CONCLUSION: Our study showed that patients with atypical Meniere's disease had endolymphatic hydrops (EH) in both the cochlea and the vestibule. Using an imaging method, we provide evidence that atypical Meniere's disease is a true variant of classic Meniere's disease.
OBJECTIVES: The aim of present study was to investigate images of the endolymphatic space in patients with atypical Meniere's disease (cochlear and vestibular Meniere's disease).
METHODS: Sixty-four patients divided into two groups were enrolled in this study. The first group included 36 patients who had fluctuating hearing lo…

Secondary endolymphatic hydrops (SEH) has clinically been found to have a significant incidence of occurrence in patients with subjective idiopathic tinnitus (SIT) of a severe disabling type. The diagnosis is made clinically and has been established by integration in a medical audiological tinnitus patient protocol of the clinical history with results of electrodiagnostic cochleovestibular testing that fulfill the diagnostic criteria of inner-ear disease consistent with Ménière's disease. SEH is hypothesized to be a factor, not an etiology, influencing the clinical course of SIT. Alterations o…

Experimental endolymphatic hydrops was induced in guinea pigs by obliteration of the endolymphatic sac and duct. From 3 to 24 months after this operation, cochlear action potential (AP) audiograms and AP tuning curves were measured. The purpose of this study was to establish parallels, if any, between this supposed animal model of Menière's disease and the auditory symptoms of the disease in man. In some animals, low and middle frequency AP threshold elevations were observed whilst higher frequency regions maintained normal sensitivity. Other animals developed flat or very gradually sloping AP…

OBJECTIVES: The advancement of high-resolution magnetic resonance imaging (MRI) has enabled clinicians to detect endolymphatic hydrops, thereby prompting interest in distinguishing between Ménière disease (MD) and vestibular migraine (VM) in patients presenting with vertigo, headache, hearing loss, ear fullness, and tinnitus. The aim of this study is to compare cochlear and vestibular endolymphatic hydrops in diagnosing VM, MD, and vestibular migraine with Ménière disease (VMMD).
METHOD: A prospective clinical study evaluating patients with episodic vertigo meeting criteria for either MD, VM,…

Meniere's disease (MD) is an inner ear disease characterized by endolymphatic hydrops (EH). Maintaining a regular daily routine is crucial for MD patients. However, the relationship between circadian rhythms and MD remains unclear. Therefore, we investigated the effect of circadian rhythm on endolymphatic hydrops and its underlying mechanisms. Mice with endolymphatic hydrops were subjected to chronic jet lag (CJL) conditions to simulate the MD patients under circadian rhythm disruptions. We assessed whether this disruption would exacerbate inner ear damage with endolymphatic hydrops. RNA-seq o…

OBJECTIVE: To investigate whether the severity of endolymphatic hydrops (EH) influences the audiovestibular effects of intratympanic gentamicin (ITG) in patients with unilateral Ménière disease (uMD).
MATERIAL AND METHODS: Forty patients in 2 different tertiary referral centers with uMD refractory to medical treatment underwent 3T MRI 4 hours after gadolinium administration. EH severity was evaluated qualitatively and quantitatively (volumetry). Audiovestibular function was assessed with the PTA, vHIT, cVEMP, and oVEMP before and 1 month after ITG. Associations were analyzed with Kruskal-Wall…

BACKGROUND: This study aimed to clarify the prognosis of asymptomatic endolymphatic hydrops (EH) in healthy volunteers via five-year follow-ups with inner ear magnetic resonance imaging (MRI).
METHODS: Inner ear MRI was performed on 115 participants recruited as controls in a previous study on Meniere's disease. The endolymphatic space was visualized using Naganawa's method of contrast-enhanced MRI with intravenous gadolinium injection and evaluated using Nakashima's method of 2D imaging analysis.
RESULTS: Cochlear or vestibular EH was present in 7.0% of participants (n = 8), with all cases …

BACKGROUND: This study aimed to investigate the relationship between the features of endolymphatic hydrops and hearing loss in patients with Bilateral Meniere's Disease.
METHODS: A retrospective analysis was conducted on 77 patients diagnosed with Bilateral Meniere's Disease. The features of endolymphatic hydrops in the affected ear were evaluated through gadolinium-enhanced inner ear Magnetic resonance imaging. The Spearman correlation coefficient, paired t-tests, and Wilcoxon signed-rank tests were employed for data analysis.
RESULTS: The analysis revealed a significant correlation between…

In this study, we evaluated the clinical diagnostic value of vestibular evoked myogenic potentials (VEMPs) for endolymphatic hydrops (EH) by systematic review and Meta-analysis. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, diagnostic odds ratio and area under summary receiver operating characteristic curves (AUC) were calculated. Subgroup analysis and publication bias assessment were also conducted. The pooled sensitivity and the specificity were 49% (95% CI: 46% to 51%) and 95% (95% CI: 94% to 96%), respectively. The pooled positive likelihood rat…

CONCLUSION: We reported previously that hyperactivation of vasopressin type-2 receptor (V2R)-mediated signaling in the endolymphatic sac could affect endolymphatic fluid metabolism, resulting in the pathogenesis of endolymphatic hydrops. Taken together with the present endolymphatic sac tumor (ELST) study, it is suggested that disorder of V2R signaling in the endolymphatic sac for any reason could be involved in the pathogenesis of endolymphatic hydrops. Although it is due to tumor genesis in ELST, it is idiopathic in nature in Meniere's disease.
OBJECTIVE: We encountered two cases of ELST sh…
