Early postoperative walking recovery using a connected bodyweight-supported treadmill after hip fracture surgery in older patients: a retrospective cohort study (STAND-UP).

BACKGROUND: Early mobilization to promote functional recovery and reduce the adverse effects of immobilization is one of the main goals of acute postoperative management of hip fracture (HF). The primary objective of this study was to assess, in usual care, the association between the use of a bodyweight-supported treadmill (BWST) and early postoperative walking recovery in older adults after hip fracture surgery. Secondary objectives were to evaluate delays to first sitting and first walking, daily walking distances, functional outcomes, length of stay, discharge destination, mortality, and t

Impact of an early mobilization protocol on the reduction of medical complications after surgery for chronic subdural hematoma: the GET-UP Trial.

OBJECTIVE: Timing of mobilization after chronic subdural hematoma (cSDH) surgery is highly heterogeneous among neurosurgical centers. Past studies have suggested that early mobilization may reduce medical complications without increasing recurrence, but evidence remains scarce. The purpose of this study was to compare an early mobilization protocol with a 48-hour bed rest practice, with a focus on the occurrence of medical complications. METHODS: The GET-UP Trial is a prospective, randomized, unicentric, open-label study with an intention-to-treat primary analysis designed to evaluate the imp

A Novel Limb Salvage Technique of External Fixation Protection of Lower Extremity Plastic Reconstructions with Immediate Postoperative Ambulation (Bibbo Flap and Frame Technique).

Non-weight bearing is mandatory after soft tissue reconstructions of the weight-bearing and the high-pressure areas in the lower extremity. The most common method of patient mobilization after surgical reconstruction of chronic foot and ankle wounds has been to place patients non-weight bearing with crutches, walkers, or a wheelchair. Often patients are older, have more complex medical comorbidities, are deconditioned, and simply cannot comply with the prescribed weight-bearing status with these methods, which leads to deconditioning, depression, or noncompliance. Noncompliance quickly leads t

Pediatric Mobility: The Development of Standard Assessments and Interventions for Pediatric Patients for Safe Patient Handling and Mobility.

Due to improvements in biomedical sciences and clinical expertise, childhood mortality has decreased. Pediatric patients experience similar complications from inactivity as adults, such as hospital-acquired conditions and delirium. Interdisciplinary collaboration is imperative to improve functional and developmental outcomes of children who suffers from either a chronic illnesses or an acute illness that results in a prolonged hospitalization. Pediatric nursing assessments should include gross motor function related to mobility. Activities to mobilize pediatric patients should be based on age

A Novel Technique of Paravertebral Thoracic and Preperitoneal Analgesia Enhances Early Recovery After Oesophagectomy.

BACKGROUND: Excellent analgesia following oesophagectomy facilitates patient comfort, early extubation, physiotherapy and mobilisation, reduces post-operative complications and should enhance recovery. Thoracic epidural analgesia (TEA), the gold standard analgesic regimen for this procedure, is often associated with systemic hypotension treated with inotropes or fluid. This may compromise enhanced recovery and be complicated by anastomotic ischaemia or tissue oedema. METHODS: We report a novel analgesic regimen to reduce post-operative inotrope usage. Infusion of ropivicaine via bilateral pre

Early ankle movement versus immobilization in the postoperative management of ankle fracture in adults: a systematic review and meta-analysis.

STUDY DESIGN: Systematic review and meta-analysis. OBJECTIVES: To compare early ankle movement versus ankle immobilization after surgery for ankle fracture on clinical and patient-reported outcomes. BACKGROUND: A significant proportion of patients undergoing surgery for ankle fracture experience postoperative complications and delayed return to function. The risks and benefits of movement of the ankle in the first 6 weeks after surgery are not known, and clinical practice varies widely. METHODS: We searched bibliographic databases and reference lists to identify eligible trials. Two indepen

Surgical versus conservative treatment for acute injuries of the lateral ligament complex of the ankle in adults.

BACKGROUND: Inversion injuries, primarily sprains, of the ankle are one of the most commonly treated injuries. The three main treatment modalities for acute lateral ankle ligament injuries are immobilisation with plaster cast or splint, 'functional treatment' comprising early mobilisation and the use of an external support (e.g. ankle brace or taping), and surgical repair or reconstruction. OBJECTIVES: We aimed to compare surgical versus conservative treatment for acute injuries of the lateral ligament complex of the ankle in adults. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal

[Treatment of inguinal hernia using laparoscopy. Clinical experiences apropos of 300 cases].

300 laparoscopic treatments of inguinal hernia repair have be done in our hospital between may 1992 and juin 1994. 134 times we have seen a median hernia and 166 times a lateral one. In 44 patients (14%) we did the repair of hernia for bilateral disease. In 66 cases we underwent for recidive; 55 times after hernia repair by the Bassini technique or Shouldice technique, and only 8 times after laparoscopic repair. In 1.4% we have seen early recidivism, no more then in the classical technique. Of course philosophy of this technique is very different to the traditional repair of inguinal hernia, b

Prosthetic programme after above-knee amputation in children with sarcomata.

A programme for early mobilisation using a temporary prosthesis was evaluated in 17 children who had had an amputation above the knee for sarcomata. The temporary prosthesis had a performed adjustable polypropylene quadrilateral socket which was able to accommodate changes in the size of the stump during the first few months after amputation. The adjustable sockets were assembled onto wooden knee-shank-foot units or onto modular components covered with foam. The wooden units were better for routine use as more adjustment was possible between the socket and the knee and because they were more d

Agreement between ambulance paramedic- and physician-recorded neurological signs with Face Arm Speech Test (FAST) in acute stroke patients.

BACKGROUND AND PURPOSE: Patients with suspected stroke first assessed by ambulance paramedics require early recognition to facilitate appropriate triage and early treatment. We determined paramedic's accuracy in detecting acute stroke signs by comparing agreement between neurological signs recorded in the Face Arm Speech Test (FAST), a stroke recognition instrument, by paramedics on the scene and by stroke physicians after admission. METHODS: Suspected stroke patients admitted by ambulance paramedics directly to an acute stroke unit through a rapid ambulance protocol were examined by a traine

Prediction of independent ambulation in subacute stroke patients: a retrospective observational study using lower limb motor and balance assessments.

OBJECTIVE: To investigate whether motor and balance assessments predict independent ambulation in patients with subacute stroke. DESIGN: Retrospective observational study. SUBJECTS/PATIENTS: 109 patients admitted for inpatient rehabilitation within 3 months of stroke onset. METHODS: Independent ambulation was defined as a Functional Ambulation Category score of 4 or higher on discharge. The Fugl-Meyer Assessment for Lower Limb and Berg Balance Scale were administered on admission. Associations with walking independence on both admission and discharge were examined using binary logistic regr

International retrospective natural history study of LMNA-related congenital muscular dystrophy.

Muscular dystrophies due to heterozygous pathogenic variants in LMNA gene cover a broad spectrum of clinical presentations and severity with an age of onset ranging from the neonatal period to adulthood. The natural history of these conditions is not well defined, particularly in patients with congenital or early onset who arguably present with the highest disease burden. Thus the definition of natural history endpoints along with clinically revelant outcome measures is essential to establishing both clinical care planning and clinical trial readiness for this patient group. We designed a larg

The 30-day survival and recovery after hip fracture by timing of mobilization and dementia : a UK database study.

AIMS: The aim of this study to compare 30-day survival and recovery of mobility between patients mobilized early (on the day of, or day after surgery for a hip fracture) and patients mobilized late (two days or more after surgery), and to determine whether the presence of dementia influences the association between the timing of mobilization, 30-day survival, and recovery. METHODS: Analysis of the National Hip Fracture Database and hospital records for 126,897 patients aged ≥ 60 years who underwent surgery for a hip fracture in England and Wales between 2014 and 2016. Neurology Care, Diagnostics, and Emerging Therapies of the Patient With Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy is the most common form of childhood muscular dystrophy. A mutation in the DMD gene disrupts dystrophin (protein) production, causing damage to muscle integrity, weakness, loss of ambulation, and cardiopulmonary compromise by the second decade of life. Life expectancy has improved from mid-teenage years to mid-20s with the use of glucocorticoids and beyond the third decade with ventilator support and multidisciplinary care. However, Duchenne muscular dystrophy is associated with comorbidities and is a fatal disease. Glucocorticoids prolong ambulation, but their side
