Is the Sphere Value Measured by an Autorefractor Reliable in Children under Cycloplegia? Comparison with Streak Retinoscopy.

PURPOSE: To compare and repeatedly measure cycloplegic refraction in a cross-section of children using retinoscopy and automated refraction. METHODS: A total of 560 children (corresponding to 1120 eyes) ranging in age from 3 to 10 years participated in the study. Each child underwent a comprehensive eye examination, which included table mounted autorefractor evaluation (Kowa KW-2000) and streak retinoscopy, both after cycloplegia with 1% cyclopentolate hydrochloride. Data were analyzed using Fourier decomposition of the power profile. RESULTS: More positive values of M component and sphere v

[Benign osteoblastoma of the parietal bones].

Benign osteoblastoma accounts for less than 1% of the primary bone tumors and the calvarial lesion is extremely rare. There are only 12 reported cases in the literature as far as we could collect. We have presented a case of benign osteoblastoma originated from the parietal bone. This 9-year-old boy struck his head on the parietal region and noticed the bulging at the same site. A month later he visited to Fukuoka University Hospital because of persistent bulging of the same site. Neurological examinations were normal. Plain skull roentgenogram on Towne view showed radiolucent area in the midl

Agenesis of the corpus callosum: clinical, neuroradiological and cytogenetic studies.

This study examined 35 patients with developmental disabilities who were referred for diagnostic evaluation that later revealed agenesis of the corpus callosum (ACC) by computerized tomography (CT). Sixteen had partial ACC, six had complete ACC, and one had a hypoplastic corpus callosum. In the other twelve cases, ACC existed, but the degree of callosal defect was not specified. Other intracranial defects were frequently present. Clinically, 15 patients (43%) had a history of seizures, 28 (82%) were mentally retarded or developmentally delayed and an additional five patients (15%) possessed bo

Dynamic ultrasonography of the infant hip with suspected instability. A new technique.

PURPOSE: The combining of clinical examination of the infant hip with dynamic ultrasound (US) into one examination, performed by one examiner, should result in a sensitive test for hip instability. In this study a new method of dynamic US examination of the hip in newborn and very young children, that combined US with stress testing (BARLOW method) was tested. MATERIAL AND METHODS: A specially designed examination table, with a US probe holder, enabled the radiologist to perform a stress test of the hip according to BARLOW (using both hands), and simultaneously monitor it on US. Hydrocephalus in infancy and childhood: diagnosis and indication for operation.

Improvement of the prognosis for children suffering from hydrocephalus requires prompt diagnosis and reliable indication of surgical treatment. Today, intrauterine hydrocephalus is detectable within the first three months of pregnancy; in infancy, before the cranial sutures have fused, pathological growth of the head is the principal sign confirming together with anatomical examinations (ultrasound, CT scan) the indication of operative treatment. In later childhood, surgical treatment is only definitely indicated by symptoms and morphological examination of clearly active hypertensive hydrocep

Uncommon causes of knee pain in the athlete.

There are many causes of knee pain in the athlete. Clearly, common things, such as meniscal tears, ligament sprains, contusions, and patellofemoral dysfunction, are common. The infrequency of other entities, however, should not make the physician complacent. A thorough history and physical examination will, in most cases, elicit inconsistencies in the occasional athlete presenting with one of these uncommon causes of knee pain (Table 1). Diagnosing these uncommon causes, when they do present to the health care provider, allows for early and correct treatment with a more rapid return to sports.

The detection of lice (Bovicola ovis) in mobs of sheep: a comparison of fleece parting, the lamp test and the table locks test.

Knowledge of the presence or absence of lice in a flock of sheep enables wool growers to make informed decisions as to the need for insecticidal treatments. However, with inapparent infestations, traditional methods of detection are not sufficiently sensitive and, as a consequence, flocks may be left untreated. Conversely, the routine application of insecticide to sheep with no sign of infestation is an unnecessary cost. The sensitivity of 3 procedures for detecting lice was evaluated in 68 mobs of sheep from 50 farms. In 24 mobs of sheep known to be lightly infested, lice were detected in 17%

[Autonomic failure as the cause of vertigo and syncope].

BACKGROUND: Vertigo and syncope often occur during orthostatic conditions. A possible cause is asympathicotonic dysregulation with a fall of blood pressure and no increase in heart rate. After the exclusion of secondary disturbances of autonomic innervation. g. in diabetes mellitus, and pharmacological side effects, e.g. tricyclic antidepressants, primary reasons must be considered. We present 2 cases of primary autonomic failure. PATIENTS: Patient 1 had Parkinson-syndrome, impaired bladder function and orthostatic dysregulation with a maximal drop of blood pressure to 60/40 mm Hg. Patient 2

[Descriptive and epidemiologic characteristics of patients with malignant upper urothelial tumors in the endemic area of Lazarevac].

Although there is permanent increase in incidence of malignant upper urothelial tumours [1, 2], these malignancies are rare neoplasms in relation to both all malignant tumours and urotract tumours. Upper urothelial tumours, i.e. tumours of the renal pelvis and ureter are more frequent in the regions affected by endemic nephropathy [3-5]. The aim of this paper was to describe the main epidemiological characteristics in patients with upper urothelial tumours (UUT) in endemic nephropathic (EN) foci in Lazarevac. We analyzed 73 patients treated at the Institute of Endemic Nephropathy, Lazarevac an

An examination procedure for cavo-adducto-varus.

This examination is used in cava-adducto-varus feet to determine the primary areas producing the secondary forefoot pathology. Table I is a compilation of the data and shows by means of an asterisk those pathologies possible with an additional primary deformity. The fold face test parts, when indicated in the exact order, are diagnostic. Once the primary areas have been elucidated, the remaining components are secondary, and the prognosis of biochemical and specific surgical treatment can be made. The specific treatment procedures and their rational use will be discussed in a future presentati

Ecstasy: an important cause of acute liver failure.

A twenty year old female was referred to hospital by her GP, after he received the results of blood tests taken earlier in the day. She had presented to him complaining of malaise, nausea and anorexia over a 3 day period. On the day of referral she had also become jaundiced with dark urine, but normal stool colour. There was no abdominal pain. She had no significant past medical history with no history of jaundice, liver disease or autoimmune conditions, and no apparent risk factors for blood-borne hepatitis infection. There was no relevant family history. She was taking no prescribed medicati

[Heme synthesis and iron turnover in rabbits with experimental lead poisoning].

The nature of anemia formation in lead poisoning is still unknown, especially of its mechanism. Rimington proposed a hypothesis that lead interferes with a specific enzyme (ferrochelatase) system, which is responsible for incorporating iron into the protoporphyrin IX molecules to form heme. In the present study, heme synthesis in mitochondria (Mt) of liver and bone marrow, and 59Fe behavior in vivo were investigated in rabbits to verify his hypothesis. The experimental five groups were as follows: 1) lead (Pb) poisoning by administration of 10-20 mg Pb/kg at a time, subcutaneously injected 10

Vasovagal syncope in patients with reduced left ventricular function.

Vasovagal syncope (VVS) is mediated by arterial mechanoreceptors, resulting in reflexive changes in heart rate and vascular tone. The Bezold-Jarisch reflex was originally described as enhanced contraction and activation of left ventricular mechanoreceptors, but later studies implicated other triggers, including coronary, carotid, and cerebral arterial mechanoreceptors. VVS is uncommon in patients with left ventricular dysfunction. We hypothesized that VVS could occur in this subset and examined patient characteristics and hemodynamic responses during tilt table testing. From 1996 through 1998,

Optic neuritis in relation to multiple sclerosis.

Available estimates of the frequency with which a patient with optic neuritis develops multiple sclerosis range from as low as 13% to as high as 87%. In an effort to obtain a better estimate, a nation-wide study of optic neuritis was carried out in Israel. Patients who fulfilled strict diagnostic criteria of optic neuritis were identified and examined periodically. Between 1955 and 1964, 105 patients were found and on the basis of these, the average annual age-adjusted incidence of optic neuritis in Israel was 0.56 per 10(5) population compared to 1.2 per 10(5) cases of multiple sclerosis per
