Gender differences in risk factors for aberrant prescription opioid use.

UNLABELLED: This is a longitudinal predictive study to examine gender differences in the clinical correlates of risk for opioid misuse among chronic pain patients prescribed opioids for pain. Two hundred seventy-five male and 335 female patients prescribed opioids for chronic noncancer pain were asked to complete a series of baseline questionnaires, including the revised Screener and Opioid Assessment for Pain Patients (SOAPP-R). After 5 months, the subjects were administered a structured prescription drug use interview (Prescription Drug Use Questionnaire; PDUQ) and submitted a urine sample f

Effects of virtual patient race and opioid misuse on physicians' opioid stewardship strategies and nonopioid treatments for pain.

OBJECTIVE: The aim of the current study was to determine whether and how physicians' use of opioid stewardship (OS) strategies and nonopioid, multimodal treatments differed by patient race and prior opioid misuse behaviors for patients with chronic pain. METHOD: Physicians (N = 135) made pain care decisions for eight virtual patients with chronic pain who varied by race (Black, White) and prior opioid misuse behaviors (adherent, misuse). For each patient, physicians indicated their use of OS strategies (agonist medication, urine drug testing, prescription drug monitoring, opioid contract, and

Using Social Media to Examine Themes Surrounding Fentanyl Misuse and Risk Indicators.

Background: Opioid misuse is a crisis in the United States, and synthetic opioids such as fentanyl pose risks for overdose and mortality. Individuals who misuse substances commonly seek information and support online due to stigma and legal concerns, and this online networking may provide insight for substance misuse prevention and treatment. We aimed to characterize topics in substance-misuse related discourse among members of an online fentanyl community. Method: We investigated posts on a fentanyl-specific forum on the platform Reddit to identify emergent substance misuse-related themes pot

Benzodiazepine misuse in adults with alcohol use disorder: Prevalence, motives and patterns of use.

OBJECTIVE: Benzodiazepines are among the most commonly misused drugs. Despite the known risks of combining benzodiazepines and alcohol, little is known about misuse among people with alcohol use disorder (AUD). Our aim was to characterize the prevalence, correlates, and patterns of misuse of benzodiazepines in adults with AUD. METHOD: Adults receiving treatment for AUD (N = 258) completed a battery of questionnaires. We used descriptive statistics to characterize the prevalence and patterns of misuse and we used logistic regression models to identify correlates of misuse. RESULTS: Almost hal

Building a city wide service for exchanging needles and syringes.

How best can injecting drug misusers obtain clean injecting equipment in a city where drug injecting is widespread? An exchange service for needles and syringes throughout Glasgow has been established in health centres and clinics in the evening. Over the past four years seven new exchanges have been opened and over 2700 injecting drug misusers have attended. Attendances rose from under 1000 in 1988 to 28,000 in 1992. The exchanges also provide a wide range of other health and social services. Public hostility to the exchanges has abated. During the same period equipment sharing in the city di…

Factors Associated with Benzodiazepine Misuse Among Those Who Misused Opioids Within the Past Year: A U.S. National Analysis.

INTRODUCTION: Rarely lethal in isolated ingestion, benzodiazepines show an increasing degree of morbidity and mortality associated with overdose when co-ingested with opioids. Several studies have identified a limited number of associated risk and protective factors surrounding concomitant use. Of those studies that used national data, most focused on the consequences of benzodiazepine and opioid co-ingestion rather than risk factors associated with their misuse. Consequently, we aim to elucidate risk factors associated with benzodiazepine and opioid misuse using national data. METHODS: This

Opioid Use and Misuse and Suicidal Behaviors in a Nationally Representative Sample of US Adults.

Prior research has shown associations between opioid misuse and suicidal behaviors, but the relationship between medical opioid use and suicidal behaviors is not known. We assessed associations of opioid use and misuse with suicidal ideation, suicide plans, and suicide attempts among adults aged 18-64 years (n = 86,186) using nationally representative cross-sectional data from the 2015 and 2016 administrations of the National Survey on Drug Use and Health. We used logistic regression to estimate associations between opioid use/misuse and suicidal behaviors and propensity score-weighted logisti

The epidemiology of prescription fentanyl misuse in the United States.

BACKGROUND: US opioid overdose deaths continue to climb, with a 12.0% increase from 2016 to 2017. Fentanyl, a synthetic opioid, has been a major contributor to opioid-related overdose deaths. While fentanyl-related overdose is driven by illicit fentanyl, little is known about individuals who misuse prescription fentanyl, which is also linked to elevated overdose and mortality risk. This work aimed to fill that gap through analyses of prescription fentanyl misuse correlates. METHODS: Data were from the 2015-16 National Survey on Drug Use and Health (N = 114,043), a nationally representative su

Incidence of opioid misuse by cigarette smoking status in the United States.

BACKGROUND: The combination of opioid misuse and cigarette smoking contributes to increased morbidity and mortality compared to each substance use alone. We estimated the incidence of opioid misuse for persons who currently or formerly smoked versus never smoked. METHODS: Data came from the 2015-2020 National Surveys on Drug Use and Health cross-sectional surveys of US civilians aged 12+ (n = 315,661). Weighted opioid misuse incidence and average time between cigarette use initiation and opioid misuse initiation were calculated annually by smoking status. Logistic regression models tested tim

The need to monitor emerging issues in etomidate usage: the misuse or abuse potential.

Since 2011, the misuse or abuse of etomidate has gradually increased when propofol was designated a controlled drug under the Narcotics Control Act in Korea. Accordingly, the Ministry of Food and Drug Safety announced that etomidate would be under the 'Regulation on the designation of drugs that may cause concerns of misuse or abuse' rule in June 2020, which is less stringent than the Narcotics Control Act. Therefore, this review investigates potential misuse or abuse cases of etomidate to consider strengthening its management. A literature search was conducted to compare etomidate with other

Graded Associations of Tranquilizer Use and Misuse with Suicidal Thoughts and Behaviors in a National Sample of U.S. Veterans.

U.S. military veterans face an elevated suicide risk compared to the general population. Both the use and misuse of tranquilizers (i.e. several classes of medications that treat anxiety and muscle spasms via central nervous system action, including benzodiazepines) have been associated with suicidal thoughts and behaviors among veterans. However, it remains unclear whether risk varies across different patterns of tranquilizer use. Participants included 12,584 U.S. military veterans (13.0% female; 78.1% non-Hispanic White; 49.3% age 65 and older), from the 2015-2019 National Survey on Drug Use

Prescription medicine misuse in the Asia-Pacific region: An evolving issue?

Prescription medicine misuse, especially misuse of opioids has become a major public healthcare issue in many developed countries such as the USA and Australia where this is associated with significant morbidity (Emergency Department visits due to acute toxicity) and mortality. In this review, we looked at the available data obtained from peer-reviewed articles and population surveys to gain an insight into the current situation in the Asia-Pacific region. There is currently limited information available, but data from subpopulation surveys in a number of countries suggests that prescription m
