[Extracorporeal shock-wave lithotripsy for the treatment of calcific tendonitis of the glutaeus maximus tendon].

Calcific tendinitis at the femoral insertion of the glutaeus maximus tendon is an unusual but well-described location for an ossifying enthesopathy. Extracorporeal shock-wave lithotripsy has become a standard therapy for various pathologies in orthopaedics. We report a case of a 50-year-old man with painful calcific tendinitis at the femoral insertion of the glutaeus maximus tendon who successfully underwent extracorporeal shock-wave treatment.

Diffuse idiopathic skeletal hyperostosis in ancient clergymen.

Diffuse idiopathic skeletal hyperostosis (DISH) is a common but often unrecognized systemic disorder observed mainly in the elderly. DISH is diagnosed when the anterior longitudinal ligament of the spine is ossified on at least four contiguous spinal levels or when multiple peripheral enthesopathies are present. The etiology of DISH is unknown but previous studies have shown a strong association with obesity and insulin-independent diabetes mellitus. DISH can lead to back pain, dysphagia, myelopathy, musculoskeletal impairment and grossly unstable spine fractures after minor trauma. In archeol

Ankylosing Spondylitis With Bilateral Symmetrical Uptake in the Greater Trochanters on Bone Scan.

Ankylosing spondylitis is a chronic inflammatory bone disease that commonly affects the spine and the sacroiliac joints. Ankylosing spondylitis can also cause enthesitis, or enthesopathy, which is an inflammation at the tendinous or ligamentous insertion sites on the bones. In this study, we present the case of a 37-year-old man with ankylosing spondylitis with a unique Tc-HDP bone scan finding of symmetrical increased uptake bilaterally in the greater trochanters.

Calcific tendonitis of the quadriceps.

The case reported is of a 46 year old male who presented with a history of acute on chronic knee pain. The clinical features and investigations suggested a tear of the quadriceps tendon, with pre-existing chronic calcific enthesopathy. The operative findings were of an acute collection of calcific material within the quadriceps tendon. This acute presentation and calcific collection have not previously been reported in the quadriceps tendon.

Iliotibial band friction syndrome after knee cementoplasty: a case report.

Iliotibial band pathologies can result in lateral knee pain. These are commonly seen in runners and cyclists. Lateral knee pain following knee arthroplasty can be due to distal iliotibial band enthesopathy or impingement by the femoral component. Cementoplasty is a common procedure performed during treatment of osseous lesions. We describe a case of ITB friction syndrome due to small focus of cement following cementoplasty for GCT (giant cell tumour).

Inflammatory arthritis and the elbow surgeon.

The treatment of inflammatory arthritis with disease modifying drugs and biological agents had reduced the number of patients needing surgical treatment. Surgical treatment of patients with inflammatory arthritis is challenging not only due to the factors such as bone stock and status of soft tissue but also due to the comorbidities associated with inflammatory arthritis. Multidisciplinary approach to these patients is recommended to deal with the complex poly-articular involvement and systemic physiological impairment especially when planning surgery. This review will cover the key articular

Entheseal surface (Sharpey's fiber insertion) alterations identify past trauma; bone base robusticity, level of routine activity.

Sharpey's fiber alterations, referred to as entheseal reaction or enthesopathy, have long been considered an indicator of daily activities. Such semantic transformation seems to conflate processes which alter the characteristics of tendonous and ligamentous attachments to bone with the rugosity and extent of their base/footprint. Rather than reflecting normal activities, it is suggested that surface reactions are actually the response to the application of sudden or unconditioned repetitive stresses-analogous to stress fractures. Thus, they are distinct from enlargement of the base/footprint, …

A role for neutrophils in early enthesitis in spondyloarthritis.

BACKGROUND: Neutrophils are present in the early phases of spondyloarthritis-related uveitis, skin and intestinal disease, but their role in enthesitis, a cardinal musculoskeletal lesion in spondyloarthritis, remains unknown. We considered the role of neutrophils in the experimental SKG mouse model of SpA and in human axial entheses. METHODS: Early inflammatory infiltrates in the axial and peripheral entheseal sites in SKG mice were evaluated using immunohistochemistry and laser capture microdissection of entheseal tissue. Whole transcriptome analysis was carried out using Affymetrix gene arr

Suspecting and Diagnosing the Patient with Spondyloarthritis and What to Expect from Therapy.

Spondyloarthritis is a common rheumatologic disease, present in up to 2% of the population, characterized by inflammatory arthritis, often with enthesitis, dactylitis, spondylitis, and skin disease. It has historically been characterized as ankylosing spondylitis, psoriatic arthritis, arthritis associated with inflammatory bowel disease, reactive arthritis, and undifferentiated spondyloarthritis. These subsets are now classified as axial-predominant and peripheral-predominant spondyloarthritis. This article provides an updated understanding of disease classification and practical advice about

Intestinal and enthesis innate immunity in early axial spondyloarthropathy.

Axial SpA (axSpA), encompassing AS, is a multifactorial disease that localizes to sites of high spinal biomechanical stress. Much has been written on T cells and adaptive immunity in axSpA, which is understandable given the very strong HLA-B27 disease association. Extra-axial disease characteristically involves the anterior uveal tract, aortic root, lung apex and terminal ileum. Under recent classification, axSpA is classified as an intermediate between autoimmunity and autoinflammatory disease, with the latter term being synonymous with innate immune dysregulation. The purpose of this review

Getting to the heel of the problem: plantar fascia lesions.

Heel pain is a frequent disabling symptom. Clinical diagnosis is often difficult with a large range of possible diagnoses. Lesions of the plantar fascia form an important group. We present a review describing the common lesions of the plantar fascia, including plantar fasciitis, plantar fascia rupture, plantar fibromatosis, and plantar xanthoma, and illustrate them with appropriate magnetic resonance imaging (MRI) and ultrasound imaging. We also address foreign-body reactions, enthesopathy, and diabetic fascial disease.

[The local therapy of enthesitis and bursitis of the calcaneal area in seronegative spondylarthritis].

Four methods of local treatment of calcaneal enthesopathy and bursitis (hydrocortisone phonophoresis, laser radiation, glucocorticosteroids injections into the calcaneal region, physiotherapy with sinusoidal modulated currents) were tried in 69 patients with seronegative spondylarthritis. Local injections of glucocorticosteroids produced the best effect. Laser therapy or hydrocortisone phonophoresis is recommended in dominating pain syndrome in the calcaneal region. SMC therapy was not good in inflammation of tendinous-ligamentous system and calcaneal bursas in patients with spondylarthritis.

Magnetic resonance imaging in pachydermoperiostosis.

Pachydermoperiostosis is a rare inherited disorder that is manifest clinically by digital clubbing, extremity enlargement, painful and swollen joints, hypertrophic skin changes, and periosteal bone formation. This report illustrates the skeletal findings that may be seen with long-standing disease as evaluated with conventional radiography and with magnetic resonance (MR) imaging, with emphasis on the MR appearance of periosteal reaction in this disorder. There are features that suggest pachydermoperiostosis may represent a generalized enthesopathy.

Radiologic features of DISH may mimic ankylosing spondylitis.

Intervertebral bony bridging and enthesopathy can occur in both diffuse idiopathic skeletal hyperostosis (DISH) and ankylosing spondylitis (AS) and we have seen difficulties in the radiological differentiation between them. We describe 6 patients selected because the bridges resembled syndesmophytes and this initially suggested a diagnosis of AS, although other considerations showed them to be part of the radiologic picture of DISH. The previously reported suggestions of the coexistence of DISH and AS are reviewed.

Low-dose Radiation Therapy for Benign Orthopaedic Conditions.

Low-dose radiation therapy is increasingly used for benign orthopaedic conditions, including osteoarthritis and enthesopathies, with typical regimens of 0.5 to 1 Gy per fraction and total doses of 3 to 6 Gy. Clinical studies demonstrate notable pain relief and functional improvement, particularly in patients refractory to traditional therapies, with minimal acute and long-term toxicity. Mechanistic insights reveal anti-inflammatory and immunomodulatory effects, supporting its use in degenerative and hyperproliferative musculoskeletal disorders. However, randomized trials and systematic reviews
