Right-sided descending colon with necrotizing enterocolitis: a rare case report.

BACKGROUND: Anatomical variations of the colon are commonly reported, with the majority involving the right colon. However, anomalies affecting the left colon, particularly the descending colon, are rarely described in the literature. METHODS: We reviewed the clinical records of right-sided descending colon with necrotizing enterocolitis involvement at our hospital, detailing the patients' onset, imaging studies, complications, and treatment. RESULTS: A newborn experienced repeated vomiting. Upper and lower gastrointestinal imaging revealed gastroesophageal reflux, the right-sided descending

Necrotizing enterocolitis in monochorionic twins: Insights from an identical twin model.

Necrotizing enterocolitis (NEC) is a major cause of neonatal morbidity and mortality in preterm neonates, yet its pathophysiology remains unclear. The aim of this study is to evaluate risk factors for NEC using an identical twin model. In this case-control study, all monochorionic twin pairs born in our center in 2002-2020 were retrospectively reviewed for NEC. Potential risk factors for NEC were studied. For within-pair comparison, outcomes were compared between affected and unaffected twins. Within-pair analyses showed that the twin with NEC had a lower birth weight compared to its unaffecte

Maternal body mass index and necrotizing enterocolitis: A case-control study.

INTRODUCTION: Our aim was to determine if maternal body mass index (BMI) is associated with necrotizing enterocolitis (NEC) in a large urban delivery center. METHODS: This single center retrospective case-control study included 291 infants under gestational age of 33 weeks admitted to the neonatal intensive care unit (NICU) during a 10-year period. Cases of stage 2 and 3 NEC were matched at a ratio of 2 controls (n = 194) to 1 case (n = 97). Maternal BMI was categorized as normal (≤24.9), overweight (25-29.9) and obese (≥30). Chi-square and stepwise logistic regression were used for analysis.

Development of necrotizing enterocolitis after blood transfusion in very premature neonates.

BACKGROUND: Prior studies report conflicting evidence on the association between packed red blood cell (PRBC) transfusions and necrotizing enterocolitis (NEC), especially in early weeks of life where transfusions are frequent and spontaneous intestinal perforation can mimic NEC. The primary objective of this study was to evaluate the association between PRBC transfusions and NEC after day of life (DOL) 14 in very premature neonates. METHODS: A retrospective cohort analysis of very premature neonates was conducted to investigate association between PRBC transfusions and NEC after DOL 14. Prima

Intestinal perforation in necrotizing enterocolitis: Does cardiac surgery make a difference?

BACKGROUND: The aim of this study is to evaluate the patients with intestinal perforation secondary to necrotizing enterocolitis (NEC) following cardiac surgery in the terms of risk factors and diagnosis/treatment process. METHODS: A series of cases operated for intestinal perforation secondary to NEC were retrospectively reviewed in two groups. Group I involved patients who had cardiac surgery for congenital heart disease previous to intestinal perforation secondary to NEC. Group II consisted patients who had intestinal perforation secondary to NEC without any previous cardiac surgery. Demog

Necrotizing Enterocolitis and Spontaneous Intestinal Perforation: A Spatiotemporal Case Cluster Analysis.

OBJECTIVE: To expand existing statistical methods to identify clusters of necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) cases in the neonatal intensive care unit. METHODS: In an academic, tertiary referral center, possible NEC or SIP clusters were identified using a binomial distribution scan test. The incidence-density rate (IDR) was calculated as the number of cases per 1,000 patient-days during each possible cluster and compared with the baseline IDR. A structured chart review compared cluster and noncluster cases. Spatial clustering analyzed the physical dis

Intestinal microcirculation and necrotizing enterocolitis: The vascular endothelial growth factor system.

Necrotizing enterocolitis (NEC), a leading cause of morbidity and mortality in preterm neonates, is a devastating disease characterized by intestinal tissue inflammation and necrosis. NEC pathogenesis is multifactorial but remains unclear. Translocation of bacteria and/or bacterial products across a weak intestinal barrier in the setting of impaired mucosal immunity leads to an exaggerated inflammatory response and secondary mucosal epithelial injury. In addition to prematurity, other risk factors for NEC include congenital heart disease, maternal pre-eclampsia with placental vascular insuffic

Necrotizing enterocolitis and high intestinal iron uptake due to genetic variants.

BackgroundIntestinal iron is a nutritional compound, which is essential for enteric microbiota. We evaluated the hypothesis that polymorphisms, which are known modifiers of intestinal iron uptake in adults, are associated with necrotizing enterocolitis (NEC) in preterm infants.MethodsPreterm infants (birth weight below 1,500 g) were studied. Single-nucleotide polymorphisms with known effects on serum iron levels (rs1800562, rs1799945, and rs855791) were determined using PCR. The effects of polymorphisms on NEC surgery were tested by Mendelian randomization. Outcome data were compared with χ2-t

Cytokines associated with necrotizing enterocolitis in extremely-low-birth-weight infants.

BACKGROUND: The goal was to identify cytokines associated with necrotizing enterocolitis (NEC). Based on our earlier reports of decreased tissue expression of transforming growth factor (TGF)-β, we hypothesized that infants with NEC also have low blood TGF-β levels. We further hypothesized that because fetal inflammation increases the risk of NEC, infants who develop NEC have elevated blood cytokine levels in early neonatal period. METHODS: Data on 104 extremely-low-birth-weight infants with NEC and 893 without NEC from 17 centers were analyzed. Clinical information was correlated with blood

Abdominal Radiograph and Abdominal Ultrasound for Necrotizing Enterocolitis Diagnosis and Management.

Necrotizing enterocolitis (NEC) is one of the most common gastrointestinal emergencies in preterm infants and is a leading cause of morbidity and mortality in this population. Although NEC is localized to the bowel, there are serious associated systemic complications with significant morbidity and mortality. Historically, the diagnostic standard has been plain abdominal radiography. However, the availability of abdominal ultrasound has been emerging as a complementary diagnostic tool allowing for direct visualization of the bowel and abdominal cavity. This article reviews the roles of both mod

Gut Microbiome in Neonatal Necrotizing Enterocolitis - A Comprehensive Review of Evidence.

Necrotizing enterocolitis (NEC) is one of the most catastrophic gastrointestinal emergency occurring predominantly in preterm neonates. It contributes to substantial neonatal morbidity and mortality. Disturbances in the intestinal microbiome are crucial to disease pathogenesis. In preterm infants, an immature intestinal barrier, dysregulated immune responses, and environmental exposures altogether predispose to alteration in microbial colonization and intestinal inflammation. This review was done to present the current evidence on gut microbiome alterations associated with NEC in preterm infan

The role of abdominal ultrasound in surgical decision making for necrotizing enterocolitis.

Despite advances in neonatal care, Necrotizing enterocolitis (NEC) remains one of the most devastating gastrointestinal complications affecting most often preterm infants and full -term infants with congenital heart disease. Early and accurate diagnosis is vital to reduce morbidity and mortality. While abdominal radiography has traditionally served as the cornerstone of diagnosis, abdominal ultrasound (AUS) emerges as an additional tool offering the benefits of a real-time, radiation-free and functional assessment of the bowel. However, the role of AUS in the assessment of need for surgery rem
