Dupuytren's Contracture: A Review of the Literature.

Dupuytren's contracture is a chronic condition that affects the palmar fascia, leading to progressive flexion of the fingers, particularly the ring and little fingers. This article provides an in-depth review of the current understanding of the condition and its management. Commonly seen in older men of Northern European descent, Dupuytren's can significantly impair hand function as contractures develop. The underlying mechanisms involve complex cellular processes, particularly the role of transforming growth factor-beta in promoting fibroblast activity and collagen buildup. Treatment strategi…

Stretch for the treatment and prevention of contractures.

BackgroundContractures are a common complication of neurological and non-neurological conditions, and are characterised by a reduction in joint mobility. Stretch is widely used for the treatment and prevention of contractures. However, it is not clear whether stretch is effective. This review is an update of the original 2010 version of this review.ObjectivesThe aim of this review was to determine the effects of stretch on contractures in people with, or at risk of developing, contractures. The outcomes of interest were joint mobility, quality of life, pain, activity limitations, participation …

Contractures in nursing home residents.

Contractures represent a common but preventable source of excess disability among nursing home residents. They result in many negative consequences such as pain, increased fall risk and decreased functional ability. Studies of prevalence vary in the definition of contracture and thus the exact enumeration, but consistently report the commonality in the nursing home setting. In this population, the clinician should focus on tertiary prevention and/or treatment. Methods such as those consistent with restorative care nursing have shown improvement in function and are a reasonable recommendation a…

Partial fasciectomy for Dupuytren's contractures.

One hundred ninety-six patients with Dupuytren's contractures were treated by partial fasciectomy and adequate postoperative rehabilitation. All patients had flexion contracture of the proximal interphalangeal joint of >20 degrees ; 93 patients had flexion contracture of the associated metacarpophalangeal joint of >30 degrees ; 143 patients had risk factors for Dupuytren's disease. Primary skin closure and splinting were done in all patients. Range of motion was begun by the 1st week. Splinting was discontinued by the 2nd week, followed by night-time splinting until the 8th week. The mean foll…

Management of neglected joint contractures.

This review is based on the author's experience treating over 5,000 joint contractures of the lower limbs in developing countries in the tropics and subtropics over the past 40 years. It includes realistic methods of correction of neglected, untreated joint contractures mainly due to poliomyelitis and other paralytic conditions by general duty medical officers. Corrections were performed in centers where medical facilities were often limited and where millions of untreated patients still await treatment. The treatment includes percutaneous correction of flexed hips, knees and equinus ankles an…

[Repair of flexor contracture of the hand].

OBJECTIVE: To investigate the etiology of the flexor contracture of the hand, to diagnose and to explore its surgical treatment. METHODS: From May 1997 to June 2004, 212 cases of flexor contraction of the hand were treated with scar excision, thorough loosening the contracture, covering the secondary skin defects with free skin grafting, "Z"-plasty, double "V-Y" plasty, transposition of the palmar dorsum flap of the hand and iliac-inguinal flap. There were 163 males and 49 females, whose ages ranged from 3 to 61. There were 85 cases of left hands, 54 cases of right hands, and 73 cases of both…

Oral burn contractures in children.

Oral burn contractures in children present major reconstructive problem. Only few reports in literature discussed oral burns in children. Electrical, chemical, and thermal agents are the main causative agents for oral burns. Oral contractures can be classified into anterior, posterior, and total. Anterior contractures are usually caused by electrical burns and involve the oral commissure, lips, anterior buccal sulcus and surrounding mucosa, and anterior tongue. Posterior oral contractures are caused by caustic ingestion and involve the posterior buccal mucosa, posterior tongue, retro-molar are…

Treatment of elbow contractures in burns.

Contractures of the elbow after burn injury are frequent and are often connected with other contractions. Surgical correction must take into consideration the topography, the likelihood of possible heterotopic ossification and the condition of the surrounding skin. A study of 66 contractures allowed us to reach the following conclusions: Linear contractures surrounded by normal skin require IC-type local flaps. Widespread contractures can be treated with thoracic flaps if healthy skin remains or else with the 'backwards flap'. Surgery on existing associated heterotopic ossification must be in …

Knee flexion contractures in myelodysplasia.

From 1976 to 1982, 12 children with myelodysplasia had surgery at our institution for knee flexion contractures (21 affected knees). Follow-up was a minimum of 18 months and averaged 47 months. Soft tissue lengthening of the hamstring tendons and release of the posterior capsule with or without femoral osteotomy yielded uniformly excellent results; femoral osteotomy alone produced 40% unsatisfactory results. Based on these data, we no longer recommend osteotomy alone as an effective treatment for fixed knee flexion contractures in children with myelodysplasia.

Post-burn contractures in Ibadan.

A review of our experience with post-burn contractures over a ten-year-period at the University of Ibadan is reported. Fifty-two patients (32 males, 20 females) were treated; 52% were under the age of 5 years. In 25% the contractures were due to therapeutic burns. Sixty-two per cent of the contractures occurred in the upper limbs. Best results were obtained by release of contracture, application of split-thickness skin grafts, and splinting for up to six months.
