[Micturition disorder as a sequela of sacral autonomic diabetic neuropathy].

Diabetic neuropathy may manifest itself also in the autonomous nervous system and thus lead to a functional disorder of the urinary bladder. Urodynamic studies carried out in 11 diabetic patients with unclear bladder dysfunction allowed 2 forms of bladder anomalies to be identified: 1. detrusor hyperreflexia, 2. hyporeflexia and areflexia of the detrusor. Whereas hyperreflexia is clinically characterised by signs of motor urgency, hyporeflexia is mainly identifiable by residual urine formation and recurrent infections of the urinary tract. Therapy depends on the type of dysfunction and consist

Neuropathy associated with transient diabetes mellitus in 2 cats.

A 9-year-old spayed cat that flinched when touched responded poorly to methylprednisolone and phenobarbital. Hyperalbuminemia, hyperproteinemia, hyponatremia, hypochloremia, hyperglycemia and glycosuria resolved with insulin treatment. The flinching disappeared in 2 weeks and insulin use was unnecessary after 6 weeks. A 6-year-old castrated cat with diabetic ketoacidosis and azotemia responded to fluid and insulin therapy but developed a head tilt and insulin shock 2 weeks later. The cat recovered after 6 days of supportive treatment and no longer required insulin after 4 weeks.

Preservation of the hypoxic drive to breathing in diabetic autonomic neuropathy.

1. Unexplained cardiorespiratory arrests have been reported in patients with diabetic autonomic neuropathy and these could be due to denervation of the carotid chemoreceptors. 2. We have studied the ventilatory response to transient hypoxia (Ve/Peto2) during exercise in 22 male diabetic patients, six with symptomatic and cardiovascular evidence to suggest diabetic autonomic neuropathy (DAN+) and 12 without these features (DAN-). 3. There was no difference in the ventilatory response to transient hypoxia between the different groups of diabetic patients (Ve/Peto2 in DAN+ patients = -0.9 +/- 0.2

Ischaemic optic neuropathy with painful ophthalmoplegia in diabetes mellitus.

Two patients with mild, adult-onset diabetes mellitus developed a painful ophthalmoplegia and ipsilateral optic neuropathy that was relatively unresponsive to steroids. Histopathological study of the optic nerve of one patient revealed an extensive ischaemic infarct. There was ultimate recovery from the cranial nerve palsies in both patients and the optic neuropathy in one patient. Ischaemic polyneuropathy involving the cranial nerves may cause an orbital apex/cavernous sinus syndrome in patients with diabetes mellitus.

Anterior ischemic optic neuropathy. VI. In juvenile diabetics.

3 cases of acute optic neuropathy in juvenile diabetics are reported. The neuropathy included initially optic disc edema (ODE), usually with prominent, dilated and frequently telangiectatic vessels over the disc, and later, generally, development of optic disc-related visual field defects. On resolution of the neuropathy, the disc was normal in all eyes except for a mild pallor in one, and the visual acuity and fields recovered to normal. 2 of the patients also had early diabetic retinopathy. The nature of the optic neuropathy is discussed. We feel the available evidence indicates that the con

Electrodiagnostic analysis of peripheral neuropathy in dogs with diabetes mellitus.

An elecrodiagnostic analysis was conducted on 5 dogs with spontaneously occurring diabetes mellitus that were maintained for a long-term study on canine diabetes. Abnormal electromyographic findings consisted of fibrillation potentials, positive sharp waves, and fasciculation potentials in the limb muscles of most dogs, The mean values (+/- SD) for motor nerve conduction velocities (NCV) of the ulnar and sciatic-tibial nerves were 57.6 +/- 11.2 and 52.1 +/- 7.4 m/s, respectively, for the 5 diabetic dogs. Values for motor NCV for clinically normal dogs in this laboratory were 56.0 +/- 7.6 and 6

Diabetes mellitus and neuropathy following Vacor ingestion in man.

Two patients ingested Vacor, a rodenticide containing the active ingredient N-3 pyridylmethyl-N'-p-nitrophenyl urea. Both patients developed ketosis-prone diabetes mellitus and severe autonomic neuropathy. Niacinamide therapy given nine hours after Vacor ingestion in one patient and 14 hours after ingestion in the other was not successful in preventing these sequelae. Physicians need to be aware of the toxicity of Vacor, and the potential therapeutic benefit of early niacinamide therapy.

Dermal fibroblast mitochondrial profiles in painful diabetic neuropathy.

AIMS/HYPOTHESIS: Dermal fibroblasts have emerged as potential contributors to chronic pain, yet their role in diabetic polyneuropathy (DPN) and neuropathic pain remains poorly defined. Mitochondrial dysfunction and low-grade inflammation have been implicated in different pain conditions, but whether fibroblast mitochondrial health and cytokine secretion contribute to painful DPN is unknown. METHODS: We conducted an integrated cellular and molecular profiling of dermal fibroblasts and skin biopsies from 30 participants, grouped into control participants (n=5), diabetes without DPN (n=7), pain-

Mediation of Smoking and Cardiometabolic Factors on Diabetic Peripheral Neuropathy.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is a common complication of type 2 diabetes, yet the combined roles of smoking behavior and cardiometabolic factors in shaping neuropathy risks remain insufficiently understood. OBJECTIVES: To examine the associations of smoking behavior and cardiometabolic factors with DPN risk and evaluate their independent, combined, and mediating effects. METHODS: This cross-sectional study included 905 adults with type 2 diabetes attending primary care services in Indonesia. DPN risk was assessed using the Michigan Neuropathy Screening Instrument. Diabetic neuropathy: cutting-edge research and future directions.

Diabetic neuropathy (DN) is a prevalent and debilitating complication of diabetes mellitus, significantly impacting patient quality of life and contributing to morbidity and mortality. Affecting approximately 50% of patients with diabetes, DN is predominantly characterized by distal symmetric polyneuropathy, leading to sensory loss, pain, and motor dysfunction, often resulting in diabetic foot ulcers and lower-limb amputations. The pathogenesis of DN is multifaceted, involving hyperglycemia, dyslipidemia, oxidative stress, mitochondrial dysfunction, and inflammation, which collectively damage

Involvement of cellular and enzymatic aspects in the complexity of diabetic neuropathy.

Diabetic neuropathy (DN) is a multifaceted and progressive complication of diabetes mellitus, characterized by functional and structural damage to peripheral, autonomic, and sensory nerves. Despite its high prevalence and debilitating consequences, current therapeutic approaches remain largely symptomatic, with limited disease-modifying strategies available. The pathogenesis of DN is driven by a complex network of molecular, cellular, and enzymatic interactions, primarily instigated by chronic hyperglycemia. This review unravels the intricate molecular and cellular crosstalk underlying DN, emp

Diabetic Neuropathy Part 1: Overview and Symmetric Phenotypes.

Diabetes is the most common cause of neuropathy in the United States, with diabetic neuropathy being the leading complication, ultimately affecting up to 50% of patients with type 1 and type 2 diabetes mellitus. Various types of neuropathies can be associated with diabetes, with symptoms commonly including numbness, tingling, pain, and occasionally weakness. Metabolic, vascular, and immune theories have been proposed for the pathogenesis of diabetic neuropathy. Management of diabetic neuropathy should begin at the time of diabetes diagnosis and primarily focuses on glycemic control, along with

Diabetic Peripheral Neuropathy: New Diagnostics and Treatment Perspectives.

Diabetic peripheral neuropathy (DPN) is one of the most significant chronic complications in people with diabetes. It is a highly heterogeneous condition that affects various parts of the nervous system and presents with a wide range of symptoms. Early diagnosis of diabetic neuropathy is possible if regular screening for this complication is conducted using modern diagnostic methods. Every diabetes clinic should perform annual screening for DPN to identify the risk of diabetic foot disease using a monofilament and tuning fork (or biothesiometer). The treatment of diabetic neuropathy remains li

Diabetic Neuropathy Part 2: Proximal and Asymmetric Phenotypes.

Diabetic neuropathy is the most common cause of peripheral neuropathy, with distal symmetric polyneuropathy (DSPN) being the most common form. There are other, less common subtypes that have been described since the 1800s. Unlike DSPN, these subtypes can be asymmetric and affect proximal regions. They include lumbosacral and cervical radiculoplexus neuropathy, thoracic radiculopathy, and cranial and entrapment mononeuropathies. Several theories have been proposed to explain the development of these neuropathies. In most cases, spontaneous recovery occurs, and treatment typically focuses on sym

A Study of Cardiac Autonomic Neuropathy in Patient with Diabetes Mellitus.

BACKGROUND: Atherosclerotic coronary artery disease, diabetic cardiomyopathy, and cardiac autonomic neuropathy (CAN) are the three categories into which the cardiovascular consequences of diabetes can be grouped. After all other potential causes have been ruled out, cardiovascular autonomic neuropathy-often referred to as CAN in the literature-is defined as the impairment of autonomic regulation of the cardiovascular system. Finding people with CAN is crucial because, if detected early enough, comprehensive therapies focusing on lifestyle, glucose management, and cardiovascular risk factors ca
