Percutaneous needle lung aspiration for diagnosing pneumonitis in the patient with acquired immunodeficiency syndrome (AIDS).

Fourteen patients with acquired immunodeficiency syndrome (AIDS) or suspected AIDS underwent percutaneous needle lung aspiration (PNLA) for evaluation of 16 occurrences of acute pneumonitis. A 22-gauge spinal needle was passed 2 to 3 times in the area of greatest radiographic involvement under fluoroscopic guidance. The specimen was immediately placed on microscope slides for Gomori's methenamine silver and Papanicolaou staining. The needle was then flushed with sterile water for bacterial, Legionella, viral, mycobacterial, and fungal cultures, and for Legionella immunofluorescent staining. Di

Does bacteremia occur during flexible sigmoidoscopy?

Up to 10% of patients may have bacteremia after rigid sigmoidoscopy. The aim of our study was to determine the frequency of bacteremia accompanying flexible sigmoidoscopy. Blood samples for aerobic and anaerobic cultures were obtained before, during, and after flexible sigmoidoscopy in 100 patients who were examined a mean distance of 49.5 cm, range 15-60 cm, after a bowel preparation of two Fleet enemas. In one patient, a transient bacteremia with Streptococcus intermedius was documented and was attended by no associated clinical manifestations. This organism has been previously isolated from

Anatomical extent of fiberoptic sigmoidoscopy.

The anatomical extent of fiberoptic sigmoidoscopy was determined by placing a clip on the colonic mucosa and subsequently locating the clip on barium enema. A 60 cm exam viewed the entire sigmoid colon in 81% of patients. Exams of 50-55 cm, 40-45 cm, and 30-35 cm extent viewed the entire sigmoid colon in 68%, 27%, and 13%, respectively. The 50-55 cm and 60 cm exams collectively viewed 90% and 95% of the sigmoid colon surface, respectively. The 30-35 cm and 40-45 cm exams collectively viewed 53% and 70% of the sigmoid colon surface respectively. The anatomical extent of fiberoptic sigmoidoscopy

A 20-gauge illumination and irrigation instrument for pars plana vitrectomy.

We developed a 20-gauge combination illumination-irrigation probe for use in pars plana vitrectomy. This instrument makes the vitrectomy procedure less traumatic and technically easier.

Sigmoidoscopic examinations with rigid and flexible fiberoptic sigmoidoscopes in the surgeon's office: a comparative prospective study of effectiveness in 1,012 cases.

The results obtained from 1,012 examinations in an on-going, cooperative study indicate that the overall yield provided by use of the flexible fiberoptic sigmoidoscope is 3.2 times greater than that of examinations with the rigid sigmoidoscope. More than twice (2.4 times) the number of polyps and more than three times the number of cancers were detected with the flexible fiberoptic sigmoidoscope. Experienced endoscopists can perform an examination with the flexible fiberoptic sigmoidoscope expeditiously in the office with minimal patient preparation, a high level of patient and physician accep

Evaluation of fibergastroscopic biopsy in the diagnosis of gastric cancer: a study of 339 cases.

Three hundred and sixty-five biopsies under direct vision, using a fibergastroscope, were performed on 339 patients with gastric cancer from 1966 to 1975. Diagnostic accuracy improved with an increased in the number of biopsy specimens and reached 100% when 6 or more biopsy specimens were obtained. Positive specimens were obtained with 48.5% of the biopsied tissue from the center of the lesion (A), 52.2% from the inner margin of the lesion (B1), 19.6% from the outer margin of the lesion (B2), and 1.6% from the area around the lesion (C). The combined result of the biopsies from A and B1 was hi

[A case of congenital double pyloric canal (author's transl)].

In the case of 47-year-old patient who had been suffering for several months from a duodenal ulcer disease, a so called double pylorus was traced by applying fiberoptic gastroscopy. At the same time proof was found for the ulcera duodeni. Owing to the fiberoptic result of two regularly pyloric canals it was already doubted, that this was only a question of a secondary pyloro duodenal fistula caused by peptic ulcer. A particular and differentiating pathological anatomical examination of the surgical specimen showed as a result, that the patient was suffering from a congenital duplication of the

Inhibition of Pain Signal Conduction in Dorsal Root Ganglion Neurons by Optogenetic Technique Mediated with Upconversion Nanoparticles.

Traditional analgesic strategies not only have limited efficacy but also tend to cause side effects such as drug dependence and gastrointestinal tract damage. As a popular nonpharmacological neural modulation approach, optogenetic technology has seen increasingly in-depth research in the field of pain regulation. The dorsal root ganglion (DRG), a crucial relay station for pain information transmission, serves as a key target for pain regulation. Optogenetic technology holds broad prospects for pain regulation at the DRG. However, the application of traditional optogenetic technology in pain re

Percutaneous dilatational tracheostomy in a patient with a large midline aberrant artery.

PURPOSE: A midline aberrant artery is an absolute contraindication to percutaneous dilatational tracheostomy (PDT). In this case report, we highlight a number of technical modifications that resulted in a successful PDT in a patient with a large midline aberrant artery. CLINICAL FEATURES: A 72-yr-old woman with a posterior cranial fossa hematoma underwent PDT due to prolonged mechanical ventilation in the neurointensive care unit. On clinical examination, the patient had a huge, pulsatile midline neck mass. Ultrasonography (US) showed an aberrant artery that covered the entire tracheal length
