Arthroscopic Management of Elbow Osteoarthritis.

The incidence of osteoarthritis in the general population is low, but it can be seen in manual laborers, throwing athletes, and people dependent on crutches and wheelchairs. Patients often complain of pain at the terminal extents of motion, and imaging shows osteophyte formation at the tips of the coronoid and olecranon processes as well as thickening of the bone between the coronoid and the olecranon fossae. Recent advances in arthroscopic instrumentation and techniques have led to a growing interest in the arthroscopic treatment of elbow osteoarthritis. This article provides a review of basi

Arthroscopic Management of Elbow Fractures.

Several types of elbow fractures are amenable to arthroscopic or arthroscopic-assisted fracture fixation, including fractures of the coronoid, radial head, lateral condyle, and capitellum. Other posttraumatic conditions may be treated arthroscopically, such as arthrofibrosis or delayed radial head excision. Arthroscopy can be used for assessment of stability or intra-articular fracture displacement. The safest portals are the midlateral (soft spot portal), proximal anteromedial, and proximal anterolateral. Although circumstances may vary according to the injury pattern, a proximal anteromedial

"Sensory Switching" in Elbow Reconstruction.

In the treatment of the soft tissue defect of the elbow, flap reconstruction is necessitated in many cases because of thinness of soft tissue at this region. In addition, reacquirement of tactile sensation is desirable because of the anatomical and specific functions of the elbow. Of three cases treated for elbow defects, one was reconstructed with a pedicled island forearm flap containing the lateral cutaneous nerve of the forearm, another was reconstructed with a venoneuro-accompanying artery fasciocutaneous flap (VNAF flap) containing the basilic vein, and the third with the VNAF flap conta

Elbow rehabilitation in traumatic pathology.

The elbow, intermediate joint of the upper limb, frequently undergoes to pathological events and is especially prone to stiffness. Rehabilitation plays an important role in recovering functional activities. For the rehabilitation team, this goal always represents a challenge, as the treatment has to be continuously modeled and calibrated on the needs of the individual patient, even many times during the same rehabilitation cycle. Containing the effects of immobilization, avoiding to excessively stress the healing tissues, satisfying specific clinical criteria before moving to the next rehabili

Simultaneous shoulder and elbow dislocation.

Ipsilateral shoulder and elbow dislocation is very rare and only six articles are present in the literature mentioning this kind of a complex injury. With this presentation we aim to emphasise the importance of assessing the adjacent joints in patients with trauma in order not to miss any accompanying pathologies. We report a case of a 43-year-old female patient with ipsilateral right shoulder and elbow dislocation treated conservatively. The patient reported elbow pain when first admitted to emergency service but she was diagnosed with simultaneous ipsilateral shoulder and elbow injury and tr

Charcot's Elbow Following Syringomyelia: Revisited.

Neuropathic arthropathy of the elbow joint is uncommon with a reported incidence of 3-8%only. The natural history, pathogenesis and management approaches are debatable with an absence of clear guidelines. We report two cases of Charcot's elbow following syringomyelia with ulnar nerve manifestations. One patient required debulkingsynovectomy and ulnar nerve decompression while the other was managed conservatively. The article aims to enrich the limited knowledge of managing Charcot's elbow.

Elbow lateral collateral ligament injuries.

The lateral collateral ligament (LCL) of the elbow is a complex capsuloligamentous structure critical in stabilizing the ulnohumeral and radiocapitellar articulations. LCL injury can result in elbow instability, allowing the proximal radius and ulna to externally rotate away from the humerus as a supination stress is applied to the forearm. Elbow dislocation is the most common cause of LCL injury, followed by iatrogenic injury. LCL pathology resulting in late recurrent instability is rare but disabling. The diagnosis requires a high index of suspicion, detailed history, and focused physical ex

[Open arthrolysis of elbow stiffness].

According to the literature approximately 5% of all elbow injuries result in elbow stiffness. Following pathomorphological aspects elbow stiffness can be classified into intrinsic (intra-articular) and extrinsic (extra-articular) types. Mixed forms are common. Patient evaluation should include age, degree of stiffness, predisposition and anamnesis of professional and leisure time. Basic diagnostics for planning surgical treatment routinely include clinical evaluation, x-rays and computed tomography. Before indications for open arthrolysis of the elbow are given conservative treatment of more t

Pediatric complex divergent elbow dislocation.

A divergent dislocation of the elbow is a very rare injury, and only a few cases have been described in the literature. It is characterized as a dorsal dislocation of the ulnohumeral joint combined with a lateral dislocation of the proximal radius. All three articulations of the elbow joint are involved. Like in our case, it can be accompanied by an avulsion fracture of the coronoid and a distal radius fracture. For correct understanding of the injury, proper radiographic studies are imperative. In contrast to some earlier reports that advise a conservative approach, we performed a very aggres

Osteocapsular debridement for elbow arthritis.

Open capsular debridement is an excellent option for the treatment of elbow arthritis. This technique is particularly indicated in a patient population physiologically younger than 60 years. Given the young age and high functional demand of patients with primary osteoarthritis of the elbow, prosthetic replacement is generally not recommended. Open capsular debridement preserves the native joint and thus does not inherently require permanent activity modification as does replacement arthroplasty.

Arthroscopic management of stiff elbow.

Elbow stiffness is a common problem encountered by orthopedic surgeons. Various management options have been described in the literature, including conservative measures and open and arthroscopic surgery. Arthroscopic management of stiff elbow remains controversial. The purpose of this study was to evaluate the functional results of arthroscopic management of stiff elbow.Thirty patients with stiff elbow underwent arthroscopic release surgery and were followed up for an average of 27.3 months. Surgery included anterior and posterior capsular release, coronoid process debridement, bony spur exci

Management of complex elbow instability.

Complex elbow instability is a challenging injury even for expert elbow surgeons. The preoperative radiographs should be carefully evaluated to recognize all lesions that may occur in complex elbow instabilities. Recognizing all the possible lesions is critical to achieve an optimal outcome. The most common types of injuries are as follows: (1) radial head fractures associated with lateral and medial collateral ligaments lesions (with or without elbow dislocation); (2) Coronoid fractures and lateral collateral ligament lesion (with or without elbow dislocation); (3) Terrible Triad; (4) Transol

Nerve injuries about the elbow.

The ulnar, radial, median, medial antebrachial cutaneous, and lateral antebrachial cutaneous nerves are subject to traction and compression in athletes who place forceful, repetitive stresses across their elbow joint. Throwing athletes are at greatest risk, and cubital tunnel syndrome (involving the ulnar nerve) is clearly the most common neuropathy about the elbow. The anatomy and innervation pattern of the nerve involved determines the characteristic of the neuropathy syndrome. The most important parts of the work-up are the history and physical examination as electrodiagnostic testing and i

Acute elbow dislocations in athletes.

Elbow dislocation in the athletic population is not an uncommon injury. The literature does not clearly establish treatment guidelines, with treatment being extrapolated from the experience in the general population. A short period of immobilization with early range of motion exercises limits disability and allows return to sports participation within 6 weeks.

Congenital differences about the elbow.

The elbow is frequently affected by congenital differences that limit flexion-extension and supination-pronation. The limitation can occur in utero or develop over time. Delayed causes usually gradually disrupt the anatomy and function of the paired bone forearm. Accurate diagnosis is based on a careful examination and perusal of radiographs. Certain diagnoses can be helped by surgery and will result in improved limb usage and function. In contrast, other diagnoses are recalcitrant to modern operative techniques but will benefit from occupational therapy and adaptive equipment to enhance indep

[Pediatric "floating elbow"--case reports].

Floating elbow is an injury that is characterized by simultaneous fracture of humerus and forearm of the same extremity. The injury is caused by high energy (falls, car accidents) and is often associated with extensive soft tissue contusion of affected limb and there may be present lesion of neurocircular bundle. Operative treatment (minimally invasive percutaneous pinning) should be performed as soon as possible after patient's admission and this treatment should be preferred before circular casting, which may cause compartment syndrome. In the paper we present two cases of pediatric floating

Elbow dislocation with arterial injury.

Elbow dislocations are most often the result of a hyperextension mechanism with the forearm levered and displaced posterior to the humerus. Recurrent instability and arterial disruption are uncommon sequelae of this injury. Any asymmetry in distal pulses after joint reduction demands a study of arterial anatomy (arteriogram or duplex scan) to rule out avulsion or intimal damage.
