AIMS: To examine early adverse pulmonary effects of exposure to cotton dust, and to identify potential risk factors, including atopy for pulmonary responses to cotton dust.
METHODS: Spirometry, methacholine challenge testing, and questionnaire; performed among 101 non-smoking newly hired textile workers at baseline (prior to starting work), and at 3, 12, and 18 months after starting work. Exposure to wood dusts may cause various skin and mucosal symptoms. We examined endotoxin exposure and wheezing episodes during the first year of life in a birth cohort of 499 infants with one or both parents having a history of asthma or allergy. We measured endotoxin in settled dust from the baby's bed, bedroom floor, family room, and kitchen floor within the first 3 mo after birth. In a 37-year-old worker employed in a coffee roastery who suffered from work-related rhinoconjunctivitis, allergologic investigations demonstrated sensitization to the dust of the green, unroasted coffee bean. This particular allergy is uncommon in Switzerland. The case is discussed and the literature on the subject is reviewed.

Pulmonary Siderosis results from inhalation of iron dust or fumes. It falls into the group of pneumoconioses in which the pulmonary reaction is minimal despite a heavy dust load. Since fibrosis is not caused by inhalation of iron dust, the clinical course is benign and pulmonary function tests and blood gases are within normal limits. We report the first case of sideropneumoconiosis from Israel, in a 39-year-old iron welder. Since this condition is still not recognized as an occupational disease in Israel, the aim of this presentation is to bring it to appropriate medical attention.

In this study (1980-1987) the Authors considered the incidence of sensitization to house dust in patients with respiratory allergic disease. The Authors identified 399 subjects with sensitization to house dust out of 1825 subjects sensitized to at least one of the tested allergens (21.9%). The number of patients exclusively sensitized to house dust resulted very small (13 patients, 0.7%). Atopic sensitization of children in the Stockholm area to house dust mites (HDM) was investigated in a case-control study. Sixty children with and 60 without positive skin prick tests for HDM were matched for age and sex. HDM-sensitized children had previously more often lived in other areas known to be mite infested than the control children. We report 2 cases of aluminium sensitivity in adults. Each time, the means of sensitization was the inoculation of aluminium-precipitated pollen or dust extracts for hyposensitization. We conclude that aluminium allergy is not exceptional and that in these cases one can observe allergic reactions to the Finn Chamber tests.

Organic dusts present in the farm environment may cause a series of lung cell reactions involving inflammation and immune responses. Clinical diseases related to this exposure and based on the inflammation may be divided into acute and chronic inflammation. These can be accompanied by airway hypersensitivity. Individual clinical cases often present atypical or mixed symptoms that must be recognized as a work-related disease.

Cement dust has been found to be a source of selenium when included in the rations of sheep. In the work reported, rations including 3% by weight of cement dusts were fed to sheep for 165 days. The dust were taken from 3 collectors located at progressively greater distances from the coal-fired end of the kiln. Selenium levels and blood and tissues of the animals fed the cement dusts increased significantly above those of the control sheep. Adolescent abuse of phencyclidine, also known as "ANGEL DUST" or "PCP," has increased dramatically in the past several years. Phencyclidine can be highly toxic, and efforts should be made to prevent its abuse. The authors advance one model of behavorioral intervention which utilizes peer group counseling to promote learning of practical skills for resisting pressures to use "hard drugs," especially phencyclidine.

Bacterial contamination of various parts of the cotton plant and of cotton from different mills was investigated. The predominant bacterial species were Gram-negative rods mainly of the Enterobacter genus. When guinea pigs inhaled strains of these bacteria cultivated from cotton, a strong leucocyte mobilising capacity was found for Pseudomonas and Enterobacter but not for Agrobacterium or Bacillus species. The aetiology of the development of pulmonary symptoms after inhalation of bacteria-containing dusts and subsequent production of endotoxins is discussed.
