Clavicular osteotomy to assess subclavian vein injury during internal fixation of midshaft clavicle fractures.

Midshaft clavicle fractures are commonly fixed with locking plates. The subclavian vein risks injury during this procedure and the consequences can be fatal. The purpose of this present study is to describe a clavicular osteotomy technique in order to equip orthopaedic surgeons with a means of rapidly accessing a subclavian vein injury. The osteotomy should only be performed following an urgent intraoperative vascular surgery assessment. There must be shared consensus from both orthopaedic and vascular surgery that direct repair of the subclavian vein is necessary, and further exposure is requ

[Work-related disease].

Work-related health complaints occur frequently. Occupational health departments and primary and secondary care physicians are confronted with these patients. Awareness of work-related disease is crucial. Three case histories with work-related diseases are described. The first concerned a 39-year-old woman who showed acute neurotoxicity due to inhalation of formaldehyde and methanol after opening a coffin with an embalmed corpse. Within weeks the symptoms diminished and complete recovery occurred within nine months. The second was a 61-year-old carpenter who experienced acute and chronic pulmo

Phylogenetic variation in flexor digitorum brevis: a Nepalese cadaveric study.

Understanding muscle architecture of the foot may assist in the design of surgical procedures such as tendon transfer, biomechanical modeling of the foot, prosthesis design, and analysis of foot function. Flexor digitorum brevis (FDB) is the most superficial intrinsic muscle of the sole of the foot. When compared to the little finger, the little toe has less function and opposition is absent in humans. Hence, the muscles acting on the little toe are undergoing evolutionary changes. The objective of this study was to review the phylogenetic variation occurring in FDB in the Nepalese cadavers. S

The anatomy and reconstruction of the medial patellofemoral ligament.

INTRODUCTION: Reconstruction of the MPFL for recurrent and acute lateral dislocation of the patella is becoming more accepted. This study aimed to further define the medial patellofemoral ligament (MPFL) anatomy and to describe the most suitable sites for graft attachment when reconstructing the MPFL. MATERIALS AND METHODS: Anatomical dissection was performed on 25 embalmed knee specimens. Four separate specimens were used to assess graft fixation points. Sites were tested for isometricity from 0 to 120 degrees of knee flexion. Length patterns were considered isometric if there was less than

Anatomical bases of superior gluteal nerve entrapment syndrome in the suprapiriformis foramen.

Observation of a 60 year-old-man with superior gluteal nerve (SGN) entrapment neuropathy in the suprapiriformis foramen encouraged us to explore, through anatomical dissection, the possible morphological etiologies of this condition. Ten SGNs in five embalmed cadavers were dissected via gluteal and pelvic access. The origin, course and distribution of the nervous trunk and its relations were studied. In most cases, the nerve fibers of the SGN arose from ventral branches of L4, L5 and S1 to constitute the nervous trunk in the pelvis, then reached the gluteal area and divided into two branches,

Anatomical basis of the posterior brachial skin flap.

The posterior brachial flap is a vascularized skin flap originally developed by AC Masquelet in 1982. Anatomical study in 37 non embalmed cadavers demonstrated the value of this flap which is taken from the posterior surface of the arm. The artery of the flap originates from the brachial or deep brachial artery and was present in all 37 subjects studied. This artery traverses the aponeurosis of the triceps brachii near the termination of the tendon of the teres major. The pedicle showed a mean length of 4.4 cm and mean diameter of 1.5 mm thus allowing satisfactory microsurgical anastomosis to

Educational training in laparoscopic gynecological surgery based on ethanol-glycerol-lysoformin-preserved body donors.

PURPOSE: Educational training of laparoscopic skills performed on body donors is considered the gold standard prior to surgery in living patients. Appropriate, safe, and feasible fixation of body donors, reflecting true-to-life conditions of tissues, is an essential prerequisite for training workshops. MATERIALS AND METHODS: Here we describe and evaluate a modified ethanol-glycerol-lysoformin-based fixation technique. Body donors were fixed by perfusion (70% ethanol, 30% glycerol, 0.3% lysoformin; ca. 20l) via femoral artery and stored in a humid atmosphere (1% thymol) at 4°C. Technical equip

Analysis of 132 submandibular salivary glands using the Randox Evidence Investigator and Randox DOA ULTRA WB array.

Occasionally, obtaining an adequate or acceptable postmortem blood specimen for drug analysis is not possible due to factors such as decomposition, exsanguination, or embalming. Submandibular salivary gland tissue, one of three major types of salivary gland tissue in the oral cavity of humans, has been reported to be a viable alternative postmortem specimen for toxicological testing. In this study, we evaluated the performance of the Randox Evidence Investigator instrument and Randox DOA (Drugs of Abuse) Ultra Whole Blood Array for the semi-quantitative determination of 21 immunoassays in an a…

Occupational Exposure to Formaldehyde and Cancer Risk Assessment in an Anatomy Laboratory.

Dissecting a human cadaver is an irreplaceable practice in general training of medical students. Cadavers in anatomy laboratories are usually preserved in formalin, an embalming fluid whose basic component is formaldehyde (FA). The aim of this study is to assess the cancer risk of employees and students that are exposed to FA based on the results of three monitoring campaigns, as well as to suggest permanent solutions to the problem of FA exposure based on the results obtained. Three sampling campaigns of formaldehyde concentration in indoor environments were conducted at five different locati…

[Consequences of autopsies for the living : Causes of death in the clinical diagnosis "septic and toxic shock"].

BACKGROUND: There is reason to believe that the diagnosis of septic and toxic shock, as indicated on the death certificate, cannot be confirmed as the cause of death without autopsy and subsequent histological analysis. The external examination of the corpse can therefore not represent the sole basis for a reliable statement about the infection status of a corpse, e. g. as a prerequisite for embalming. MATERIAL AND METHODS: The validity of autopsy in determining septic and toxic shock as the cause of death is demonstrated in 7 exemplary cases. RESULTS: Decades of experience in a university p…

Anatomical considerations for nerve transfer in axillary nerve injury.

This study investigated the anatomical details of the axillary and radial nerves in 50 upper limbs from 29 adult formalin-embalmed cadavers, and ten fresh upper limbs. The focus was on understanding the course, division, and ramifications of these nerves to improve treatment of shoulder dysfunction caused by axillary nerve damage. The axillary nerve divided anteriorly and posteriorly before passing the quadrangular space in all specimens, with specific distances to the first ramifications. It was found that the deltoid muscle's clavicular and acromial parts were always innervated by the anteri

Anatomical validation of needle placement for cervical paraspinal mapping of the multifidus.

INTRODUCTION/AIMS: The paraspinal muscles are an important component of electrodiagnostic testing for radiculopathy. In the cervical region, a protocol for cervical paraspinal mapping assumes accurate placement of the electromyography (EMG) needle into the cervical multifidus. However, there is scant information regarding the accuracy of needle placement in that muscle. This study examines the accuracy of this protocol in sampling the intended multifidus muscle of cadavers. METHODS: An experienced electromyographer directed needles to multifidus at C5, C7, and T2 spinous processes of 19 embal

Low CERVICAL-2 plane block, a proposed regional anesthetic technique for acute cholecystitis.

INTRODUCTION: Acute cholecystitis is a painful inflammatory disease of the gallbladder. The Low Cervical-2 Plane Block is a retrolaminar block that targets the C3, C4, and C5 spinal nerves, which provide sensory innervation to the gallbladder, in order to potentially provide analgesia to patients with pain associated with acute cholecystitis. METHODS: In this brief report, a softly embalmed cadaver was injected with a dye mixture bilaterally. RESULTS: Subsequent cadaveric dissection revealed spread of the injectate deep to the prevertebral fascia to the C4 lamina on the right side and the C5

Thyroidea ima artery multiple branching pattern over the trachea.

PURPOSE: The current cadaveric report describes a rare case of a thyroidea ima artery (TIA) with multiple branching pattern over the trachea. METHODS: A cadaver dissection of the neck and thorax region of a formalin-embalmed 90-year-old male cadaver of a body donor took place. The body donation was made after a signed informed consent. RESULTS: The TIA variant originated from the brachiocephalic artery before its bifurcation into the right common carotid artery (CCA) and right subclavian artery (SCA). TIA further divided into three anterior and two posterior branches, with subsequent multipl

The three muscle layers in the pyloric sphincter and their possible function during antropyloroduodenal motility.

This study was conducted to determine the muscular arrangement of the human pyloric sphincter using a comprehensive approach that involved microdissection, histology, and microcomputed tomography (micro-CT). The stomachs of 80 embalmed Korean adult cadavers were obtained. In all specimens, loose muscular tissue of the innermost aspect of the sphincter wall ran aborally, forming the newly found inner longitudinal muscle bundles, entered the duodenum, and connected with the nearby circular bundles. In all specimens, approximately one-third of the outer longitudinal layer of the sphincter entered

Suprazygomatic maxillary nerve block: an ultrasound and cadaveric study to identify correct sonoanatomical landmarks.

UNLABELLED: PURPOSE: Suprazygomatic maxillary nerve blocks (SMB) are used in adult and pediatric patients to provide analgesia for midface surgery and chronic maxillofacial pain syndromes. The ultrasound-guided SMB technique ensures visualisation of the needle tip, avoidance of the maxillary artery and confirmation of local anesthetic spread. The goal of this study was to correctly identify SMB sonoanatomical landmarks to ensure the nerve block is performed safely and effectively. METHODS: Following an ultrasound-guided SMB with dye injection on 2 embalmed cadavers, pre-tragal face-lift styl

The Important Relationship Between the Radial Artery's Course and the Radius during Henry's Anterior Forearm Surgical Exposure.

The aim of this study was to investigate the intersection point of the radial artery (RA) with Henry's approach. Ninety forearms from adult human cadavers which had been embalmed using Thiel's method underwent dissection. Henry's approach was performed alongside the whole length of the forearm, and the RAs course was investigated. Its crossing point with the approach was identified, and the distance from this point to the radial styloid process was determined. In addition, the total radial length (RL) was measured from the radial styloid process to the proximal margin of the radial head. The f
