The debate concerning the choice of crystalloids or colloids for resuscitation of the critically ill child is still unsettled. Moreover, the use of albumin in critically ill patients has been increasingly questioned because of the lack of clear-cut advantages over crystalloids as well as the concern for cost and the very minor risk of infection. Despite several meta-analyses addressing these issues, there is no data that supports the use of albumin unequivocally in any specific disease states. The suggestion that the use of albumin increases mortality in critically ill patients is not supporte

INTRODUCTION: Amniotic fluid volume reduction can have serious consequences: prior to 24 gestational weeks hypoplasia of fetal lungs and later in pregnancy cord compression and fetal hypoxia as well as contractions of extremities. Diagnosis of oligohydramnios is made either after a subjective estimation of amniotic fluid reduction or if the biggest amniotic fluid pocket is less than 1 cm. Amnioinfusion is a procedure during which a crystalloid solution, most often saline, is instilled into the amniotic cavity substituting amniotic fluid. To estimate the efficacy and safety of amnioinsfusion us

BACKGROUND: Increased leukocyte-endothelial cell interaction (LE) and deterioration of capillary perfusion represent key mechanisms of septic organ dysfunction. The type of volume support, however, which may be used during septic disorders, remains controversial. Using intravital microscopy, the authors studied the effect of different regimens of clinically relevant volume support on endotoxin-induced microcirculatory disorders, including the synthetic colloid hydroxyethyl starch (HES, 130 kD) and a crystalloid regimen with isotonic saline solution (NaCl).
METHODS: In Syrian Golden hamsters,

A significant fall in the levels of plasma albumin and a corresponding fall i in the levels of extracellular calcium occurred following cardiopulmonary bypass with the crystalloid prime. Postoperative hypoalbuminaemia was well tolerated, except in the patients with poor left ventricular function. Postoperative hypocalcaemia may increase digoxin sensitivity and may augment the negative inotropic effect of verapamil.

Septic shock is an emergency that is best treated by establishing priorities beforehand. The first priority is early recognition. The earliest recognizable clinical presentation is fever and hyperventilation. The second priority is augmenting normal compensatory mechanisms by intravenous infusion of crystalloid, with measurement of the response so that vasoactive drugs can be instituted as needed. The third priority is selected antibiotic therapy and drainage of pus. The fourth is corticosteroid therapy if the patient's response has been suboptimal.

Critically ill patients may be at higher risk for fluid overload and the development of interstitial edema due to damage to the endothelial glycocalyx, myocardial dysfunction, vasoplegia, renal dysfunction, increased vascular permeability, and hypoalbuminemia. While isotonic crystalloids are commonly used for resuscitation of hypovolemic shock, their use should be limited in critically ill patients to avoid complications, including the development of interstitial edema. Blood products should be considered for resuscitation of hemorrhagic shock to limit the volume of crystalloids required. To l

BACKGROUND: Fluid resuscitation is a core stone of hemorrhagic shock therapy, and crystalloid fluids seem to be associated with lower mortality compared to colloids. However, as redistribution starts within minutes, it has been suggested to replace blood loss with a minimum of a three-fold amount of crystalloids. The hypothesis was that in comparison to high volume (HV), a lower crystalloid volume (LV) achieves a favorable coagulation profile and exerts sufficient haemodynamics in the acute phase of resuscitation.
METHODS: In 24 anaesthetized pigs, controlled arterial blood loss of 50 % of th

In this narrative review, the studies and analyses are discussed that pertain to benefits and detriments of synthetic colloids versus natural colloids or crystalloids used for fluid resuscitation in sepsis and septic shock. The relative amount of fluid infusions used to reach clinical or hemodynamic end-points are reviewed, as well as potential toxicity of starch solutions on the kidney. Hence, it cannot be excluded that adverse effects partly offset beneficial hemodynamic effects that are similar to that of natural colloids, so that in most analyses a mortality benefit of synthetic colloid fl

BACKGROUND: Controversy regarding crystalloids or colloids for resuscitation has existed for over five decades, and large numbers of clinical trials have failed to resolve the controversy. In fact, the limitations of these studies have intensified the debate. This overview aims to revisit the debate of fluid resuscitation in trauma patients by critically appraising the meta-analyses on the subject.
METHODS: This study was a critical analysis of six meta-analyses found by MEDLINE search.
RESULTS: Overall, the choice of fluid may have a small or no effect on mortality. In trauma, the use of co

The colloid and crystalloid solutions used for resuscitation should preferably be free from effects on coagulation. In 10 volunteers, the effects of haemodilution with various concentrations of 0.9% sodium chloride and 4% succinylated gelatin were assessed by Sonoclot analysis, which describes the whole coagulation process. Small and moderate haemodilution (up to 40%) with 0.9% sodium chloride promoted coagulation. Similar haemodilution with 4% succinylated gelatin impaired coagulation, and at 60% haemodilution coagulation was very poor. These findings need to be confirmed in vivo and their cl…
