Exhaled nitric oxide in the diagnosis of asthma: comparison with bronchial provocation tests.

BACKGROUND: Bronchial provocation tests such as exercise, methacholine (MCH), and adenosine-5'-monophosphate (AMP) challenges are used extensively in the diagnosis of asthma. A study was undertaken to determine whether exhaled nitric oxide (eNO) can be used to diagnose asthma in patients with non-specific respiratory symptoms and to compare this test with conventional provocation tests. METHODS: Patients with non-specific respiratory symptoms and normal spirometric parameters were included in the study. eNO was measured and exercise, MCH and AMP challenges performed in all subjects. Patients

Exhaled nitric oxide levels correlate with measures of disease control in asthma.

BACKGROUND: Asthma guidelines emphasize maintaining disease control. However, objective measures of asthma disease control are lacking. OBJECTIVE: We sought to examine the relationship between exhaled nitric oxide (NO) levels and measures of asthma disease control versus asthma disease severity. METHODS: We performed a cross-sectional study of 100 patients (age range, 7-80 years) with asthma. We administered a questionnaire to identify characteristics of asthma, performed spirometric testing before and after administration of a bronchodilator, and measured exhaled NO levels in all participan

Influenza vaccination: changes in exhaled nitric oxide levels and sputum cytology.

OBJECTIVE: Influenza vaccination is routinely recommended for patients with chronic lung disease, but has been reported to cause a small increase in airway reactivity. The use of simple, non-invasive methods of assessing changes in airway inflammation could potentially allow improved understanding of the cellular mechanisms underlying such changes. METHODOLOGY: We studied a group of 44 hospital workers before and after routine influenza vaccination, using spirometry, and exhaled nitric oxide (eNO) as a marker of pulmonary inflammation (Group A). In addition, methacholine challenge and sputum

Modeling the concentration of ethanol in the exhaled breath following pretest breathing maneuvers.

A previously developed mathematical model that describes the relationship between blood alcohol (ethanol) concentration and the concentration of alcohol in the exhaled breath at end-exhalation (BrAC) has been used to quantitate the effect of pretest breathing conditions on BrAC. The model was first used to "condition" the airways with different breathing maneuvers prior to simulating a single exhalation maneuver, the maneuver used in standard breath alcohol testing. On inspiration, the alcohol in the air reaches local equilibrium with the alcohol in the bronchial capillary bed prior to enterin…

Challenge from methacholine, natural rubber latex, or 4,4-diphenylmethane diisocyanate in workers with suspected sensitization affects exhaled nitric oxide [change in exhaled NO levels after allergen challenges].

OBJECTIVE: Nitric oxide (NO) levels in the exhaled air of asthmatic patients have been shown to be increased. This observation has also been reported in workers who are allergic to laboratory animals. To determine if a challenge test with natural rubber latex (NRL) or 4,4'-diphenylmethane diisocyanate (MDI) would also produce an increase of NO exhalation in sensitized patients, we carried out this study. METHODS: Nine subjects with suspected occupational asthma were exposed to MDI, and 18 took part in a challenge test with gloves powdered with NRL. Nineteen subjects underwent a challenge test…

Efficacy of the exhalation delivery system with fluticasone in patients with chronic rhinosinusitis with nasal polyps whose symptoms recur after sinus surgery.

BACKGROUND: Sinus surgery removes inflamed tissue, restores airflow, and improves delivery of medication into surgically opened spaces. The exhalation delivery system with fluticasone (EDS-FLU; XHANCE® ) uses a novel delivery system to create closed-palate, positive-pressure, bidirectional mechanics that significantly alter the deposition of the topically acting anti-inflammatory medication. We ask whether EDS-FLU efficacy differs for patients with recurrent symptoms after sinus surgery versus patients without surgery. OBJECTIVE: We aimed to compare EDS-FLU treatment responses in patients wit…

Fluctuations of CO₂ and 222Rn concentration in the karst vadose zone: Comparing exhalation, indoor concentrations, and cave air dynamics (Nerja Cave, Southern Spain).

To investigate the influence of atmospheric and subsurface karst conditions on gas transport in the vadose zone and across the epikarst-atmosphere interface, we measured: a) radon (222Rn) and carbon dioxide (CO2) concentrations in the air of two boreholes drilled in the vadose zone, where high levels of these gases had been previously reported, and b) indoor radon concentrations in two dwellings built on potentially karstifiable lithologies. One dwelling (D1) is located near the boreholes and a major tourist cave (Nerja Cave), while the other (D2) is in the town of Nerja. Periodic fluctuations

Accessing the Eustachian tube: Conventional nasal spray vs. exhalation delivery system and the impact of targeted endoscopic sinus surgery on topical distribution patterns.

BACKGROUND: Eustachian tube dysfunction (ETD) may occur distinct from, or in conjunction with, chronic rhinosinusitis (CRS+ETD). Intranasal corticosteroid sprays are often prescribed for ETD, although ET distribution may be limited. To date, no anatomic studies compare nasopharynx (NP) distribution between conventional nasal sprays (NS) and exhalation delivery systems (EDS) after surgery. This study utilizes a cadaver model to examine topical NP delivery using EDS vs. NS before and after targeted endoscopic sinus surgery (ESS). METHODS: Sixteen sinonasal cavities were administered fluorescein…

Ultrasensitive Exhaled H2S Sensor Enabled by Pt@ZIF-8/Cu2O Heterostructures.

The rapid and accurate detection of H2S in exhaled breath at room temperature remains a critical technical challenge. Herein, we developed a Pt@ZIF-8/Cu2O heterojunction sensor via a two-dimensional electrodeposition in-situ assembly strategy for sensitive H2S detection. This sensor exhibits superior room-temperature sensing performance: a remarkably high response of 8931 toward 10 ppm of H2S, an ultralow detection limit of 10 ppb (response ∼0.61), and a 11.8-fold enhancement in response to 500 ppb H2S relative to pristine Cu2O. The exceptional sensing performance is attributed to the synergis

Microdroplet Phase Change Leads to Substantial Variation in Exhaled Breath Condensate Sampling.

Exhaled breath condensate (EBC) has been proposed as a promising non-invasive diagnostic biofluid for 50 years, yet no EBC-based test has reached clinical deployment. A major barrier to clinical translation is its measurement variability, which can span orders of magnitude, far exceeding the 10% clinical tolerance. Whether this variability originates from human biology or physical measurement error remains unknown. To separate these contributions, we examined how EBC biomarkers are transported during the condensation and evaporation of EBC droplets, a physical process common to all EBC sampler

A green and non-invasive approach for quantitative analysis of psychoactive substances in exhaled breath.

Exhaled breath has emerged as a promising alternative biological matrix in forensic toxicology due to its non-invasive collection and its potential to reflect recent substance use. In this study, a custom-made breath collection device was developed using 3D printing technology, and its analytical performance for detecting psychoactive substances was evaluated. A quantitative analytical method for target analytes in exhaled breath was validated in accordance with ANSI/ASB guidelines and subsequently applied to samples collected from individuals undergoing substance use treatment. The method dem

Fractional exhaled nitric oxide and nasal nitric oxide as biomarkers in chronic rhinosinusitis.

PURPOSE OF REVIEW: This article summarizes current evidence on the role of fractional exhaled nitric oxide (FeNO) and nasal nitric oxide (nNO) as noninvasive biomarkers in chronic rhinosinusitis with nasal polyps (CRSwNP), focusing on their potential to refine endotype-driven diagnosis and monitoring in the context of united airways disease. RECENT FINDINGS: Recent studies show that nNO levels are markedly reduced in CRSwNP, especially in eosinophilic phenotypes, where they correlate inversely with radiologic severity and may reflect both ostiomeatal obstruction and epithelial nitric oxide sy
