Outcomes of cannulation strategies under antegrade cerebral perfusion in type a aortic dissection surgery: a systematic review and meta-analysis.

OBJECTIVE: Arterial cannulation strategies remain a topic of debate in type A aortic dissection (ATAAD) repair surgery due to their variability and the inconsistent use of concomitant cerebral perfusion techniques. This study aims to compare clinical outcomes associated with different arterial cannulation strategies in patients undergoing ATAAD repair with antegrade cerebral perfusion (ACP). METHODS: A systematic literature search was conducted to identify studies comparing clinical outcomes of at least two arterial cannulation strategies (subclavian, femoral or direct aortic) in ATAAD repair

[Ruptured thoracic aortic aneurysm complicated by acute aortic dissection and aortic valve regurgitation; report of a case].

An 87-year-old man was found loss of consciousness after falling. He was found in a state of shock. Computed tomography showed rupture of aneurysm of the ascending aorta and aortic arch with acute aortic dissection. Echocardiography revealed aortic valve regurgitation and cardiac tamponade. As the result of emergency operation, a large hematoma in the mediastinum and pleural cavity as well as massive serous pericardial effusion were found. The dissection was seen in aneurysm of the ascending aorta and aortic arch with an intimal tear located in the aortic arch. After aortic valve replacement w

Stanford type B aortic dissection associated with pregnancy in patients with Marfan syndrome-A case report and review of the literature.

A 36-year-old female patient known to have Marfan syndrome (MFS) presented with Stanford type B aortic dissection (type B-AD) 3 days after delivery although she had taken oral β-blocker and underwent prophylactic cesarean section at 34 weeks when she showed 42 mm of the ascending aorta. She was successfully treated medically without further progression of the dissection. A review of the literature revealed an additional 19 patients with MFS who suffered from type B-AD associated with pregnancy. Of 20 patients, 1 (5%) died but the remaining 19 patients were successfully treated either medically

Late reoperations after repaired acute type A aortic dissection.

OBJECTIVE: Late complications can develop in patients after surgery for aortic type A dissection, mandating redo surgery on the ascending aorta and arch. METHODS: From 2006 to 2010, 23 patients (aged 41-69 years) who had late complications related to previous aortic surgery for acute type A dissection underwent redo surgery. Initial surgery included ascending aorta replacement in all cases. RESULTS: The main indications for reoperation were progressive enlargement of the false lumen of the aortic arch or descending aorta and suture line dehiscence in 10 patients each. All patients with progr

Visualizing the malperfusion effect of coronary occlusion as a sequela of limited type A aortic dissection.

Limited type A aortic dissection (LTAAD) is a rare subtype of dissection that is confined within a well-defined border of the ascending aorta. These dissections may occur in the remaining native portion of the aortic root following aortic root replacement and can be complicated by malperfusion syndrome-a syndrome where dissections compromise the aortic branches and lead to end-organ ischemia. Because LTAAD is confined within the ascending aorta, malperfusion syndrome may preferentially affect the coronary arteries resulting in coronary malperfusion, myocardial infarction, and increased mortali

Histological and genetic studies in patients with bicuspid aortic valve and ascending aorta complications.

OBJECTIVES: Aneurysm diameter and growing rate does not represent a definite parameter for operation in bicuspid aortic valve (BAV), ascending aortic aneurysm and normal root patients. Thus, we investigated histological and immunohistochemical aspects of different segments of ascending aorta (precisely, aortic root without dilatation, aneurysmatic tubular portion, dissected ascending aorta) and genetic features of patients with BAV and ascending aorta complication (aneurysm or dissection). METHODS: Aorta tissue samples of 24 BAV patients were examined. The patients comprised of 18 men and 6 w

Anatomically realistic aortic dissection simulator as a potential training tool for point-of-care ultrasound.

AIM: Simulators for aortic dissection diagnosis are limited by complex anatomy influencing the accuracy of point-of-care ultrasound for diagnosing aortic dissection. Therefore, this study aimed to create a healthy ascending aorta and class DeBakey, type II aortic dissection simulator as a potential point-of-care ultrasound training model. MATERIAL AND METHODS: 3D mould simulators were created based on computed tomography images of one healthy and one DeBakey type II aortic dissection patient. In the next step, two polyvinyl alcohol-based and two silicone-based simulators were synthesised. RE

Late sudden death after surgical correction of coarctation of the aorta. Importance of aneurysm of the ascending aorta.

Follow-up studies averaging 12 years postcorrective surgery of 343 patients with coarctation of the aorta disclosed 38 late deaths, 15 of which were sudden, unexpected and probably cardiovascular. All but two patients were normotensive postoperatively, and in 4 of these the cause of death was proven dissecting aneurysm of ascending aorta. In another patient this aneurysm was repaired surgically and in 3 other patients chest X-ray had shown a dilated ascending aorta before death. At follow-up the ascending aorta was dilated angiographically in 4 survivors, who had moderate systolic hypertension

Replacement of the ascending aorta and aortic valve with a composite graft. Results in 86 patients.

We reviewed our entire experience with composite graft replacement of the ascending aorta and aortic valve during a 63 month interval ending in December, 1979. Anuloaortic ectasia was the most common indication for operation, followed by aortic dissection (acute and chronic). Hospital mortality was 5% and was related to the preoperative functional status and the duration of intraoperative myocardial ischemia. Reoperation on the ascending aorta for reasons other than postoperative hemorrhage was required in five of the 82 hospital survivors (6%). By actuarial analysis, 90% of hospital survivors

Multiple aneurysms and aortic dissection: an unusual manifestation of Marfan's syndrome.

Annuloaortic ectasia and dissection of the ascending aorta are common cardiovascular complications of Marfan's syndrome. True aneurysms of the subclavian arteries in these patients are rare. We present an unusual case of a 37-year-old woman who over the span of 10 years sequentially developed an aneurysm of the ascending aorta (annuloaortic ectasia), a right subclavian artery aneurysm, a left axillary-subclavian artery aneurysm, and a dissection of the aortic arch. Aside from the aortic dissection, the aneurysms produced no symptoms. All vascular complications were detected with computed tomog

Aortic wrapping for stanford type A acute aortic dissection: short and midterm outcome.

BACKGROUND: Conventional surgical treatment of Stanford type A acute aortic dissection (AAD) is associated with considerable in-hospital mortality. As regards very elderly or high-risk patients with type A AAD, some may meet the criteria for less invasive surgery likely to prevent the complications associated with aortic replacement. METHODS: We have retrospectively analyzed a cohort of patients admitted to our center for Stanford type A AAD and having undergone surgery between 2008 and 2012. The outcomes of the patients having had an aortic replacement under cardiopulmonary bypass (group A)

Zone 0 TEVAR with femoral artery inflow for an anastomotic pseudoaneurysm.

We describe the case of an 89-year-old female with a distal anastomotic pseudoaneurysm of the ascending aorta after an ascending aorta replacement for an acute type A aortic dissection. Initially, we attempted endovascular repair using a semi-custom-made thoracic fenestrated stent graft. However, this treatment failed due to an endoleak. Two weeks later, we performed a total arch vessel debranching using femoral artery inflow and thoracic endovascular repair. Postoperative computed tomography revealed no signs of the endoleak. This hybrid approach could be an effective treatment option for ana

Adventitial adaptive immune cells are associated with ascending aortic dilatation in patients with a bicuspid aortic valve.

BACKGROUND: Bicuspid aortic valve (BAV) is associated with ascending aorta aneurysms and dissections. Presently, genetic factors and pathological flow patterns are considered responsible for aneurysm formation in BAV while the exact role of inflammatory processes remains unknown. METHODS: In order to objectify inflammation, we employ a highly sensitive, quantitative immunohistochemistry approach. Whole slides of dissected, dilated and non-dilated ascending aortas from BAV patients were quantitatively analyzed. RESULTS: Dilated aortas show a 4-fold increase of lymphocytes and a 25-fold increa

Interventional closure of artificial vascular anastomotic fistula after aortic replacement: a case report.

Two years ago, the patient suffered from type A aortic dissection. As a result, partial aortic dissection artificial vascular replacement and partial aortic arch artificial vascular replacement were performed. Six months after the operation, an anastomotic fistula in the ascending aorta was detected, which subsequently progressed to chronic heart failure of New York Heart Association (NYHA grade) class III. After eliminating the operation - related contraindications, the patient successfully had the fistula occluded through transcatheter ascending aorta - right atrial fistula in our hospital.

[Marfan syndrome].

Marfan syndrome is a genetic illness, generally linked to an anomaly in fibrillin type 1, which is transmitted in an autosomally dominant fashion. Its diagnosis is sometimes difficult and should always be evoked in the face of a non-atheromatous aneurysm of the ascending aorta, lead to family screening in the search for aneurysms in any relatives, and an ophthalmological and rheumatological examination in the search for extracardiac signs. The diagnosis made, the dilatation of the aorta can be limited by proscribing violent sports and prescribing beta-blockers. The diameter of the ascending ao
