Non-operative management is still the treatment of choice for closed fractures of the femoral shaft in children. Indications for operative intervention would include: children with multiple injuries; severe soft tissue damage; cases where reduction is difficult to maintain; and children not suitable for management by traction. Since 1984, 16 children (mean age 10.3 years) have had stabilization of their femoral shaft fractures by external fixation (Monofixateur) in the Trauma Department of the Hannover Medical School. The external fixation remained in place for a mean of 63 days. Of the 16 chi

External fixation was used to treat complex femur fractures in 10 children. These injuries were associated with head trauma, cerebral palsy, epidermolysis bullosa, open wounds, and failed casting. The duration of external fixation averaged 8 weeks, and the mean follow-up was 5 years 8 months. At follow-up, one patient had 2.6 cm of residual shortening, and two had overgrowth. Complications included one refracture. Three children developed superficial pin site drainage. Although most femur fractures can be treated with traction or casting, external fixation may simplify overall care in children

External fixation apparatus (EFA), hing-distraction and compression-distraction, were used in 30 hemophiliacs for the treatment of hemophilic arthrosis, intra-articular and extra-articular fractures, contractures, dislocations, as well as after synovectomy. The treatment was conducted under cryoprecipitate protection, the dose of which was significantly decreased after granulation around the pins was completed. Immediate and late results of the treatment showed its higher effectiveness as compared to other correction methods (gypsum etc). After correction with EFA repeated hemorrhage and traum

Thirty-two open fractures of the tibial shaft were treated with external fixation between 1973 and 1981. Early amputation was necessary in one patient. In the remainder, including 14 with extensive soft-tissue lesions, wound healing was obtained within 18 weeks, and the median time until full weight bearing without pain was 32 weeks (range 8-60 weeks). Two deep infections healed during the observation period. Among 26 patients examined 1-9 years after the injury, the result was excellent in six, good in 11, fair in four, and poor in five patients (including the amputation). One fracture had no

A technique of osteosynthesis of the clavicle with Hoffmann external fixation was used to treat twenty patients for selected indications, including an open fracture, inability of a patient to tolerate prolonged conservative treatment, or a painful non-union. No vascular or pleural complication was observed. The average time that the external fixator was retained was fifty-one days. All of the clavicles united well. Mobility of the shoulder returned to normal in all patients.

In ten cases of complex carpal fracture dislocations an external fixator between the radius and the second or third metacarpals was used as an adjunct to open reduction and internal fixation. The external fixator facilitates reduction, ligamentous repair, and internal fixation of the carpal bones. Furthermore, continuous postoperative distraction of the wrist maintains a stable reduction of the carpus permitting cast free after treatment and early active motion of the neighboring joints. This method is particularly useful for carpal stabilization when combined injuries of the same extremity co

External fixation is an accepted method of treatment for many injuries, however, only a limited number of reports concern its use in open, comminuted fractures of the hand. Ten patients with eleven fractures of the proximal phalanx were treated using the AO/ASIF small external fixator. Five fractures involved the interphalangeal joint (IPJ), two the metacarpophalangeal joint (MPJ), and four the shaft. Nine fractures were caused by small caliber gunshot wounds, one was due to a power saw injury, and one occurred during a three wheeler accident. The patients with interphalangeal joint involvemen

Ilizarov's method of external fixation with compression or distraction for lesions of the limbs demonstrates new possibilities in osteogenesis. Its performance with double horizontal pinning on several external rings calls for precautions to avoid lesions of the vessels, nerves and joints. This study, based on anatomic sections radiographed after opacification of the arterial system, makes it possible to propose rules for insertion of the pins. Insertion of the anteromedial pins of the thigh should be made 2 cm in front of the line of projection of the femoral artery, between the middle of the

Arthrodesis is currently the treatment of choice for symptomatic degenerative arthropathy of the ankle. Thirty-seven patients underwent arthrodesis for post-traumatic disorders using either a Hoffmann external fixator or a Calandruccio frame. There was degenerative joint disease in 19 (51%), septic arthritis in 11 (30%), severe comminution in five (14%), and uncontrollable equinus in two (5%). The patients were divided into two groups. Twenty-six (70%) were considered to have sustained high energy open or comminuted injuries and 11 (30%), low energy injuries. Twenty-nine (78%) achieved a radio

The Hoffmann external fixator has proved to be an effective means of stabilizing complex femoral fractures in patients in whom prolonged traction or internal fixation is not advisable. It provides for the prompt mobilization of multiple-trauma victims and assists in the evaluation of the "floating knee." Pin tract infections and lack of stability or adaptability of the frame are not major problems. Increased risks of delayed union or nonunion are often attributed to this mode of therapy. Within the limits of the present study these difficulties could not be directly related to the use of the f

Among the various methods for treating fractures of the base of the first metacarpal, use of the external fixator appears to be of interest for it maintains a satisfactory reduction while respecting the arch of the first web interspace as well as allowing early mobilization of all joints not involved in the injury. The miniaturization of the material, today possible, facilitates its use. The experience with 20 cases gained at the CTO de la CRAM de Strasbourg is presented. The authors discuss the utilization of the method, the practical means of doing it and their results. Its limitations are i

AIM: Limb reconstruction with external fixators requires appropriate pain management to promote effective analgesia and healing while minimising adverse events of the analgesic technique used. The objective of this prospective case series was to evaluate a multimodal analgesia regimen designed to reduce opioid requirements and hence reduce the opioid-related side effect profile.
MATERIALS AND METHODS: A prospective cohort of patients undergoing lower limb reconstruction surgery (LRS) were managed through an evidence-informed multimodal analgesia guideline (MMAG), including acetaminophen, preg…

BACKGROUND: Severe knee valgus/varus or complex multiplanar deformities are common in clinic. If not corrected in time, cartilage wear will be aggravated and initiate the osteoarthritis due to lower limb malalignment. Internal fixation is unable to correct severe complex deformities, especially when combined with lower limb discrepancy (LLD). Based on the self-designed digital six-axis external fixator Q spatial fixator (QSF), which can correct complex multiplanar deformities without changing structures, accuracy of correction can be improved significantly.
METHODS: This retrospective study i

Patella fractures represent for 0.5 to 1.5% of all bony injuries in adults. Open reduction and the modified tension-band technique is the most common surgical technique used for patellar fractures. The purpose of this study is to present the outcomes of 26 comminuted patellar fractures treated with circular external fixator (CEF) under arthroscopic control and discuss its potential advantages over conventional surgical methods. This retrospective study included 26 patients who had closed comminuted patellar fractures and treated by CEF under arthroscopic control between January 2002 and March
