A 24-year-old man with no significant medical history was admitted to this hospital because of an acute exanthema, myalgia and malaise. A fourth-generation screening test for HIV was reactive but the following confirmation test was not. Although the patient did not belong to a high risk group, we decided to perform a HIV-RNA-PCR. The result showed a high virus load so that we finally diagnosed a primary HIV infection.

Allergic reaction to nitinol is rarely reported, and its incidence, symptoms, and course have not been clearly defined. We report an occurrence of severe progressive generalized exanthema 3 days after the implantation of an Amplatzer occluder for a patent foramen ovale, with symptoms disappearing immediately after surgical removal of the device 3 months later. The risks and possible prevention of allergic reaction to nickel and especially to titanium are discussed.

The mucocutaneous manifestations of Mycoplasma pneumoniae infection appear in approximately 20% of all the infections produced by this microorganism. Maculopapular rash, vesicular or urticarial exanthemas, are frequent manifestations that can constitute Erythema multiforme or more rarely, Stevens-Johnson syndrome or epidermal toxic necrolisis. We describe the clinical evolution, diagnosis and treatment of four children with mucous and cutaneous manifestations associated to infection by Mycoplasma pneumoniae and a review of the medical literature.

Toxic Shock Syndrome (TSS) is a very rare and severe complication of Staphylococcus aureus infections. However, bacteremia is very uncommon in this disease. We present here the case of a healthy 15-year old boy who presented septic shock and diffuse exanthema four hours after eating in a fast food restaurant. Blood cultures were positive for a TSST-1 producing Staphylococcus aureus. The patient was treated with antibiotics and fully recovered.

Autoinflammatory diseases represent a group of disorders that are distinct from autoimmune diseases, infections and malignancies. They are characterised by attacks of unprovoked noninfectious inflammation. A key feature of autoinflammatory diseases is dysregulation of innate immune system and overproduction of pro-inflammatory cytokines. The best-characterized group of autoinflammatory diseases arises from mutations in single genes. They are typically manifested by recurrent attacks of fever and serositis as well as arthralgia, myalgia and skin exanthema.

Diffuse exanthema with fever followed by superficial nonfollicular aseptic pustules was observed in an 11-year old boy after conducting a thiomersal epicutaneous test and manipulation of a broken medical thermometer. The patient healed within two days without any treatment. The initial responsibility of mercury and its derivatives, and the primary sensitization and relevance of the epicutaneous tests are discussed. Mercury and its derivatives should be limited as much as possible in medicine and cosmetology.

We report a neonate with a rash that appeared on the second day of life in association with elevated body temperatures and increased C-reactive protein levels. The rash was evanescent and recurred on a daily basis during the first year of life. At 15 months, the infant developed swelling of the right knee joint. Early-onset systemic juvenile rheumatoid arthritis can mimic congenital infections and should be considered in the differential diagnosis of neonatal exanthemas.

The typical papulomacular picture of boutonneuse fever is described. A series of atypical exanthemas is presented: simply erythemato-macular and typhus-like, nodular, vesicular. Skin biopsies showing the endothelial, thrombotic and parietal aspects of periadventitial infiltration (this being sometimes moderately and extensively pericapillary) are also presented. The reason for the absence of extensive thrombisation in extensive, marked rickettsiotic endothelitis are discussed, and stress is laid to the hitherto little-discussed, but significant involvement of dermal collagen.

A case of mercury-induced baboon syndrome was reported. A 31-year-old woman presented with itching papules and vesicles in the right axilla, which extended to the left axilla, arms, fossa poplitea, buttocks, and groin. A mercury thermometer was broken 2 days before exanthema. Patch testing to ammoniated mercury, mercury, mercuric chloride, and mercurochrome were positive. Blood and urine mercury level was marginally normal, yet showed a descending trend over time.
