Policy statement--The future of pediatrics: mental health competencies for pediatric primary care.

Pediatric primary care clinicians have unique opportunities and a growing sense of responsibility to prevent and address mental health and substance abuse problems in the medical home. In this report, the American Academy of Pediatrics proposes competencies requisite for providing mental health and substance abuse services in pediatric primary care settings and recommends steps toward achieving them. Achievement of the competencies proposed in this statement is a goal, not a current expectation. It will require innovations in residency training and continuing medical education, as well as a co

Transport and Management of Patients With Confirmed or Suspected Ebola Virus Disease.

The foundation of safe care for patients with confirmed or suspected Ebola virus disease is effective infection control practice, which requires implementation of appropriate administrative policies, work practices, and environmental controls, accompanied by focused education, training, and supervision. In 2002, Emory University partnered with the Centers for Disease Control and Prevention to develop a capability for the evaluation and management of individuals with serious communicable disease. In 2005, the University of Nebraska developed a similar isolation capability. In each case, the hos

Cardiac arrest witnessed by emergency medical services personnel: descriptive epidemiology, prodromal symptoms, and predictors of survival.

STUDY OBJECTIVE: The Utstein guidelines recommend that emergency medical services (EMS)-witnessed cardiac arrests be considered separately from other out-of-hospital cardiac arrest cases. The objective of this study was to assess EMS-witnessed cardiac arrest and to determine predictors of survival in this group. METHODS: This prospective cohort included all adults with an EMS-witnessed cardiac arrest in the 21 communities of the Ontario Prehospital Advanced Life Support (OPALS) study. Systems provided a basic life support with defibrillation (BLS-D) level of care. Case and survival definition

Burial workers' perceptions of community resistance and support systems during an Ebola outbreak in the Eastern Democratic Republic of the Congo: a qualitative study.

BACKGROUND: Community Health Workers (CHWs) provide vital services during disease outbreaks. Appropriate burials of those who died from an infectious disease outbreak is a critical CHW function to prevent infection and disease spread. During the 2018 Ebola Virus Disease (EVD) outbreak in Beni Town, North Kivu, Democratic Republic of the Congo, we sought to understand the levels of understanding, trust, and cooperation of the community in response to the outbreak, the barriers burial workers faced in their health work and its impact on local burial workers and other CHWs. METHODS: 12 EVD buria

Dilemmas of early dementia screening among community-dwelling older adults: a qualitative study from the perspective of social determinants of health.

BACKGROUND: Dementia is one of the most urgent public health challenges globally. Early screening is indispensable for timely identification and intervention. However, early dementia screening among community-dwelling older adults remains low, and the mechanisms underlying screening barriers remain insufficiently understood. METHODS: A qualitative study involving semi-structured interviews was conducted with 23 older adults who were purposively recruited from three communities in Wuhan, China. Interviews were transcribed verbatim and analyzed using thematic analysis that combined deductive co

Microbial biofilms: impact on the pathogenesis of periodontitis, cystic fibrosis, chronic wounds and medical device-related infections.

The majority of chronic infections are associated with mono- or polymicrobial biofilms, having a significant impact on the patients' quality of life and survival rates. Although the use of medical devices revolutionized health care services and significantly improved patient outcomes, it also led to complications associated with biofilms and to the emergence of multidrug resistant bacteria. Immunocompromised patients, institutionalized or hospitalized individuals, elderly people are at greater risk due to life-threatening septic complications, but immunocompetent individuals with predisposing

CNN-BiLSTM-Based Hybrid Deep Learning for Multi-Metric Anomaly Detection and Mitigation in Secure IoMT Healthcare WBANs.

Wireless Body Area Networks (WBANs) have become an essential component of modern Internet of Medical Things (IoMT) healthcare systems, enabling continuous monitoring of patient physiological signals through wearable sensors. Despite their advantages, WBAN environments remain highly prone to cyber threats, privacy breaches, and single points of failure. To address these risks, this work proposes a Hybrid Multi-Metric Anomaly Detection (HM-MAD) framework deployed on the NodeMCU-32S platform with BLE 5.0 connectivity for secure continuous glucose monitoring (CGM) data transmission. The detection

Spain: Health System Review.

This review of the Spanish health system analyses recent developments in health organization and governance, financing, health care provision, recent reforms and health system performance. Overall health status continues to improve in Spain, which presents the highest life expectancy in the European Union - although some socioeconomic inequalities in health persist and risk factors such as overweight, tobacco and alcohol consumption and illegal drug use remain a concern. The Spanish national health system (SNS) provides universal coverage, and it is mainly funded by taxes. Health competences a

A Hospital Avoidance Pathway With Digitally Supported Follow-Up for Low Back Pain: An Alternative to Emergency Department Care.

BACKGROUND: Low back pain (LBP) is a common reason for presentation to an emergency department (ED) but LBP-care in this setting is frequently low-value, costly and often fails to meet system and patient needs. Other service options are urgently required. This study evaluated a novel care pathway that diverted people with suspected non-serious LBP seeking ED-care to a community-based, hospital avoidance service in a large South Australian health network. METHODS: A retrospective quantitative analysis was conducted on data extracted from electronic medical records (demographics, service activi

Epinephrine Underuse for Anaphylaxis in Infants and Toddlers: A Practical Review for Pediatricians.

A recent consensus report defined anaphylaxis as a serious allergic reaction that may involve the skin/mucosa, respiratory, cardiovascular, and/or gastrointestinal systems that can progress rapidly and may cause death. Epinephrine treatment is recommended in suspected anaphylaxis cases; however, numerous barriers exist to consistent, widespread epinephrine use, including difficulty identifying anaphylaxis signs and symptoms; challenges recognizing when to use epinephrine; low propensity of prescribing and filling prescriptions; low epinephrine carrying rates; knowledge gaps in epinephrine admi

Institutional barriers and professional integration of physicians with an Arab migration background in Germany: a qualitative study.

BACKGROUND: Germany faces a persistent physicians shortage f, particularly in hospitals and rural regions. Migrant physicians, especially from Arab countries, represent a crucial component. However, their professional integration is shaped by complex licensure procedures, demanding working conditions, administrative burdens and sociocultural dynamics. While previous research has examined structural and regulatory barriers, limited empirical evidence exists on integration processes from the perspective of Arab physicians themselves. PURPOSE: This study aimed to explore the challenges and exper
