Management of the diabetic foot.

Treatment of diabetic foot pathology requires an accurate assessment of the etiologic factors involved. Dysvascular deterioration may be amenable to vascular reconstruction. When the vascular status cannot be improved, however, amputation is preferable to continued conservative care. Neuropathic ulceration is the product of decreased sensation, deformity, and increased pressure at the site of the ulcer. Pressure relief by conservative or surgical means is required for healing. Neuroarthropathy is increasing in frequency and results primarily from autonomic denervation. Protection from weight b…

Infections of the diabetic foot.

Diabetic foot infections are frequently much worse than the initial clinical appearance would indicate. Altered immune response, peripheral vascular disease, and neuropathy are key factors in the production of infection. Treatment must be based upon careful bacteriologic analysis and individualized antibiotic therapy. Surgical debridement, when indicated, is the mainstay of treatment. Partial foot amputation and salvage are always preferred over higher amputation when possible.

Seronegative arthropathies in the foot.

It has been seen that involvement of the foot in the seronegative arthropathies forms a regular and varied part of the clinical picture. This is often quite different from that seen in rheumatoid arthritis; its components, whether in joints, periarticular structures, or as surface manifestations, may be characteristic enough to raise the diagnosis of 'spondarthritis'. The features described, though characteristic of the spondarthritides, are, however, not pathognomonic. Thus, the osteolysis in psoriatic arthritis also occurs in neuropathic arthritis (e.g. syringomyelia, leprosy), psoriatic per…

Trauma to the ankle and foot.

Fractures and ligamentous injuries of the foot and ankle will be illustrated and discussed with respect to the normal anatomy of the region and the pathomechanics of the injury involved. Injuries to the ankle are best considered subdivided into those caused by inversion or eversion. Injuries to the foot include those caused by direct trauma to the bones of the hindfoot, as well as stress and other fractures of the forefoot. Fracture dislocations at various levels including the tarsometatarsal joint (Lisfranc injuries) will also be discussed. The importance of making the correct diagnosis will …

Neuropathic ulcers of the foot.

We report a prospective study of the causes and treatment of 26 long-standing neuropathic ulcers of the foot in 21 patients. The most important causal factor, well illustrated by pressure studies, was the presence of a dynamic or static deformity leading to local areas of peak pressure on insensitive skin. All but one of the 26 ulcers had healed after an average of 10 weeks of treatment in a light, skin-tight plaster cast, with the prohibition of weight-bearing. Recurrent ulceration was prevented in all but one foot by early operation to correct the causative deformity; this was performed afte…

Surgery of the rheumatoid foot.

The typical rheumatoid foot deformity is that of hallux valgus, bunion, hammer claw toes with contracted dorsal soft tissue, depressed metatarsal heads and associated painful callosities and corns both under metatarsal heads and on dorsal surfaces of cocked-up lesser toes. Deformity and pain will be amendable with surgery if patients are selected properly and correct surgery is performed to achieve a pain free weight-bearing foot.

Subtalar dislocation of the foot.

We studied a series of seventeen cases of subtalar dislocation of the foot. There were on anterior, twelve medial, and four lateral dislocations. Of the seventeen dislocations, fourteen were closed and three, open. Two of the four lateral dislocations were open. Associated fractures involving the talocalcaneal or talonavicular joints were noted in eight feet, including three of the four with a lateral dislocation. In two feet these fractures were noted only on post-reduction polytomography. Associated fractures of the ankle mortise and of the metatarsals were present in seven feet. Associated …

Ganglia of the foot and ankle.

This report documents the treatment of 33 foot and ankle ganglia. The average patient age was 39 years (versus 25 years for patients with ganglia about the wrist joint). Twenty-one ganglia were treated operatively and nine recurred. Three patients suffered cutaneous nerve injury. The other 12 ganglia were treated by aspiration and steroid injection. There were four recurrences, one of which was cured by a second injection. Two of the four recurrences were small and asymptomatic and required no treatment. There were no complications with aspiration and injection. In view of the high recurrence …

Xeroradiography of the diabetic foot.

The authors carried out comparative xeroradiographic and radiologic examinations of 34 patients suffering from diabetes. It has been established that xeroradiography allows an earlier and more convincing diagnosis of arterial calcifications, indurations of small size arteries, phleboliths and fistulous tracts. In diabetic gangrene the initial lesions and bone sequestra are more clearly visualized, as well as the intra-articular bone fragments and parosteal calcifications in diabetic osteoarthropathy.
