Between 1917 and 1984, we performed 217 arthrodesis of the ankle joint. Posttraumatic states were the cause of the osteoarthritis in 153 cases, whereas 118 of these cases were malleolar fractures. The accident X-rays were analyzed, and the fractures were classified in accordance with the classification of Weber (11). Our own system was used for the radiological classification of the degree of arthritis. The extent of malunion after the initial care was assessed. We found type-C malleolar fractures most frequently, the most striking malunion being the shortening of the fibula. In all fractures

INTRODUCTION: The Bosworth fracture-dislocation is a rare injury originally described as an irreducible dislocation of a distal fibula fracture where the proximal fibular fragment becomes incarcerated behind the distal tibia. There have since been a few cases described of an intact distal fibula with dislocation and similar difficulties in closed reduction attempts, typically seen with associated injuries to the ankle or proximal fibula. Regardless of the specific fracture or dislocation pattern, closed reduction attempts have consistently failed in the significant majority of these cases, whi

The present study compares a recent approach for congenital pseudarthrosis of the tibia, combining a free vascularized fibula graft and an Ilizarov external fixator. Between 1984 and 1993, seven vascularized fibula grafts were performed. In the first six cases, casts or unilateral external fixators were applied, and five of these cases were complicated by fractures of the graft, necessitating secondary procedures or long-term immobilization. The mean follow-up was 12.1 years. Five patients can now walk unassisted, and one uses a brace only outdoors. On average, a brace was required for 18 mont

The trail plate is a modular system consisting of a trail bar and synthesis modules made of stainless steel, originally developed for humans. This study aimed to evaluate the applicability and effectiveness of a modified trail plate for the treatment of long bone fractures in dogs. Initially, the trail plate was designed by using FEA to set the final model. Three trail plate sizes were designed according to corresponding locking screw diameters. The designs were created using computer-aided design software, and 3D models were produced from the commercial Ti-6Al-4 V powder via direct metal lase

Adult-acquired flatfoot deformity is a progressive flattening of the arch of the foot that results from posterior tibial tendon insufficiency with a predilection for middle-aged women. A lateralized force vector associated with hindfoot valgus in adult-acquired flatfoot produces lateral ankle pain due to impingement at the lateral hindfoot, which can even lead to stress fractures of the distal fibula. Here, we present the rare case of a 73-year-old woman who presented with stress fractures of the bilateral taluses and unilateral distal fibula accompanied by severe adult-acquired flatfoot defor

In rare cases of ankle fracture dislocation, the posterior tibial muscle tendon (TP tendon) is incarcerated between the tibia and fibula, thereby impeding reduction. Here we describe a case that presented with such a condition, in which ankle reduction was achieved and surgical repair of the incarcerated TP was delayed. The subject was a 30-year-old male who sustained a fracture dislocation of the left ankle (AO:44-C1.3) in a motorbike accident. After repairing the ankle dislocation, external fixation was performed and osteosynthesis was conducted 10 days after the injury. Plate fixation for t

The authors herein introduce a modification of parallel reconstruction with a vascularized fibula autograft (VFA) for cases of femur allograft complications. Conventional parallel reconstruction, in which the fibula with its vascular pedicle is placed on the medial side of the femur and allogeneic bone, may be an effective means to solve the allograft complications. However, the limited contact area between the fibula and femur/allogeneic bone can affect the bone healing ability. Furthermore, the rigid internal fixation method for the VFA may cause stress shielding and result in bone resorptio

BACKGROUND: Distal fibula fractures at the ankle level are common and are usually accompanied by ligament injuries. This study aims to evaluate the effects of ankle ligament ruptures on ankle joints, fracture instability, and plate stress after distal fibula fracture fixed with a plate created by finite element analysis modeling and loading applied to ligament rupture models that may accompany this fracture.
METHODS: A finite element model consisting of three-dimensional fibula, tibia, foot bones, and ankle ligaments was designed to investigate the effects of ligament injuries accompanying pl

UNLABELLED: We believed open reduction with internal fixation is required for supination-external rotation ankle fractures located at the level of the distal tibiofibular syndesmosis (Lauge-Hanssen SER II and Weber B) with 2 mm or more fibular fracture displacement. The rationale for surgery for these ankle fractures is based on the notion of elevated intraarticular contact pressures with lateral displacement. To diagnose these injuries, we presumed that in patients with a fibular fracture with at least 2 mm fracture displacement, the lateral malleolus and talus have moved at least 2 mm in a l

BACKGROUND: Although classically the fibula has been reported to be in external rotation after supination-external rotation (SER) or pronation-external rotation (PER) ankle fractures, a previous CT study demonstrated that what had traditionally been interpreted as external rotation of the distal fibular fracture fragment is actually internal rotation of the proximal fibular fragment. The purpose of this study was to evaluate a series of CT scans in patients who have suffered type IV SER or PER ankle fractures to assess the rotational deformity of the fibular fragment.

The role of fibular fixation in patients with distal tibia fractures is controversial. Although the stability of the fibula is critical in patients with syndesmotic instability or highly comminuted pilon fractures, fibular fixation in extraarticular distal tibia fractures or elementary intraarticular distal tibia fractures is more controversial. Biomechanical studies, as performed in sawbones or cadaveric models, denote advantages to fibular fixation with respect to specific uniplanar motion. However, the increased stability is susceptible to the fracture pattern of the tibia, fixation strateg

A severe closed pronation-eversion fracture-dislocation of the ankle that was irreducible by closed means was encountered. Exploration to accomplish open reduction revealed displacement of the tibialis posterior tendon through the diastasis between the distal tibia and fibula. The tibialis posterior tendon was found to pass posteriorly to an anteriorly between the distal tibia and fibula and laterally to medially across the anterior surfaces of the distal tibia and neck of the talus. Reduction was blocked by the tibialis posterior tendon in its abnormal course. After replacement of the tibiali

Bone repair models in animals may be considered relevant to human fracture healing to the extent that the sequence of events in the repair process in the model reflect the human fracture healing sequence. In the present study, the relevance of a recently developed segmental defect model in rat fibula to human fracture healing was investigated by evaluating temporal progression of rigidity of the fibula, mineral content of the repair site, and histological changes. In this model, a surgically created 2-mm-long defect was grafted with a 5-mm-long tubular specimen of demineralized bone matrix (DB

BACKGROUND: Determining the stability of ankle fractures, particularly Weber B fibula fractures, can be challenging. Ability to weight-bear after injury may be predictive of stability. We sought to determine whether patients' ability to weight-bear immediately after injury was an effective indicator for ankle stability following fracture.
METHODS: A prospective review was conducted of patients sustaining ankle fractures. Patients' ability to weight-bear after injury was elicited and correlated with ankle radiographs, which were deemed stable or unstable based on commonly used indices to asses

BACKGROUND: Comminuted fibular fractures can occur with pilon fractures as a result of valgus stress. Transverse fibular fractures can occur with varus deformation. No definitive guide for determining the proper location of tibial fixation exists. The purpose of this study was to identify optimal plate location for fixation of pilon fractures based on the orientation of the fibular fracture.
METHODS: One hundred two patients with 103 pilon fractures were identified who were definitively treated at our institution from 2004 to 2013. Pilon fractures were classified using the AO/OTA classificati

A 28-year-old man developed compartment syndrome in the right lower leg after fracture of the tibia and fibula. Despite fasciotomy, many arteries collapsed and union of the tibial and fibula fractures did not occur. Autologous bone marrow mononuclear cell (BMMNC) implantation for therapeutic angiogenesis and subsequent bone regeneration was performed and 4 weeks later, angiography showed a marked increase in collateral vessels surrounding the tibial fracture, and union was completed 6 months later. BMMNC implantation therapy might provide therapeutic angiogenesis and osteogensis in patients wi
