Effects of antithrombotic therapy on bleeding after endoscopic submucosal dissection.

BACKGROUND AND AIMS: Bleeding is the most common adverse event after endoscopic submucosal dissection (ESD). Although several studies have reported on the use of antithrombotic agents and post-ESD bleeding, many issues remain controversial. We conducted a meta-analysis and systematic review to evaluate the effects of antithrombotic therapy on post-ESD bleeding. METHODS: The published literature was searched on online databases, and all studies were included up to January 2017. Standard forms were used to extract data by 2 independent reviewers. The Newcastle-Ottawa Scale score was used to ass

Biological agents in polyarticular juvenile idiopathic arthritis: A meta-analysis of randomized withdrawal trials.

BACKGROUND AND OBJECTIVE: Although various biological agents are in use for polyarticular juvenile idiopathic arthritis (pJIA), head-to-head trials comparing the efficacy and safety among them are lacking. We aimed to compare the efficacy and safety of biological agents in pJIA using all currently available randomized withdrawal trials (wRCTs). METHODS: A systematic search of MEDLINE, EMBASE, CENTRAL, and clinicaltrials.gov was performed. Eligible wRCTs: patients with pJIA where a biological agent was compared with another biological agent or placebo. Efficacy was evaluated using disease flar

Rationales for abrupt discontinuation of long-term opioid therapy for chronic pain: A thematic analysis.

OBJECTIVE: Evidence and clinical guidelines document serious negative health consequences of abrupt discontinuation of long-term opioid therapy (LTOT). Nevertheless, recent studies suggest that up to 80 percent of patients with chronic pain who undergo LTOT deprescribing have their prescriptions abruptly discontinued. Given the frequency and high-risk nature of this practice, it is critical to understand the decision-making processes of physicians implementing abrupt opioid discontinuations while treating patients with chronic pain prescribed LTOT. DESIGN: In the present study, physicians wer

Evaluating the impact of medication review and deprescribing on prescribing appropriateness and clinical outcomes in older people residing in long-term care facilities: a systematic review and meta-analysis.

BACKGROUND: Polypharmacy is a major concern among older adults in long-term care facilities (LTCFs), as it increases the risk of potentially inappropriate medications (PIMs) and related adverse outcomes. Medication review and deprescribing interventions may help optimise therapy and reduce harm. DESIGN: Systematic review and meta-analysis. METHODS: This study was conducted according to PRISMA guidelines (PROSPERO: CRD42023486056). PubMed, Embase and Scopus were searched up to 27 August 2024, for experimental studies evaluating the impact of medication review/deprescribing interventions in ol

Insights Into Pharmacist-Led Deprescribing of Benzodiazepines and Related Drugs: A Qualitative Study With Physicians and Pharmacists.

While deprescribing benzodiazepines and related drugs (BZRDs) is crucial for preventing prolonged use and their associated adverse effects, it presents challenges from a healthcare perspective, because of limited resources and time. Recently, a pharmacist-led deprescribing of BZRDs in the treatment of insomnia was introduced in Helsinki's primary care health centres. To explore pharmacists' and physicians' insights, qualitative semi-structured interviews were conducted via Microsoft Teams with physicians and pharmacists involved in a pharmacist-led deprescribing. The interviews were recorded,

Barriers and Facilitators to Pharmacist-Led Deprescribing of Antihypertensives in Long-Term Care: A Survey-Based Study.

Pharmacist-led deprescribing is an approach of addressing polypharmacy in the long-term care (LTC) setting. However, the sustainability of this practice has not been widely studied. This study describes facilitators and barriers to pharmacist-led deprescribing from pre- and post-surveys completed as part of a randomized controlled antihypertensive deprescribing trial in Alberta, Canada. The surveys, targeting facilitators and barriers to deprescribing, were developed based on current evidence and designed through an iterative process with pharmacist input and consist of open- and closed-ended

Attitudes and beliefs towards medication burden and deprescribing in Parkinson disease.

BACKGROUND: Deprescribing of potentially inappropriate medications is recommended for older adults and may improve health outcomes and quality of life in persons living with Parkinson disease (PD). Patient attitudes, beliefs, and preferences play a crucial role in the success of deprescribing interventions. We aimed to examine the attitudes and beliefs about medication burden and deprescribing among persons living with PD. METHODS: We administered a survey to participants of Fox Insight, a prospective longitudinal study of persons living with PD. The survey included the revised Patients' Atti

Moxifloxacin induced encephalopathy in disseminated tuberculosis: A case report.

Moxifloxacin is a third-generation fluoroquinolone antibiotic with broad spectrum activity and also used as component of anti-tubercular therapy (ATT). Though frequently prescribed, moxifloxacin induced encephalopathy is uncommonly seen. Here we describe a forty four year old male, with features of disseminated tuberculosis (TB) and suspected case of multi-drug resistance (MDR), who developed acute encephalopathy. Extensive laboratory investigations, neuroimaging of brain, cerebrospinal fluid analysis was unremarkable. On stopping moxifloxacin, which was being administerd as part of ATT for MD

Dosage reduction and discontinuation of biological disease-modifying antirheumatic drugs in patients with rheumatoid arthritis, psoriatic arthritis and axial spondyloarthritis: protocol for a pragmatic, randomised controlled trial (the BIOlogical Dose OPTimisation (BIODOPT) trial).

INTRODUCTION: The The BIOlogical Dose OPTimisation (BIODOPT) trial is a pragmatic, multicentre, randomised controlled, open-label, parallel-group, equivalence study designed to evaluate tapering of biological disease-modifying antirheumatic drugs (bDMARDs) in patients with rheumatoid arthritis (RA), psoriatic arthritis (PsA) and axial spondyloarthritis (axSpA) in sustained clinical remission or low disease activity (LDA). Traditionally, these patients maintain standard dosage of bDMARD lifelong; however, recent studies indicate that a significant proportion of patients in sustained remission o

Overlapping approach Proton Pump Inhibitors/Nux vomica-Heel as new intervention for gastro-esophageal reflux management: Delphi consensus study.

BACKGROUND: Gastro-esophageal reflux disease (GERD) may affect the upper digestive tract; up to 20% of population in Western nations are affected by GERD. Antacids, histamine H2-receptor antagonists, and Proton Pump Inhibitors (PPIs) are considered the referring medications for GERD. Nevertheless, PPIs must be managed carefully because their use, especially chronic, could be linked with some adverse effects. An effective and safe alternative pharmacological tool for GERD is needed. After the identification of potentially new medications to flank PPIs, it is mandatory to revise and improve good

"Safer supply is the thing that changes everything": the role of prescribed safer supply in rebuilding trust following experiences of medical abandonment.

BACKGROUND: Prescribed safer supply provides people dependent on street-acquired opioids with pharmaceutical opioids to reduce overdose risk. Previous experience with the healthcare system - including opioid agonist treatment or high-dose opioids prescribed for pain - is common among people dependent on unregulated opioids, as are experiences of significant stigma when seeking care. Our objective was to examine previous healthcare experiences and how these differ from receiving care in safer supply programs. METHODS: Using reflexive thematic methods, qualitative interviews and focus groups we

SAGE (safer analgesia): a protocol for a pragmatic cluster randomised controlled trial to reduce the use of high-risk medicines in people with chronic back and/or neck pain.

INTRODUCTION: Chronic back and neck pain are commonly treated with opioid analgesics, gabapentinoids and benzodiazepines, despite their high-risk profile and potential limited efficacy. Deprescribing interventions (aimed at medicine reduction or cessation) have shown some effectiveness in reducing the use of these medications, but similar approaches tailored to individuals with complex chronic back and/or neck pain are lacking and urgently needed. This trial will evaluate the effect of the safer analgesia (SAGE) intervention (training of general practitioners (GPs) in deprescribing and support

Prescriber Acceptability of a Direct-to-Patient Intervention for Benzodiazepine Receptor Agonist Deprescribing and Behavioural Management of Insomnia in Older Adults.

Behavioural treatments are recommended first-line for insomnia, but long-term benzodiazepine receptor agonist (BZRA) use remains common and engaging patients in a deprescribing consultation is challenging. Few deprescribing interventions directly target patients. Prescribers' support of patient-targeted interventions may facilitate their uptake. Recently assessed in the Your Answers When Needing Sleep in New Brunswick (YAWNS NB) study, Sleepwell (mysleepwell.ca) was developed as a direct-to-patient behaviour change intervention promoting BZRA deprescribing and non-pharmacological insomnia mana

Opioid and dependence-forming medications co-prescription reduction and oversight: the OPIO-CHECK Quality Improvement Project (QIP) protocol and interim analysis.

BACKGROUND: Long-term opioid prescription to manage chronic non-cancer pain (CNCP) is rapidly rising, despite the lacking evidence supporting their safety and efficacy. Co-prescribing opioids with other dependence-forming medications (DFMs) causes fatal side effects. Clinicians are advised to avoid combinations of DFMs and, where suitable, deprescribe to improve patient safety. AIM: To review the number of patients registered to the Grange Medical Centre (Nuneaton, Warwickshire) who are co-prescribed an opioid and either benzodiazepine/gabapentinoid for CNCP and to reduce usage of these DFMs.
