Fracture-dislocation of the elbow: diagnosis, treatment, and prognosis.

There are two main goals for the surgeon treating injuries around the elbow joint: the maintenance of a stable, concentric reduction of the joint and the initiation of early motion to help prevent stiffness and maximize functional out-come. The presence of a fracture decreases joint stability and increases the risk for early subluxation or dislocation with motion. Operative repair thus is indicated for most of these injuries to restore sufficient osseoligamentous support to allow safe, early motion and provide a stable functional elbow in the long term.

Conjoined fracture-dislocation of the forearm at the elbow.

Conjoined fracture-dislocation of the elbow were first described by Marotte in 1982. In this study, a total of 41 patients treated between 1975 and 1995 at the Rizzoli Orthopaedic Institute and 4 patients treated between 1997 and 2000 at the Division of Orthopaedics at the Maggiore Hospital are evaluated. Based on the Marotte classification, the patients are divided into 4 groups, by site of dislocation of the radial capitellum and amount of comminution of the fracture of the ulna. A clinical and radiographic evaluation was made after a mean follow-up of 5.3 years based on Anderson criteria. T

Bilateral isolated tarsal navicular fracture dislocation: a case report.

Fracture-dislocation of the tarsal navicular is rare injury because of strong ligamentous support on both its plantar and dorsal sides. The authors report a case of a twenty-five-year-old man who sustained bilateral isolated tarsal navicular fracture-dislocations, treated by open reduction and internal fixation combined with external fixation. The patient reported no difficulty in activities of daily living, but T1-weighted magnetic resonance imaging showed a low-intensity area in right tarsal navicular, raising suspicion of osteonecrosis when he was last seen two year three months after opera

Tibial-talar dislocation without fracture: treatment principles and outcome.

The incidence of tibial-talar dislocations without fracture is unknown and has been sparsely reported in the literature. The diagnosis of the injury is straightforward with the appropriate examination and roentgenograms. Good to excellent results can be achieved with open or closed dislocations treated by closed reduction and immobilization for 4-6 weeks. Although open dislocations require irrigation, debridement, and possibly delayed closure, controversy exists with regard to acute ligament repair. Because good to excellent results are possible without acute ligament repair, and delayed repai

Treatment of shoulder dislocation with ipsilateral humeral shaft fracture.

A case presentation of shoulder dislocation with ipsilateral humeral shaft fracture is presented along with a review of the literature regarding nine reported cases. In the current case, closed treatment of the humeral shaft fracture was undertaken with an unsuccessful attempt at closed reduction of the anterior shoulder dislocation, resulting in a radial nerve deficit. Successful treatment required open reduction of the humeral fracture with compression plating followed by closed reduction of the shoulder dislocation. We compare this treatment (with outcome) to other methods of treatment.

Dislocation of the shoulder with ipsilateral humeral shaft fracture.

Dislocation of the shoulder with ipsilateral shaft fracture is an uncommon, but serious injury. I present one case report and the guidelines used for treatment. The method employed was Hackethal's pins through open reduction. The dislocation was reduced during surgery. The fracture and dislocation healed without any problems, and there was no damage to the radial nerve. After a recent follow-up, the patient was allowed to return to work without restrictions, and he has regained normal mobility.

Transscaphoid perilunate fracture dislocation and pseudarthrosis of the scaphoid.

The results of conservative treatment of a perilunate fracture dislocation were retrospectively studied. In six patients, the treatment consisted of closed reduction and immobilization in a below the elbow cast for 12 to 16 weeks. Five patients had a compromised healing of the scaphoid fracture. The frail vascularization of the scaphoid, extensive ligamentous injury and possible mid-carpal instability in case of perilunate fracture dislocation as well as a non-anatomical reduction of a scaphoid fracture are causative factors in ensuing scaphoid pseudarthrosis. If anatomical reduction of a peri…

Triquetral fracture associated with hamate dislocation: a case report.

A case of hamate dislocation associated with triquetral fracture is presented. No other carpal or metacarpal injury was involved. The mechanism of injury was volar flexion and ulnar deviation. An attempt at closed treatment failed because the proximal tip of the hamate was incarcerated between the triquetral fragments. Open reduction, K-wire fixation, and casting were done. The patient was able to use her hand nearly normally 12 weeks postoperatively.

Acromioclavicular dislocation associated with fracture of the coracoid process.

Two cases of acromioclavicular dislocation associated with fracture of the coracoid process are described. This association is unusual and the coracoid fracture may only be recognised on an axillary radiograph of the shoulder. Both the cases were treated conservatively, with good results. The management and outcome of other cases described in the literature are reviewed and discussed.

[Isolated trapezium dislocation fracture after a serious traffic accident].

Fractures and luxations in the area of the carpal bones are in comparison with other fractures of the upper and lower extremities comparatively rare. Isolated fractures of luxations of the distal row of the carpal bones, by comparison with those of the proximal row, are still rarer incidents. Because of tight ligamentous unions between the carpal bones on the one hand and between the distal row of the carpal and the metacarpal bones on the other hand, a luxation or fracture is connected with a violent contusion or sprain. In the case of seriously or multiply injured persons the carpal injuries

Posterior fracture-dislocation of the hip: an epidemiologic review.

A retrospective study of 46 posterior fracture-dislocations of the hip showed that reduction of the dislocation within 24 hours was the most important initial treatment. Open reduction and fixation of the fracture could be delayed up to 4 weeks to allow associated injuries to be treated, and this technique was indicated to remove intra-articular bone fragments or for residual instability. Long-term disability resulted in every patient, regardless of the severity of the initial injury or the method of treatment. Half of the patients never returned to work. Of the 46 patients 43 were unrestraine

Lumbar fracture-dislocation related to range-of-motion exercises.

Range-of-motion exercises are an established and widely accepted therapeutic modality in the treatment of the person with spinal cord injury and are usually uneventful. Occasionally, however, accidents do occur; among them are fractures and dislocations of limbs. An unusual "slice" fracture of L4, related to range of motion exercises, occurred in a young man who was a quadriplegic and converted him from upper to lower motor neuron bowel, bladder and sexual function. This iatrogenic fracture, although extremely rare, can be prevented if symptoms are properly interpreted.

Fracture-dislocation of the mandibular condyle: report of case.

A rare type of ramus fracture-dislocation has been presented. The inadequacy of standard mandibular fracture radiographic views for proper diagnosis of variable dislocations has been stressed. It was noted that proper positioning of the mandible in panoramic radiography would facilitate a satisfactory radiographic survey of the glenoid fossa and the condyle. In this case, interpositional silastic gap arthroplasty was performed to correct an extraarticular ankylosis and to restore mandibular function.
