Phase 2 Trial of Long-Acting Cabotegravir and VRC07-523LS for Viral Suppression in Adults With HIV-1: ACTG A5357.

BACKGROUND: Long-acting regimens are needed to expand antiretroviral therapy (ART) options for people with human immunodeficiency virus type 1 (HIV-1). Combining broadly neutralizing antibodies (bNAbs) with long-acting small-molecule antiretrovirals may offer an alternative to daily oral therapy. METHODS: We conducted a phase 2, open-label, single-arm trial at AIDS Clinical Trials Group (ACTG) sites across the United States. Eligible adults had HIV-1 virologically suppressed on ART for ≥2 years, CD4 count ≥350 cells/μL, and susceptibility to VRC07-523LS (half-maximal inhibitory concentration

Impact of Switching to Long-Acting Injectable Cabotegravir Plus Rilpivirine on Rectal HIV-1 RNA Shedding and Implications for Transmission Risk.

BACKGROUND: The impact of long-acting injectable cabotegravir plus rilpivirine (CAB/RPV) on rectal human immunodeficiency virus 1 (HIV-1) RNA dynamics and the factors associated with viral shedding remain poorly understood. METHODS: This prospective study evaluated HIV-1 RNA dynamics by analyzing sequential paired plasma and rectal fluid samples from virologically suppressed individuals who transitioned from oral antiretroviral therapy (ART) to every-2-month CAB/RPV (preceded or not by oral lead-in), over a 9-month follow-up period. RPV trough concentrations were measured in 384 rectal sample

Strategies to Manage Dosing Deviations and Interruptions of Cabotegravir Long-Acting Intramuscular Injections.

Long-acting cabotegravir is approved for HIV treatment and prevention. To guide management of dosing deviations and interruptions, concentration-time profiles for monthly and every 2 months regimens were simulated using a population pharmacokinetic (PPK) model. Adequate exposure was defined as trough concentration (Ctau) >0.45 µg/mL (observed 5th percentile of first Ctau in pivotal studies) in >95% of subjects and maximum concentration (Cmax) <13.1 µg/mL (highest observed median steady-state Cmax in previous studies) in >50% of subjects. Simulations showed: (1) median Cmax remained ≤6.35 µg/mL

Early Implementation of Long-Acting Injectable Cabotegravir for HIV Prevention in a Safety Net Hospital-based Primary Care Center in U.S. South.

BACKGROUND: Long-acting injectable cabotegravir (CAB-LA), approved in 2021, is a more effective HIV prevention method than daily oral PrEP. CAB-LA expansion addresses key HIV prevention gaps, especially in the Southern United States (U.S.), to support national Ending the HIV Epidemic (EHE) goals. However, complex implementation requirements hinder CAB-LA expansion, highlighting the need for real-world data to guide effective and equitable scale-up. OBJECTIVE: Describe the implementation and early outcomes of implementing CAB-LA in a large safety-net primary care center in the Southern U.S. D

Clinical outcomes of long-acting cabotegravir and rilpivirine in people with HIV aged 60 and older: Real-world data from the Spanish RELATIVITY cohort.

BACKGROUND: Long-acting injectable cabotegravir and rilpivirine (LAI CAB + RPV) is an established maintenance strategy for people with HIV who are virologically suppressed, providing high efficacy and convenience in treatment. However, evidence in older adults (≥60 years), a growing and complex population, is limited. METHODS: We conducted an ambispective real-world sub-analysis of the Spanish RELATIVITY cohort, including people with HIV aged ≥60 years who initiated LAI CAB + RPV across 58 centres. Outcomes (virological suppression, safety, discontinuation and adherence) were compared with th

Early experiences with usage of long-acting injectable cabotegravir among adults in rural Ugandan and Kenyan communities: qualitative research from the SEARCH "Dynamic Choice HIV Prevention" intervention trials.

INTRODUCTION: Despite oral HIV pre-exposure prophylaxis (PrEP) effectiveness, uptake and adherence remains a challenge. Newer HIV prevention technologies, including long-acting injectable cabotegravir (CAB-LA), are promising for addressing known barriers to oral PrEP uptake and adherence, yet research remains limited on experiences with CAB-LA among at-risk adults in community settings. This descriptive qualitative study explored experiences with CAB-LA among adults participating in the SEARCH Dynamic Choice HIV Prevention (DCP) trial in rural Kenya and Uganda, which evaluated HIV prevention u

Case series of two persons living with HIV with detectable viral loads initiated then suppressed on cabotegravir/rilpivirine with lenacapavir.

Long-acting (LA) cabotegravir/rilpivirine (CAB/RPV) is primarily prescribed for virologically suppressed persons living with HIV (PLWH). Patients experiencing pill dysphagia or profound adherence challenges were excluded from the phase 3 studies, but recent reports demonstrate successful treatment in PWLH with baseline viremia. We describe two PLWH with detectable viral loads (VL) with multidrug resistance mutations. They were unable to sustain virologic suppression on oral therapy with historical poor adherence and dysphagia. Initiation of intramuscular CAB/RPV with subcutaneous lenacapavir (

Learning from the first: a qualitative study of the psychosocial benefits and treatment burdens of long-acting cabotegravir/rilpivirine among early adopters in three U.S. clinics.

INTRODUCTION: Perspectives on long-acting injectable cabotegravir/rilpivirine (CAB/RPV-LA) from HIV health disparity populations are under-represented in current literature yet crucial to optimize delivery. METHODS: Between August 2022 and May 2023, we conducted in-depth interviews with people with HIV (PWH) at four HIV clinics in Atlanta, Chicago and San Francisco. Eligibility criteria were current CAB/RPV-LA use with receipt of ≥3 injections or CAB/RPV-LA discontinuation. We purposefully sampled for PWH who initiated with viraemia (plasma HIV RNA >50 copies/ml) due to adherence challenges,

Qualitative evaluation of survey questions to assess treatment preference for daily oral or long‑acting injectable antiretroviral therapy among people living with HIV.

Treatment of HIV has historically required taking daily oral antiretroviral therapy (ART). A recent alternative to daily oral ART is long-acting injectable ART with cabotegravir plus rilpivirine, administered monthly or every 2 months. The purpose of this qualitative study was to evaluate the concept relevance and interpretability of five previously developed questions: one treatment preference question and four questions designed to assess how the emotional burden associated with HIV treatment impacts treatment preferences. Thirty adults in the United States currently receiving HIV treatment

Frequency of genotypic factors possibly associated with cabotegravir/rilpivirine failure in antiretroviral treatment-naïve and -experienced HIV-1- infected population.

OBJECTIVES: The long-acting injectable (LAI) cabotegravir (CAB) and rilpivirine (RPV) treatment offers important advantages over oral ART (antiretroviral therapy), however baseline factors possibly contributing to the CAB/RPV treatment failure were identified. The purpose of this study was to describe the frequency of virologic factors previously influencing efficacy of this treatment, namely RPV and CAB resistance-associated mutations (RAMs) and A1/A6 subtype among naïve and treatment-experienced HIV-1-infected patients from Poland. METHODS: The following datasets of HIV-1 sequences were ana

Comparable In Vitro Activities of Second-Generation HIV-1 Integrase Strand Transfer Inhibitors (INSTIs) on HIV-1 Clinical Isolates with INSTI Resistance Mutations.

Second-generation HIV-1 integrase strand transfer inhibitors (INSTIs) dolutegravir (DTG), bictegravir (BIC), and cabotegravir (CAB) showed a high genetic barrier to resistance and limited cross-resistance with first-generation INSTIs raltegravir (RAL) and elvitegravir (EVG). In this study, DTG, BIC, and CAB demonstrated a comparable activity on a panel of INSTI-resistant strains isolated from patients exposed to RAL, EVG, and/or DTG, with a significantly reduced susceptibility only with the pathway Q148H/K/R plus one to two additional INSTI mutations.

Ultra-long-acting injectable, biodegradable, and removable in-situ forming implant with cabotegravir for HIV prevention.

HIV is a global health issue affecting approximately 40 million people worldwide, with low patient adherence being the primary challenge for maintaining effective pre-exposure prophylaxis (PrEP). Limitations of oral PrEP are mainly attributed to lack of adherence due to pill fatigue or stigma [1, 2]. Recently introduced long-acting injectables offer several advantages over daily PrEP, namely by improving adherence and improving efficacy [3-5]. The approved long-acting injectable Apretude® is administered every two months and elicits a long pharmacokinetic tail of >15 months that can lead to po

Perceptions of the attributes of new long-acting HIV pre-exposure prophylaxis formulations compared with a daily, oral dose among South African young women: a qualitative study.

Oral PrEP is highly effective against the acquisition of HIV but is underutilised by young women. New options, like the monthly dapivirine vaginal ring (DVR) and injectable long-acting cabotegravir (CAB-LA), are emerging. However, little is known about young women's perceptions of these alternatives. This qualitative study explored perceptions of the attributes of PrEP technologies in South Africa. Young women accessing sexual health services were purposively selected to participate in 22 in-depth interviews, five focus group discussions and two workshops using the nominal group technique, bet

Cabotegravir-loaded polymeric micelles: a promising strategy for improved solubility and therapeutic outcomes.

OBJECTIVE: To design and optimize a d-α-tocopheryl polyethylene glycol 1000 succinate (TPGS)-stabilized polymeric micellar system for cabotegravir (CAB), strategically addressing its dissolution-rate and permeability constraints to potentiate oral bioavailability and maximize antiretroviral therapeutic outcomes. SIGNIFICANCE: CAB, a potent HIV-1 integrase strand transfer inhibitor, suffers from dissolution-rate limitations and P-glycoprotein efflux, restricting oral utility. Incorporation of TPGS enables superior solubilization, enhances intestinal translocation, and circumvents efflux transp
