. Eighteen patients were available for independent evaluation of motion, subjective outcome analysis, and final radiographic analysis at an average of 35

The relationship between osteoarthritis of the hands, bone mineral density, and osteoporotic fractures in elderly women.

To study the relationship between osteoarthritis (OA) and osteoporosis (OP), radiographic osteoarthritis lesions of the hands (HOA) were quantified in 300 healthy women, aged 75 years or more, as a subgroup of a cohort originally recruited for a multi-centre study of risk factors for femoral neck fracture. The HOA combined score (i.e. the sum of the grades of joint-space narrowing, osteophytes, erosions and joint misalignment), the osteophytosis score and the joint-space narrowing score were calculated on a radiograph of both hands. Bone mineral density (BMD) was measured using dual-energy X-r

The epidemiology of fragility fractures: is there a role for bone quality?

Fragility fractures, particularly those of the hip, vertebrae, and distal forearm, constitute a major public health problem. The two ultimate determinants of fracture are bone strength and propensity to trauma. Bone strength depends not only upon bone mass but also upon a variety of qualitative aspects of bone structure. These include its architecture, the amount of fatigue damage it has sustained, and changes in its bulk material properties, indices that are collectively subsumed into the term "bone quality." Fragility fractures show differences in their patterns of incidence by age, sex, eth

Low-intensity pulsed ultrasound treatment as an alternative to vascular bone graft surgery for a 5-year-long ulnar non-union in a patient with haemochromatosis.

We present the case of a 75-year-old woman with haemochromatosis who developed a 5-year-long right ulnar non-union after a shortening osteotomy to correct a malunited Colles' fracture. Standard surgical treatment for ulnar non-unions was attempted on 19 March 2008 and again on 20 April 2009, but the non-union persisted on 8 May 2012, as confirmed by CT scan. Vascular bone grafting and refixation was then considered, but the patient declined this extensive operation, instead choosing to try non-invasive low-intensity pulsed ultrasound treatment with an Exogen device. Just 4 months later, comple…

The results of examinations of 14 patients with incidentally diagnosed Kienböck's disease were reviewed. These patients had not been examined previously, clinically or radiographically, and had not received any treatment for their condition. For 12 of these patients, Kienböck's disease was diagnosed incidentally on radiographic examinations obtained for other reasons, including carpal tunnel syndrome in four patients, Colles' fracture in three patients, pseudogout attack in the wrist in two patients, infection of the hand in one patient, osteoarthritis of the carpometacarpal joint of the thumb…

Reduction techniques in distal radius fractures.

External skeletal fixation is an important minimal invasive procedure in the management of fractures at the distal end of the radius. Attention to detail is important not only in the recognition of indications and function of the external fixator but also in its specific application. To improve anatomical restoration (e.g. palmar tilt) multi-planar ligamentotaxis is recommended. The demonstrated dynamic fixator is easy to apply and the double ball joint facilitates reduction after mounting by multi-planar ligamentotaxis. This transarticular unilateral external fixation system permits restorati

Forearm fracture.

Bone mineral density of the distal forearm decreases with age in women, and this results in an increased risk of forearm fractures. Men have no increase in risk of forearm fractures with age because they have a higher peak bone mass at this site and less of a decrease in bone mineral density with ageing. Bone loss from the forearm slows in women after the age of 65 years in women and this may account for the plateau in forearm fracture rate after this age. Women with forearm fractures have twice the expected risk of vertebral and hip fractures. Forearm fractures before the age of 60 years are

Dynamic versus static external fixation for unstable distal radius fractures: an up-to-date meta-analysis.

OBJECTS: Whether dynamic or static external fixation is more appropriate for distal radius fractures is still being debated, our aim is to determine the effect of dynamic versus static external fixation for unstable distal radius fractures in terms of postoperative complication, clinical results and radiological outcomes. METHODS: We selected PubMed, Cochrane Library, EMBASE, BIOSIS, Ovid and the relevant English orthopaedic journals and pooled data from eligible trials including six eligible randomised controlled trials and two comparative studies containing 998 patients comparing dynamic an

External fixation versus conservative treatment for distal radial fractures in adults.

BACKGROUND: Fracture of the distal radius ('broken wrist') is a common clinical problem. It can be treated conservatively, usually involving wrist immobilisation in a plaster cast, or surgically. A key method of surgical fixation is external fixation. OBJECTIVES: To evaluate the evidence from randomised controlled trials comparing external fixation with conservative treatment for fractures of the distal radius in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (September 2006), the Cochrane Central Register of Controlled Trials, MEDL

[Avulsion fractures of the distal part of the radius with anterior displacement in children].

The authors report 7 cases of fractures of the distal part of the radius, with a frontal displacement with children. This fracture is rare in pediatric traumatology. There is no particular type of mechanism that will provoke that fracture; in five cases the child was falling down on the palm of his hand and in two cases in a forced flexion. The problem about this fracture is the stability of the reduction. A proper function of the wrist is impossible with an imperfect result. The orthopedic treatment is often unable to stabilize the fracture. The authors need a surgical management using a plat
