Incidence of primary cancers and intracranial tumour recurrences in GH-treated and untreated adult hypopituitary patients: analyses from the Hypopituitary Control and Complications Study.

OBJECTIVE: Speculation remains that GH treatment is associated with increased neoplasia risk. Studies in GH-treated childhood cancer survivors suggested higher rates of second neoplasms, while cancer risk data for GH-treated and untreated hypopituitary adults have been variable. We present primary cancer risk data from the Hypopituitary Control and Complications Study (HypoCCS) with a focus on specific cancers, and assessment of recurrence rates for pituitary adenomas (PA) and craniopharyngiomas (CP). DESIGN: Incident neoplasms during HypoCCS were evaluated in 8418 GH-treated vs 1268 untreate

A rare case of primary choriocarcinoma in the sigmoid colon.

Primary colorectal choriocarcinoma is an extremely rare neoplasm and is usually associated with a poor prognosis. Only 13 cases of colorectal choriocarcinoma have previously been reported. There is no standard chemotherapeutic regimen for this tumor type. A 68-year-old man presented with melena and was diagnosed with sigmoid colonic adenocarcinoma with multiple liver metastases. He underwent a laparoscopic sigmoidectomy. Pathology revealed choriocarcinoma with a focal component of moderately differentiated adenocarcinoma of colon origin. Based on the collagen gel droplet-embedded culture drug

Special AT-rich sequence-binding protein 2-positive primary small cell neuroendocrine carcinoma of the urinary bladder: A diagnostic pitfall.

Special AT-rich sequence-binding protein 2 (SATB2) expression in conjunction with CK20 expression has been established as a specific marker for the detection of colorectal adenocarcinomas. Recently, its expression in foregut, midgut, and hindgut neuroendocrine neoplasms has been documented. SATB2 expression has been documented in neuroendocrine neoplasms from various primary sites; however, its expression in the urinary bladder has not been reported. A case of primary small cell neuroendocrine carcinoma of the urinary bladder (SmCCB) is presented, where strong SATB2 expression led to the suspi

Derivation and Validation of a Scoring System to Stratify Risk for Advanced Colorectal Neoplasia in Asymptomatic Adults: A Cross-sectional Study.

BACKGROUND: Several methods are recommended equally strongly for colorectal cancer screening in average-risk persons. Risk stratification would enable tailoring of screening within this group, with less invasive tests (sigmoidoscopy or occult blood tests) for lower-risk persons and colonoscopy for higher-risk persons. OBJECTIVE: To create a risk index for advanced neoplasia (colorectal cancer and adenomas or serrated polyps ≥1.0 cm, villous histology, or high-grade dysplasia) anywhere in the colorectum, using the most common risk factors for colorectal neoplasia. DESIGN: Cross-sectional stud

Incidentally Detected Low-Grade Appendiceal Mucinous Neoplasm Following Laparoscopic Appendectomy for Acute Appendicitis.

Appendiceal mucinous neoplasms are rare entities that account for a small proportion of all gastrointestinal malignancies and are most often discovered incidentally on histopathological examination of appendectomy specimens. We present the case of a 36-year-old woman who presented to the emergency department with acute right lower quadrant abdominal pain and was found to have acute appendicitis on computed tomography imaging. She underwent an uncomplicated laparoscopic appendectomy, and final pathology revealed a low-grade appendiceal mucinous neoplasm (LAMN) confined to the appendix, staged a

Mucinous adenocarcinoma of the appendix with inguinal node metastases.

Mucinous adenocarcinoma of the appendix is a rare neoplasm with a low propensity for lymph node metastasis. The present case refers to an appendicular mucinous adenocarcinoma with inguinal lymph node metastasis. A 71-year-old woman underwent an appendectomy due to a clinical presentation of acute appendicitis. However, the histological examination of the surgical specimen revealed a mucinous adenocarcinoma of the appendix. After staging, the patient underwent a right hemicolectomy and was proposed for adjuvant chemotherapy. At the 3rd year of follow-up, inguinal lymphadenopathy was diagnosed,

Identification of Extracolonic Pathologies by Computed Tomographic Colonography in Colorectal Cancer Symptomatic Patients.

BACKGROUND & AIMS: Symptoms suggestive of colorectal cancer may originate outside the colorectum. Computed tomographic colonography (CTC) is used to examine the colorectum and abdominopelvic organs simultaneously. We performed a prospective randomized controlled trial to quantify the frequency, nature, and consequences of extracolonic findings. METHODS: We studied 5384 patients from 21 UK National Health Service hospitals referred by their family doctor for the investigation of colorectal cancer symptoms from March 2004 through December 2007. The patients were assigned randomly to groups that

Potential role of a novel endoscopic retrieval bag for large colorectal resected specimen: a proof-of-concept study.

Endoscopic submucosal dissection enables en bloc resection of large superficial colorectal neoplasms. However, it is sometimes challenging to retrieve a large resected specimen via the anus without sample fragmentation. A novel "bag-type" retrieval device has been developed to accomplish complete isolation and non-destructive delivery of oversized specimens. This single-center retrospective study was performed to demonstrate the efficacy of this device for large colorectal resected specimens. Among 17 patients, we identified 18 superficial colorectal lesions for which the use of a novel retrie…

Left Perforated Colonic Tumor with Synchronous Locally Advanced Right Colonic Tumor.

Colorectal cancer is one of the most common malignant neoplasms worldwide. Overall mortality is 33%. Synchronous colorectal cancer refers to more than one malignant tumor detected in different segments of the colon, simultaneously or within 6 months of initial diagnosis. The development of colorectal cancer is a multistep process that originates with a genetic mutation leading to a malignant phenotype and generating a growth advantage. Colorectal cancer presents up to 16% of hypermutations, of which 75% are characterized by microsatellite instability which in turn leads to poorer cell differen

Familial colorectal cancer: pathology and molecular characteristics.

Appropriate management of familial colorectal cancer revolves around the diagnosis of the underlying genetic syndrome. This necessitates an interdisciplinary approach allowing integration of clinical, morphological, and molecular evidence that may involve several members of the same family. Genetic disorders express themselves over time, whereas clinical investigation of family members is likely to be episodic. Generic features of hereditary colorectal cancer syndromes include a positive family history, early age at onset, multiple neoplasms, and extracolonic lesions of either a developmental

The Funen Adenoma Follow-Up Study. Characteristics of patients and initial adenomas in relation to severe dysplasia.

Between 1978 and 1992 a total of 1689 colorectal adenomas were removed in 1042 patients with no history of previous colorectal neoplasms. One hundred and eighteen patients had at least one adenoma with severe dysplasia. A 'clean colon' was ensured by total colonoscopy in 97% of the patients. A multiple logistic model was used to assess the independent risk factors associated with severe dysplasia. The size of largest adenoma and the extent of villous component were identified as independent patient risk factors associated with high risk of severe dysplasia. The results indicated that the risk …

Endoscopic Full-Thickness Resection for Gastrointestinal Subepithelial and Epithelial Neoplasia: Current Status, Indications, and Technical and Oncological Challenges-A Multispecialty Perspective.

Endoscopic full-thickness resection (EFTR) has emerged as a minimally invasive treatment for small gastrointestinal subepithelial neoplasms (SETs). In particular, the development of novel closure methods such as clip-line and various endoscopic suturing techniques has enabled the secure closure of any resultant defects. During the Advanced Therapeutic Endoscopy session at the Endoscopic Forum Japan 2025, we summarized the current status of EFTR for SETs and discussed future clinical applications for epithelial neoplasms from oncological and technical perspectives. Although predominantly docume

Concomitant carcinoid tumor and papillary urothelial neoplasm of low malignant potential in a patient with Crohn's disease: a case report.

Patients with a long-term history of inflammatory bowel diseases show an increased risk of developing colorectal cancers. In the case of Crohn's disease, such an increased risk is also associated with other malignant tumors. The report presents a very rare case of a concomitant carcinoid tumor and papillary urothelial neoplasm of low malignant potential in a 35-year-old patient with diagnosed Crohn's disease complicated by an enterovesical fistula. The carcinoid tumor and papillary urothelial neoplasm of low malignant potential were diagnosed by postoperative histopathology. In patients with C

Duodenal mantle cell lymphoma in a patient with advanced sigmoid adenocarcinoma.

While colorectal cancer is one of the most commonly encountered gastrointestinal malignancies, primary lymphoma is an uncommon neoplasm of the gastrointestinal tract. The duodenum is not a common location for the recurrence of colon cancer or of primary gastrointestinal lymphoma. Studies regarding mantle cell lymphoma (MCL) as a secondary synchronous or metachronous malignancy are limited. Here we report a patient who received concurrent chemoradiotherapy for his advanced sigmoid adenocarcinoma and developed MCL in the duodenal bulb 20 months later. Suspected lesions should be biopsied and exa
