Small bowel bleeding accounts for 5-10% of gastrointestinal (GI) hemorrhage and remains a diagnostic challenge because of its diverse etiologies and subtle clinical presentation. Advances in small bowel imaging have refined the diagnostic approach and have led to the replacement of the term obscure GI bleeding with suspected small bowel bleeding (SSBB) when findings on upper and lower endoscopy are unrevealing. The initial evaluation requires careful assessment of clinical history, comorbidities, medication use, and physical findings, which may help direct suspicion toward specific etiologies

INTRODUCTION: Hereditary polyposis syndromes are a group of inherited disorders associated with a high risk of developing colorectal cancer. The best known ones are familial adenomatous polyposis (FAP), Peutz-Jeghers (PJS), juvenile polyposis and Cowden syndromes, as well as conditions predisposing to cancer, such as Lynch syndrome. Some of them are characterized by an increased risk of small bowel polyps occurrence.
AREAS COVERED: Literature search in PubMed was performed in November 2022 and a narrative review was carried out. Since performing small bowel polypectomy is important in such pa

Small bowel involvement of Enterobius vermicularis infection is exceptionally rare, with few cases reported. A female presented with persistent abdominal pain for one month. Gastroscopy and colonoscopy were unremarkable, but computed tomography revealed localized small bowel wall thickening. Laboratory tests showed elevated eosinophil counts, though parasite antibody screening was negative. Double-balloon enteroscopy identified multiple shallow ulcers and a live worm in the small intestine, confirmed as Enterobius vermicularis on pathological examination. The patient was treated with anthelmin…

Endoscopy is often used in gastroenterology. It can prevent early recurrence of bleeding from esophageal varices. Computed virtual chromoendoscopy can diagnose pathological changes at the esophagogastric transitional zone in non-erosive reflux disease. In the buried bumper syndrome the internal bumper, which has migrate into the gastric wall, should be removed endoscopically. Double-balloon enteroscopy is one of the flexible enteroscopy techniques with an acceptable complication rate and remains the diagnostic and therapeutic gold standard for small bowel endoscopy. Hyperplastic polypoidal les…

A 25-year-old woman visited our department due to intermittent defecation of black stool and periumbilical pain for 2 years. Abdominal physical examination showed no obvious abnormality. Laboratory examination showed positive fecal occult blood. Because bleeding lesions were not found by gastroscopy or colonoscopy, she underwent double-balloon enteroscopy examination. It was found that a diverticulum was formed in the terminal ileum, with blood exudation. After repeated washing, the blood disappeared. However, after a period of time, blood exudation could still be detected there. She was diagn…

The case of a patient with neurofibromatosis type 1 with chronic abdominal pain and iron deficiency anemia is described. Diagnostic procedures including esophagogastroduodenoscopy and ileocolonoscopy did not disclose a definitive cause. CT scan and MRI revealed multiple intraluminal tumors in the small bowel, especially in the ileum. These findings were verified by double balloon enteroscopy. Endoscopic resection was not performed due to size and number of the polyps, and the patient was sent for diagnostic laparotomy. A conglomerate tumor of the ileum was resected. Histopathological analysis

Capsule endoscopies were performed in 6 cases, of which 3 showed positive findings. Double-balloon enteroscopy was conducted in one case and revealed an ileal diverticulum. A female patient in her 80s was referred to our hospital because of an ileal tumor identified by capsule endoscopy. FDG-PET suggested double intestinal tumors not only in the ileum but also in the jejunum. The patient has cancer past history including sigmoid colon, rectum, and endometrium, and also had cancer family history fulfilling the revised Amsterdam criteria. Double balloon enteroscopy disclosed two ulcerated tumors in the jejunum and the ileum. Biopsy was diagnosed as adenocarcinoma pathologically, and microsatellite instability-high (MSI-H) genetically. Surgical resection was perform

PURPOSE: Metastatic colorectal cancer to the small intestine is a rare condition presenting with nonspecific symptoms such as obscure gastrointestinal bleeding (OGIB). We report our experience with the use of capsule endoscopy (CE) and double-balloon enteroscopy (DBE) to identify and diagnose the metastatic colorectal cancer to the small bowel. The aim was to demonstrate the approach to the diagnosis of metastatic colorectal cancer to the small bowel including literature review.
METHODS: This is a retrospective case series from an academic tertiary referral center. Three patients with a histo…

The endoscopic evaluation and management of small intestinal diseases continue to evolve and expand. The advent of small bowel wireless capsule endoscopy and deep enteroscopy with either a double- or single-balloon enteroscope now allows complete endoscopic visualization of the entire small intestine and enables access for endoscopic interventions such as biopsies or hemostasis for most of the small bowel. New endoscopic techniques are available to treat proximal malignant small bowel obstruction, including intraluminal stents and endoscopic gastrojejunal stents. Emerging technologies also aim

Primary small intestinal lymphoma (PSIL) is a relatively rare form of non-Hodgkin lymphoma, often complicated by bleeding, obstruction, or perforation of the intestine during the clinical course. The initial diagnosis and management of these complications are often difficult in PSIL, because the small intestine is usually inaccessible in routine endoscopy. Recently, total enteroscopy with a double-balloon method, called double balloon endoscopy (DBE), has been developed for the diagnosis and treatment of small intestinal disorders. We report herein on 4 cases of PSIL (2 diffuse large B-cell ly

BACKGROUND AND STUDY AIM: Overtube-assisted enteroscopy (OAE) techniques have increased the ability to perform endoscopic retrograde cholangiopancreatography (ERCP) in patients with altered upper gastrointestinal anatomy, such as Roux-en-Y gastric bypass. The aim of this study was to compare the efficacy and safety of OAE-ERCP in patients with different configurations of upper gastrointestinal anatomy.
PATIENTS AND METHODS: A systematic review was performed following a literature search for papers published between 1966 and August 2013. The following databases were searched: MEDLINE (via PubM

Case of a 24-year-old woman presenting due to edema in lower extremities. The patient had had infectious mononucleosis three weeks prior and had medical history of suspicion of Crohn's disease (CD) (due to a non-specific ileocolitis in a colonoscopy/EnteroRM). No ongoing medication. Laboratory evaluation unveiled hypoproteinemia with severe hypoalbuminemia, no renal abnormalities. A PLE was assumed, with post-infectious or CD being the most likely culprits. Alternative causes were extensively excluded. A videocapsule revealed white-tipped or granular villi, some white nodular villi and diffuse
