A patient developed disseminated intravascular coagulation with purpura fulminans 1 month after starting Dilantin therapy for a seizure disorder. In addition, the patient developed exfoliative dermatitis, hepatitis, cutaneous vasculitis, and microangiopathic hemolytic anemia. She was successfully treated with adrenal steroids and heparin for her purpura fulminans. The hepatitic dermatologic, along with hemorrhagic, complications of Dilantin are reviewed, and the possible origin of the vasculitis and disseminated intravascular coagulation is discussed.

This paper reports on the results of a case study of a 19-year-old female who presented to the Oral Medicine clinic with a chief complaint of scaly and peeling lips. The lesions had persisted on her lips for more than 7 years and were refractory to previous treatment. Her physician's diagnosis was contact dermatitis. We diagnosed this patient as having exfoliative cheilitis (EC). A patch test using the toothpaste containing sodium lauryl sulfate (SLS) was positive and the patient discontinued using it. Instead, she started using a toothpaste not containing SLS. One year after treating her lesi

OBJECTIVE: To describe an outbreak of dermatitis induced by the beetle Paederus australis and the public health measures implemented to control it.
SETTING: A remote Aboriginal community in the Northern Territory of Australia.
METHODS: Inspection of the community and interviews with residents to document skin lesions, sleeping locations and beetle activity. Samples of the beetle were collected for identification.
RESULTS: The community was affected by a plague of beetles. All 40 residents reported painful, blistering, skin lesions. Four required hospitalisation for treatment of an extensive

Exfoliative dermatitis, including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN), is a mild-to-life-threatening process that has been described after exposure to many antiepileptic drugs. The increased use of antiepileptic drugs for treatment of bipolar disorder and neurologic disorders has extended the risk of exfoliative disorder to this population of patients, and these patients and their health care providers may not be familiar with the risks involved with these drugs. We describe the cases of a 28-year-old woman with bipolar 1 disorder initially treated with lamotrig

Idiopathic systemic granulomatous disease (ISGD), also known as equine sarcoidosis is an uncommon disease of horses, manifesting in exfoliative dermatitis and granulomatous inflammation in various organs. The current report presents a case of a 15-year-old Hanoverian mare with a 4-month history of weight loss, recurrent fever, skin lesions, and movement disorders. Pathological examination revealed granulomatous and necrotizing inflammation in the skin, regional lymph nodes, and cerebellum. Based on histological, immunohistochemical, and microbiological findings, the diagnosis of ISGD was made.

The DRESS (drug rash, eosinophilia and systemic symptoms) syndrome, is a type of drug hypersensitivity syndrome, is a severe idiosyncratic reaction to several drugs, mainly antiepileptics and antibiotics, which can occasionally produce acute hepatic failure. In this article, the authors report a case of DRESS syndrome with severe exfoliative dermatitis with acute hepatitis associated with intake of leflunomide. Rapid diagnosis with withdrawal of offending agent is crucial for survival of these cases.

We report a 65-year-old black man whose onset of an erythroderma was followed in 6 weeks by a generalized vitiligo. The etiology of the erythroderma was unclear, although a prostate adenocarcinoma was found on evaluation. This is to our knowledge the first patient reported with a vitiligo following exfoliative dermatitis, and the first report of two uncommon markers of internal malignancy (erythroderma and vitiligo) occurring with prostate adenocarcinoma in the same patient.

A trial of etretinate (Tigason) for keratinizing disorders of the skin yielded prolonged improvements beyond any results hitherto obtained. The disorders included psoriasis of severe degree, keratosis obturans, ichthyosis vulgaris, follicular ichthyosis, porokeratosis of Nékam, disseminated superficial actinic porokeratosis. Darier's disease, palmoplantar keratoderma, Oudtshoorn disease, lamellar ichthyosis, 'badpak' ichthyosis, epidermolytic hyperkeratosis, the 'half-baby' syndrome, exfoliative dermatitis, hair dystrophy and selected precancerous conditions. In twelve patients with cutaneous reactions to practolol the clinical appearance of the rash was variable, including eczematous, exanthematous, lichenoid, and psoriasiform eruptions, and one exfoliative dermatitis. The development of the rash and its reappearance after oral challenge were slow. But histological findings were almost identical and were not those of any other drug eruption. Vacuolated and dyskeratotic epidermal cells and mild disorganization with extra mitoses of the epidermal cells were seen.

Members of four generations of a family had a defect of serum opsonization for yeast phagocytosis consistent with dominant inheritance. 2 were healthy, one had chronic osteomyelitis, and the fourth developed a fatal illness in infancy characterized by exfoliative dermatitis, diarrhoea, multiple bacterial infections, and failure to thrive, which resembled the two prevously reported cases with this opsonization defect. At necropsy the infant also had lymphoid depletion, which was possibly secondary, and massive histiocytic infiltration.

Erythroderma is a serious dermatological syndrome characterized by prolonged erythema and scaling, affecting more than 80 % of the body surface. Erythroderma can have multiple etiologies, the most common of which are grouped under the acronym PALM (psoriasis, atopic dermatitis, lymphoproliferative disorders, and medications). Nevertheless, the clinical presentation is similar regardless of the cause. In this article, we will discuss the most common causes of acquired erythroderma in adults, the diagnostic approach, and treatment management, although the latter strongly depends on its etiology.

Kaposi's varicelliform eruption (KVE) is a widespread cutaneous eruption caused by viruses, especially herpes simplex virus in patients with pre-existing dermatoses. "Psoriasis herpeticum" refers to the rare occurrence of KVE in patients with psoriasis. We report a case of KVE localized to the face in a patient with exfoliative dermatitis secondary to psoriasis. This case is being reported to make the treating clinician aware of the possibility of KVE in patients with psoriatic erythroderma.

Some skin conditions, such as blistering diseases, guttate and pustular psoriasis, exfoliative erythroderma, reactive erythemas and acute contact dermatitis have an acute onset and require prompt intervention. Acute vesicular or weeping dermatoses are best treated with cool, wet compresses and topical lotions, whereas dry, scaly skin conditions should be treated with hydrating measures and topical ointments. Oral antihistamines and topical and systemic steroids are the mainstays of treatment for these severe dermatoses.
