OBJECTIVES: To report the results obtained in patients with intact tympanic membrane tympanosclerotic stapes fixation treated by labyrinthine fenestration and to discuss the value of this operation.
MATERIALS AND METHODS: Retrospective study of 28 cases of tympanosclerotic stapes fixation operated between 1982 and 2010. Labyrinthine fenestration was performed using a CO(2) laser. OBJECTIVE: To compare the effects of small fenestra stapedotomy with semiconductor diode laser and microdrill in patients with otosclerosis.
METHODS: Twenty-six patients (29 ears) undergoing stapedotomy with semiconductor diode laser and 19 patients (21 ears) with microdrill were compared for the hearing results and complication rates.

BACKGROUND: The postulated sites of perilymph fistulae involve otic capsule deficiencies, in particular, at the fissula ante fenestram. Histological studies have revealed this to be a channel extending from the middle ear, and becoming continuous with the inner ear medial to the anterior limit of the oval window. The causes for failure in surgery for stapedial otosclerosis are many. Most of these occur in the early postoperative period. OBJECTIVE: We examined the effect of 3 methods of closure of labyrinthine fenestrations.
STUDY DESIGN AND SETTING: A guinea pig animal model was developed to examine the effect of 3 methods of closure of labyrinthine fenestrations. These methods included bone wax alone, fascia and fibrin glue, and fascia alone. The use of the argon laser in chronic ear disease and otosclerosis surgery is reviewed. Its advantages and limitations are discussed, and safe parameters for use established. The argon laser is an effective tool to remove, cut, and coagulate tissue. It was used in over 500 microsurgical otologic procedures without deleterious effects. In this paper the results of vestibular fenestration and stapedioplasty in cases of congenital stapes deformity and vestibular window abnormality are presented. The problems in the surgical reconstruction of the middle ear transmission system, especially the importance of the areal ratio between the tympanic membrane and the vestibular window and the ossicular lever action, are discussed.
