INTRODUCTION: the involvement of moulds in the fungal ungueal pathology is very variable. In fact a big confusion reigns because of numerous errors in the clinico-biological diagnosis, which could be responsible for therapeutic failure of onychomycosis.

Onychomycosis is a frequently treated fungal infection of the nail plate with morbidity in high-risk populations. The diagnosis often relies on histopathologic analysis of nail plate specimens with the assistance of special stains. Pathologists utilize periodic acid schiff (PAS) and/or Gomori methenamine silver (GMS) stains to highlight fungi within the nail plate. In a recent study of 51 PAS-negative nail cases, it was concluded that GMS is superior to PAS in the diagnosis of onychomycosis. An 8-year-old girl was referred for evaluation of multiple lesions on the neck, axillae, and genitalia. A great number of milia-like lesions were also noticed on the palms and discrete keratotic plugs on the soles. Histopathology from lesions on the neck, axilla, and vulva were consistent with syringoma. On the palms and soles, eccrine ducts cystically dilated filled with proteinaceous material and calcifications were found. Although the nature of palmar and plantar lesions in our patient is uncertain, we think that they should be related to the other syringomas on her skin.

Laugier-Hunziker pigmentation (LHP) is an acquired disorder of hypermelanosis characterized by mucocutaneous hyperpigmentation. LHP may resemble various disorders characterized by mucocutaneous pigmentation. An atopic patient with tinea unguium, immediate hypersensitivity to Trichophyton, and elevated specific IgE antibody to Trichophyton had recalcitrant hand and foot eczema that resolved following the administration of systemic antifungal therapy for her tinea unguium. The patient fulfills criteria for the atopic-chronic-dermatophytosis syndrome described by Jones. The authors suggest that the patient's atopic dermatitis may have been exacerbated by her chronic dermatophyte infection and Trichophyton hypersensitivity.

Four patients with colorectal cancer treated with tegafur (a fluoropyrimidine structurally similar to 5-fluorouracil) noted a macular, spotted hyperpigmentation limited to the palms, soles, nails, and glans penis. Histopathologic examination disclosed epidermal basal hyperpigmentation with a lentiginous pattern. Mucocutaneous lesions resolved spontaneously two months after treatment was discontinued. This peculiar phenomenon seems to represent a previously unreported side effect of this cytotoxic drug.

During a 3-year period 3332 patients were patch tested with ICDRG standard series including 4 rubber mixes. Additional tests were performed with individual rubber chemicals and suspected solid rubber products. Rubber sensitivity was detected in 158 (4.7%) patients. Thiuram mix caused most of the positive reactions (73) followed by carba mix (51), PPD mix (40) and mercapto mix (19). The effect of oral doses of colchicine on pustule formation was studied in 32 patients with pustulosis palmaris et plantaris without associated psoriasis. Oral administration of colchicine, 1 to 2 mg daily, was started during periods of disease exacerbation, and the dosage was gradually decreased to 0.5 to 1 mg daily in responsive patients. Thirteen patients showed complete clearing of pustulation, and 14 patients showed a remarkable reduction in pustule formation after two to eight weeks of treatment.
