Design and Rationale of the Home Blood Pressure Telemonitoring Linked with Community Health Workers to Improve Blood Pressure (LINKED-BP) Program.

BACKGROUND: Disparities in hypertension outcomes persist among Black and Hispanic adults and persons living in poverty in the United States. The "LINKED-BP Program" is a multi-level intervention linking home blood pressure (BP) monitoring with a mobile health application, support from community health workers (CHWs), and BP measurement training at primary care practices to improve BP. This study is part of the American Heart Association RESTORE (AddREssing Social Determinants TO pRevent hypErtension) Network. This study aims to examine the effect of the LINKED-BP Program on BP reduction and to…

Knowledge and practices surrounding outbreaks and COVID-19 among community health workers in rural Rwanda: a cross-sectional mixed-methods study.

INTRODUCTION: Community Health Workers (CHW) are a critical resource for outbreak preparedness and response. However, CHWs´ ability to respond to outbreaks depends on their accurate knowledge of the disease and proper adoption of disease prevention practices. We explored knowledge and practices related to outbreaks in general, and COVID-19 among CHWs in Rwanda. METHODS: this cross-sectional multimethod study used stratified simple random sampling to recruit three cadres of CHWs (agents de santé maternelle, female Binomes, and male Binomes) from three rural Rwandan districts. We used telephone…

A randomised controlled provider-blinded trial of community health workers in sickle cell anaemia: effects on haematologic variables and hydroxyurea adherence.

Hydroxyurea (hydroxycarbamide) (HU) for sickle cell anaemia (SCA) is underutilised. Case management is an evidence-based health management strategy and in this regard patient navigators (PNs) may provide case management for SCA. We hypothesised that HU-eligible patients exposed to PNs would have improved indicators of starting HU and HU adherence. We randomised 224 HU-eligible SCA adults into the Start Healing in Patients with Hydroxyurea (SHIP-HU) Trial. All patients received care from trained physicians using standardised HU prescribing protocols. Patients in the Experimental arm received ca

Modification of an observational tool to evaluate how community health workers engage low-income latino parents in parent-child interaction therapy.

BACKGROUND: Caregiver engagement in behavioral parent training (BPT) interventions can have a significant impact on treatment outcomes and retention. Yet, low-income families of color often experience low rates of engagement in treatment due to structural and sociocultural barriers. BPT models partnering with community health workers (CHWs) have shown promise in addressing these barriers and increasing treatment engagement. The current study examines the engagement strategies utilized by CHWs when providing support to families receiving Parent-Child Interaction Therapy (PCIT + CHW) by identify…

Does near-vision correction improve digital data entry accuracy? A simulated within-subject crossover study among community health workers in Malawi.

INTRODUCTION: Presbyopia, an age-related loss of near vision, affects approximately 80% of adults over 40 and 100% of adults over 60. Although correctable with inexpensive reading glasses, fewer than 20% of those affected in low- and middle-income countries have access to appropriate correction. Uncorrected presbyopia impairs daily functioning and productivity. The impact of presbyopia on health workforce performance remains unexplored, despite the impact of data accuracy on patient safety and the increasing reliance on mobile data entry and digital tools. This study assesses whether near-visi

LEKSHIA-Sleep Disturbance Scale for Children: A simplified and culturally adapted pediatric sleep screening tool for use by community health workers.

BACKGROUND: Children's sleep disturbances are not well understood, especially in the community settings where access to specialized care is limited. Although thorough, the Sleep Disturbance Scale for Children (SDSC) might be too difficult for frontline healthcare providers to administer. The purpose of this study was to develop LEKSHIA-SDSC, a streamlined and culturally appropriate 10-item version of the SDSC, available in both English and Malayalam, and specifically designed for use by Indian community health workers. METHODOLOGY: A two-phase modification process was carried out building on …

Community-based health workers implementing universal access to HIV testing and treatment: lessons from South Africa and Zambia-HPTN 071 (PopART).

The global expansion of HIV testing, prevention and treatment services is necessary to achieve HIV epidemic control and promote individual and population health benefits for people living with HIV (PLHIV) in sub-Saharan Africa. Community-based health workers (CHWs) could play a key role in supporting implementation at scale. In the HPTN 071 (PopART) trial in Zambia and South Africa, a cadre of 737 study-specific CHWs, working closely with government-employed CHW, were deployed to deliver a 'universal' door-to-door HIV prevention package, including an annual offer of HIV testing and referral se

Monitoring, characterization and control of chronic, symptomatic malaria infections in rural Zambia through monthly household visits by paid community health workers.

BACKGROUND: Active, population-wide mass screening and treatment (MSAT) for chronic Plasmodium falciparum carriage to eliminate infectious reservoirs of malaria transmission have proven difficult to apply on large national scales through trained clinicians from central health authorities. METHODOLOGY: Fourteen population clusters of approximately 1,000 residents centred around health facilities (HF) in two rural Zambian districts were each provided with three modestly remunerated community health workers (CHWs) conducting active monthly household visits to screen and treat all consenting resi…

Examining Differences in Community Health Worker (CHW) Training Based on Organizational Experience With CHWs: A Call for CHW Workforce Sustainability.

As the COVID-19 crisis unfolded, community health workers (CHWs) in the United States were quickly identified as an essential workforce of health professionals that could reach community members who were at the highest risk for the negative effects of COVID-19. The reliance on CHWs during the COVID-19 pandemic has accelerated decades-long advocacy for sustainable funding for CHWs. Advocates argue, in part, that CHW programs must be up and running with trained CHWs and existing relationships with communities to most effectively respond to public health emergencies. Here we provide insight into …

Effect of COVID-19 pandemic on home delivery of contraceptives by community health workers in India: Time to (re) evaluate and innovate.

The World Health Organization (WHO) declared COVID-19 a global health emergency in January 2020, leading to a nationwide lockdown in India. It has been an experience from other outbreaks that governments cannot maintain the essential health services and guarantee health services. Due to COVID-19-related case management, all health schemes, including FP services, have been disrupted globally regarding availability, accessibility, appropriateness of service delivery, adequacy, and continuity of care. The impact of the pandemic on FP services listed includes disruptions in supply chain management

Differences Between the Psychological Symptoms of Health Workers and General Community After the First Wave of the COVID-19 Outbreak in Spain.

The coronavirus disease-2019 (COVID-19) has worsened the physical and mental health of the general population. Healthcare workers have a high risk of suffering a mental disorder after the first wave. In this way, psychologists, who deal with mental health issues and are considered as healthcare workers in many countries, are of interest in this context. The present study aimed to examine anxiety, depression, stress, and obsessions and compulsions across psychologists, healthcare professionals, and the general community. These variables were measured through the Depression, Anxiety, and Stress

Acceptability and Feasibility of a Nurse-Led, Community Health Worker Partnered Latent Tuberculosis Medication Adherence Model for Homeless Adults.

Homeless adults are at increased risk of latent tuberculosis infection (LTBI), which can lead to active tuberculosis (TB) disease. The purpose of this study was to assess acceptability and feasibility of a six-month, nurse-led, community health worker-partnered short-course treatment (3HP) LTBI adherence model for a high risk, LTBI positive, homeless population. Informed by our community advisory board (CAB) and community-based participatory research principles (CBPR), a qualitative study was undertaken and used focus group discussions to identify perspectives of homeless men and women who had

Acceptability and feasibility of delegating HIV counseling and testing for TB patients to community health workers in the Philippines: a mixed methods study.

BACKGROUND: The Philippines has a high burden of TB and HIV, yet the WHO estimates that only 13% of Filipino TB patients know their HIV status. This is partly attributable to the lack of trained HIV counselors and medical technologists (or laboratory technicians) at the primary healthcare level. In Africa where resources and manpower are also scarce, TB/HIV care is already delegated to community health workers. Evidence is scant however about the acceptability and feasibility of engaging community health workers to provide HIV counseling and testing (HCT) among TB patients in the Philippines. …
