BACKGROUND: Although Müllerian anomalies are relatively common they can be easily misdiagnosed as other gynecologic conditions leading to inappropriate treatment.
CASE: An 18-year-old woman presented to the hospital with abdominal pain and was found to have a 17-cm pelvic mass and absence of the cervix. Because of concern for recurrent endometrioma formation in the setting of a Müllerian anomaly, she underwent a hysterectomy. During surgery, she was noted to have complete uterine didelphys with cervical agenesis and a normal vagina.

Urethral duplication is a rare congenital malformation, especially in females. It may be associated with complex urogenital malformations, but the association with a cystic phallic urethra and a uterus didelphys is exceptional. We report a case of a newborn with urethral duplication, with the accessory urethra exteriorized by a large cyst, associated with a uterus didelphys and bone malformations. We discuss the clinical, radiographic, and therapeutic aspects as well as a literature review.

Mullerian duct abnormalities are congenital malformations that are easily missed and can lead to incorrect diagnosis and unnecessary operative procedures. In this case, a young female presented with cyclic pelvic pain that continued after previous surgical resection of an ovarian cyst. A double uterus associated with an obstructed hemivagina is a rare malformation syndrome that has been described in about one hundred cases in the world literature. The malformation is diagnosed after menarche because of symptoms caused by hematocolpos or hematometra, and it is occasionally associated with ipsilateral renal agenesis. In the case described here, the malformation was symptomatic when the patient was 5 months old, and was associated with ipsilateral renal hypoplasia. The clinical and etiologic implications are discussed.

A 27-year-old woman with ovarian immature teratoma underwent 18 F-FDG PET/CT for preoperative staging, which revealed intense FDG uptake in 2 distinct pelvic structures, mimicking primary endometrial cancer or metastasis. Although MRI showed no anatomic lesions in either uterus, it confirmed uterine didelphys-a rare Müllerian duct anomaly. This case illustrates that physiological menstrual uptake within a duplicated uterus can simulate malignancy. Clinicians must recognize such anatomic variants to avoid diagnostic pitfalls and accurately stage pelvic malignancies on PET/CT.

Angiostrongylus cantonensis is a worldwide zoonotic parasite that causes eosinophilic meningoencephalitis in many species of animals including humans. Opossums are animals that are naturally resistant to the proteolytic effects of Crotalid venoms. Opossums possess proteinase inhibitors in their sera that bind to and neutralize hemorrhagic and other proteolytic activity in many snake venoms.
