Acute management of fractures involving the head and neck of the distal ulna associated with comminuted unstable fractures of the distal radius remains difficult and controversial. Fifteen consecutive such cases treated with combined external and internal fixation together with primary resection of comminuted distal ulna fracture fragments and reconstruction of the periosteal sleeve and triangular fibrocartilaginous complex are reviewed. At an average follow-up of 5.8 years, all patients were assessed for range of motion, strength, pain and function, and radiographic appearance. All had a rang

Intra-articular incongruity of the distal radius at the radiocarpal joint is a bad prognostic feature; reduction by closed or open operative techniques is important. However, both techniques have limitations. We describe a new technique of reduction of the lunate facet under radiographic control, with very limited operative exposure and tissue trauma. In two cases anatomic restoration of the radiocarpal joint was obtained by this technique and maintained with external and limited internal fixation. Follow-up results are very encouraging. Currently we suggest use of this simple technique when f

Comminuted patellar fractures are rare in pediatric populations, often resulting from high-energy trauma such as sports or road accidents. Surgical management is essential to restore the articular surface and extensor mechanism of the knee. However, achieving anatomical reduction and rigid fixation can be challenging, potentially leading to poor functional outcomes. We report the case of a 12-year-old boy who sustained a comminuted patellar fracture following a motorcycle accident. The patient presented with significant knee swelling, pain, and inability to bear weight. Radiographs and a CT sc

In high-energy midfoot fractures with severe soft-tissue damage, immediate osteosynthesis is often challenging owing to the lack of subcutaneous tissue and forefoot swelling. We herein report a case of comminuted first-metatarsal fracture with marked shortening and poor skin condition, managed using a staged approach with external fixation. A 52-year-old man presented with left forefoot pain after a tiller-related mishap. Computed tomography revealed a comminuted first-metatarsal fracture with shortening. Owing to poor condition of the skin, a staged treatment was adopted. In the first stage,

PURPOSE: To identify the incidence of dorsal comminution using computed tomography (CT) images and identify predictors of this phenomenon in older adults with low-energy distal radius fractures (DRFs).
METHODS: A total of 150 patients aged > 50 years with fall-induced dorsally angulated DRFs were enrolled in this study. Patients were divided into two groups based on the presence of dorsal comminution, defined as a metaphyseal void of greater than one-third of the maximum posterior to anterior depth of the bone on at least three cuts in the sagittal plane on post-reduction CT images. Data on p

BACKGROUND: Isolated, highly comminuted trapezium fractures are extremely rare. While they can be initially diagnosed with plain radiographs, a computed tomography scan of the wrist will more clearly reveal the fracture details and aid in appropriate preoperative planning. Restoration of the fracture with a congruent reduction of the adjacent joints is mandatory for a favorable prognosis.
CASE DESCRIPTION: A case of a highly comminuted, biarticular, trapezium fracture in a young male patient that was treated operatively with open reduction and internal fixation (ORIF) with a cannulated minisc

Background The distal radius fracture is the most common fracture in the United States. Achieving stable reduction and fixation of complex fracture patterns can be challenging. In order to help maintain reduction of comminuted fracture to simplify plating, the calcium phosphate-based bone putty Montage has been developed. Questions/Purposes Does Montage assist in achieving stable reduction and fixation of complex distal radius fractures with an acceptable complication profile? Patient and Methods We retrospectively analyzed all patients who were treated intraoperatively with Montage bone pu

BACKGROUND: Compared with open comminuted calcaneal fractures, less emphasis is placed on postoperative surgical site infection (SSI) of closed comminuted calcaneal fractures. This study aimed to identify the risk factors associated with SSI and build a nomogram model to visualize the risk factors for postoperative SSI.
METHODS: We retrospectively collected patients with closed comminuted calcaneal fractures from the Second Affiliated Hospital of Wenzhou Medical University database from 2017 to 2020. Risk factors were identified by logistics regression analysis, and the predictive value of ri

One of the common postoperative complications of comminuted fractures of frontal sinus are contour deformity, hardware extrusion, adherence and/or palpability, and skin thinning. We are presenting our novel technique with pericranial flaps to decrease these complications. The study was performed from January 2019 to January 2020, on 40 cases of comminuted fractures of anterior wall of frontal sinus, 28 men and 12 women, with an average age of 41 years. Injury resulted from motor vehicle crashes (n = 25), motorcycle crashes (n = 11), and falling from heights (n = 4). Our novel technique involve

A case from practice is presented - the infliction of fatal self-harm with clerical scissors with the formation of a comminuted depressed terrace-like fracture of the cranial vault. The given case is interesting for forensic experts, first of all, because it demonstrates the possibility of suicide with an unusual object - scissors and an area of injury that is unlikely for self-infliction - the vault of the skull.

Surgical fixation of hip fractures in patients with below knee amputation is challenging due to the difficulty in obtaining optimal traction for reduction of the fracture. Surgeons may face difficulty in positioning such patients on the traction table due to the absence of the foot and distal lower limb. There are several techniques described to overcome this technical difficulty. In this case report, we present a case of a 64-year old gentleman with bilateral below knee amputation presenting with a comminuted right intertrochanteric fracture. We highlight a simple and effective method of appl

A four-year old male Labrador Retriever was admitted with head trauma after being hit by a car. The dog had sustained multiple nasal, maxillary, and frontal bone fractures that resulted in separation of the maxilla from the base of the skull. A severely comminuted left zygomatic arch fracture was also present. These fractures were all repaired using a point contact, locking titanium plate system, in a single procedure that resulted in excellent postoperative occlusion and immediate function. Healing was uneventful. Full function and excellent cosmetic appearance were evident 13 months after su

OBJECTIVE: To evaluate the outcomes of arthroscopy-assisted treatment of severe comminuted distal radial fracture with external fixators and kirschner wire fixation.
METHODS: Twenty-seven cases of severe comminuted distal radial fracture treated between March, 2010 and January, 2012 were reviewed. During the operation, the carpal joint space was expanded with the external fixator, and the fracture was fixed by Kirschner wire after open reduction. The carpal joint was observed intraoperatively with arthroscopy to ensure full reduction, and the distal posterior interosseous nerve was then sever
