Utilizing pharmacodynamic properties of second-generation antipsychotics to guide treatment.

Second-generation antipsychotics (SGAs) are used for the treatment of multiple psychiatric disorders including schizophrenia, bipolar depression, bipolar mania, autism and major depressive disorder. Additionally, their off-label use has been expanding to include other disorders as well, including post-traumatic stress disorder, obsessive compulsive disorder, generalized anxiety disorder, eating disorders and personality disorders. All SGAs share common properties; however, each individual SGA has a unique pharmacodynamic profile that may be utilized to guide and individualize treatment.

Diogenes syndrome and hoarding in the elderly: case reports.

Presented here are two case reports of elderly persons with Diogenes syndrome (variously known as senile breakdown, social breakdown and senile squalor syndrome). Diogenes syndrome is often (but not always) characterized by a tendency to hoard excessively (syllogomania). The first patient was diagnosed as having both a schizotypal personality disorder and obessive-compulsive disorders (OCD) while the second was diagnosed as having a schizoid personality disorder. Only the former demonstrated the tendency to hoard rubbish. The Diogenes syndrome in both cases can be hypothesized to be a reaction

Personality disorder types proposed for DSM-5.

The Personality and Personality Disorders Work Group has proposed five specific personality disorder (PD) types for DSM-5, to be rated on a dimension of fit: antisocial/psychopathic, avoidant, borderline, obsessive-compulsive, and schizotypal. Each type is identified by core impairments in personality functioning, pathological personality traits, and common symptomatic behaviors. The other DSM-IV-TR PDs and the large residual category of personality disorder not otherwise specified (PDNOS) will be represented solely by the core impairments combined with specification by individuals' unique set

Clinical types in a normal population: concurrent and construct validity.

Examined the relationship between personality and psychopathology through the use of the Multivariate Personality Inventory (MPI) (N = 100). The concurrent validity study compared the factors of the MPI with those of the Lazare-Klerman Personality Inventory, which was developed for a pathological population. Factor analysis of these two inventories yielded three factors that correspond to the three predicted personality styles: Hysteric, Compulsive, and Character Disorder. In the second part of this study the construct validity of these scales was examined. Results showed significant differenc

Self-injury often occurs for the first time in adolescence. The current classification distinguishes several groups: (1) factitious disorder imposed on self and factitious disorder imposed on another, (2) nonsuicidal self-injury (NSSI) and (3) obsessive-compulsive (OCD) and related disorders. The differential diagnosis includes a wide spectrum of psychiatric diseases, including borderline personality disorder (BPD), inherited disorders like Lesch-Nyhan syndrome (LNS) and also body modifications. In all these cases, self-harm is a subphenomenon. This review focuses on typical dermatological pic

Assumptions in borderline personality disorder: specificity, stability and relationship with etiological factors.

The specificity and stability of a set of assumptions hypothesized to be characteristic of Borderline Personality Disorder (BPD) was investigated. BPD patients (n = 16) were compared to cluster-C personality disorder patients (n = 12) and to normal controls (n = 15). All subjects were female and diagnosed with SCID-I and -II. Subjects rated a short version of the Personality Disorder Beliefs Questionnaire (PDBQ), with six sets of 20 assumptions each, hypothesized to be characteristic of avoidant, dependent, obsessive-compulsive, paranoid, histrionic and borderline personality disorder. The BPD

The Prevalence of Comorbid Personality Disorders in Treatment-Seeking Problem Gamblers: A Systematic Review and Meta-Analysis.

The aim of this study was to systematically review and meta-analyze the prevalence of comorbid personality disorders among treatment-seeking problem gamblers. Almost one half (47.9%) of problem gamblers displayed comorbid personality disorders. They were most likely to display Cluster B disorders (17.6%), with smaller proportions reporting Cluster C disorders (12.6%) and Cluster A disorders (6.1%). The most prevalent personality disorders were narcissistic (16.6%), antisocial (14.0%), avoidant (13.4%), obsessive-compulsive (13.4%), and borderline (13.1%) personality disorders. Sensitivity anal

[Personality disorders in a psychiatric unit: retrospective study].

From those patients who were admitted to the psychiatric in-patient unit of the Hospital Ramón y Cajal of Madrid across a period of eight years, those who received a diagnosis of personality disorder according to the axis II of DSM-III or DSM-III-R were selected. We analyzed the patterns of comorbidity between axis I and II, length of the admission, the percentage of readmissions and general variables as age and sex. 17.35% of the patients admitted for the first time and 27.03% of those readmitted received a clinical diagnosis of personality disorder. The most frequent disorders were: unspecif

Is Chronic Emptiness a Transdiagnostic Suicide Risk Factor?

Previous research suggests associations between suicidality (i.e., suicidal ideation and attempt) and chronic emptiness in patients with borderline personality disorder (BPD). However, approximately one-third of clinical patients without BPD endorse feelings of emptiness. The current study examined the relationship between chronic emptiness and suicidal behavior in 3,284 psychiatric outpatients. Patients were evaluated with the Structured Interview for DSM-IV Personality (SIDP-IV), the Structured Clinical Interview for the DSM-IV (SCID-IV), and the Schedule for Affective Disorders and Schizoph

Universal personality dimensions and dysfunctional obsessional beliefs in the DSM-5's OCD and related disorders (OCRDs).

This study aimed to determine the extent to which personality and cognitive factors contribute to the identification of shared associations between the DSM-5's OCD and Related Disorders (OCRDs). Participants (n = 239) were treatment-seeking outpatients with a principal diagnosis of obsessive-compulsive disorder (OCD), body dysmorphic disorder (BDD), hoarding disorder (HD), trichotillomania (TTM), or excoriation disorder (EXC), as compared to healthy community controls (n = 100). Analyses examined the relationships between diagnostic group, personality dimensions, and obsessive beliefs. Results

Gender Differences in Borderline Personality Disorder: Results From a Multinational, Clinical Trial Sample.

This study aims to extend previous research by considering gender differences in borderline personality (BPD) using both dimensional self-reported and clinical measures of symptomatology. Drawing from a cross-cultural, clinical trial sample, the authors compared female and male BPD subjects (N = 770; 211 male) between the ages of 18 and 65 using diagnostic and self-report data. The authors found that women with BPD have greater hostility and relationship disruption than men. Gender differences in eating disorders, particularly bulimia, are more divergent than in the general population. General

Personality traits and change in depression status at 18 months: Findings from a British Psychiatric Morbidity Survey.

BACKGROUND: Depression is a common mental disorder, yet it shows low remission rates. The available evidence on personality traits as factors associated with the course of depression has common methodological limitations. Identifying personality traits linked with depression can improve understanding of the course of illness. Therefore, we aimed to investigate personality traits that are associated with the course of depression over 18 months. METHODS: longitudinal data of 2366 Adult Psychiatric Morbidity Survey respondents were analysed. Assessments were applied at two-time points (baseline)

Psychiatric evaluation of the dermatology patient.

Over one third of dermatologic disorders have significant psychiatric comorbidity. The impact of the skin disorder on quality of life, the role of psychosocial stressors, and use of substances should be assessed. Major depressive disorder is the most frequently encountered psychiatric disorder in dermatology and is often associated with suicide risk. Other psychiatric syndromes comorbid with dermatologic disorders include obsessive-compulsive disorder, social phobia, posttraumatic stress disorder associated with dissociation and conversion symptoms, body image pathologies, delusional disorder,

Therapeutic implications of personality disorders in dermatology.

There is a significant psychiatric comorbidity that exists in multiple dermatological conditions, stemming from the patient''s own psychological make up. This article reviews personality disorders and their types, which influence the course and prognosis of several psychodermatological disorders. Self-inflicted skin lesions, for example, are usually associated with obsessive-compulsive behavior, but they also share connections to Narcissistic and Borderline personality disorders. Body dysmorphic disorder is another psychodermatological condition seen in dermatology, aesthetic, and cosmetic sur
