Acute ammonia-induced intoxication is a kind of acute irritant gas poisoning, which mainly causes systemic inflammatory reaction and immune dysfunction through direct and indirect lung injury, leading to chemogenic pulmonary edema and acute respiratory distress syndrome (ARDS). The mechanism of its toxic injury is complex, and the fatality rate of patients increases significantly once ARDS occurs. In this paper, the patient with acute and severe ammonia-induced intoxication was successfully treated by extracorporeal membrane oxygenation (ECMO) combined with fiberoptic bronchoscopy and other cr…

The authors evaluated an integrated approach for the screening of drugs in biosamples consisting of gas chromatography/mass spectrometry analysis of serum or whole blood (SB/GC-MS) and of high-performance liquid chromatographic and ultraviolet (HPLC-UV) analysis of urine with the REMEDi HS Biorad system (U/REM) (Bio-rad; Segrate, MI, Italy). Urine and blood samples from 26 suspected intoxicated patients and from 22 suspected lethal poisoning cases were examined. Eighty-one of the 99 parent drugs/main metabolites detected were identified by SB/GC-MS and 54 with U/REM. Thirty-six drugs/metabolit

Metal phosphides, particularly zinc and aluminum phosphide, occasionally poison horses and other equids following their use as rodenticides and insecticides. Grain-based aluminum phosphide baits are used to control rodents such as prairie dogs. The clinical course in intoxicated horses is short (<24-48 h), and animals may be found dead. Hepatic lesions caused by phosphine poisoning are not well described. Laboratory confirmation depends on detecting phosphine gas in gastric contents. Eight horses and a mule were exposed to zinc phosphide used to control prairie dogs on a Wyoming ranch. Three o

INTRODUCTION: Carbon monoxide (CO) is a colorless and odorless gas that is a leading cause of environmental poisoning in the USA with substantial mortality and morbidity. The mechanism of CO poisoning is complex and includes hypoxia, inflammation, and leukocyte sequestration in brain microvessel segments leading to increased reactive oxygen species. Another important pathway is the effects of CO on the mitochondria, specifically at cytochrome c oxidase, also known as Complex IV (CIV). The purpose of this ongoing study is the preliminary development of a porcine model of CO poisoning for invest

Accurately assessing potential side effects following botulinum neurotoxin (BoNT) injection remains a formidable challenge. To address this issue, an innovative approach is developed that combines a wearable temperature sensor with a sophisticated volatilomics technique, aimed at facilitating the rapid and convenient prediction of potential physical discomfort related to latrogenic botulism. The investigation identifies five volatile organic compounds (VOCs)-acetone, styrene, ethanol, 2-pentanone, and n-butano-as promising markers indicative of BoNT poisoning. Specifically, a handheld breath a

Human xylazine poisoning is uncommon. This report describes the use of xylazine for intentional poisoning with criminal intent. Two incidents occurred within 3 weeks: the first involved one victim, and the second involved two victims. The clinical presentations were brief coma, bradycardia, hypotension, and hyperglycemia. The victims recalled having been given a drink from a stranger in a hospital waiting room before loss of consciousness. In the first case, general drug screening by gas chromatography/mass spectrometry (MS) revealed xylazine in the gastric contents, but liquid chromatography-

A 31-year-old man presented with abdominal pain and vomiting with a smell of garlic and rotten fish. He was brought to the ER being circulatory affected, metabolic acidotic and he developed more episodes of arrhythmia. Oral poisoning with aluminium phosphide is a very serious condition due to release of the toxic phosphine gas. Treatment is symptomatic. To avoid contamination of the staff is it important to use air-tight containers for excretions and furthermore to use chemical clothing and breathing protection.

Cyanide is a powerful and rapidly acting poison. In Japan, cyanide poisoning is rare, and regular cyanide testing can be costly and time consuming. In contrast, alcohol analysis is routinely performed in most forensic laboratories. In this study, we attempted to develop a method for the simultaneous quantification of cyanide and alcohols in blood using headspace gas chromatography (HS-GC). As nitrogen-phosphorus detection (NPD) is more sensitive to hydrogen cyanide than mass spectrometry (MS), a Deans switch was used to switch the detectors during a single run. The separation provided by three

BACKGROUND: The use of suicide methods largely determines the outcome of suicide acts. However, no existing meta-analysis has assessed the c … o be the most lethal method (CFR:89.7%), followed by hanging/suffocation (84.5%), drowning (80.4%), gas poisoning (56.6%), jumping (46.7%), drug/liquid poisoning (8.0%) and cutting (4.0%). The rank of the lethality for different methods remained relatively stable across study setting, sex and age group. Method-specific CFRs for males and females were similar fo …

Fatal poisoning due to butane inhalation has been described in the clinical and forensic literature. We report the first case of a seventy-year-old obese man with a history of sleep apnea and depression, who was found dead in bed after inhaling butane gas through a homemade adaptation of his own continuous positive airway pressure (CPAP) face mask. The death scene investigation, autopsy findings and toxicological results are described. The cause of death was suspected to be due to asphyxia through butane inhalation.

BACKGROUND: The use of chemical weapons in terrorist attacks, though rare, remains a significant challenge and concern due to their ability to inflict mass casualties. Chemical attacks remain a topic of interest for Disaster Medicine (DM) and Counter-Terrorism Medicine (CTM) specialists and are clearly an area in need of enhanced preparedness. This study aims to provide an epidemiological description of all terrorism-related attacks using chemical agents as a primary weapon, sustained from 1970 - 2019. These data will be useful in the development of education programs in CTM and provide an ins

BACKGROUND: Carbon monoxide (CO) poisoning is associated with various sources of combustion, but its occurrence from hookah (waterpipe) smoking is often under-recognized. Hookah smoking can produce high levels of CO due to the burning of charcoal, leading to CO toxicity even in the absence of tobacco use. Symptoms of CO poisoning can be nonspecific, often delaying diagnosis in the emergency department (ED).
CASE REPORT: We report here a 37-year-old male brought in by ambulance after being found down by a bystander. The patient became unresponsive and apneic. Venous blood gas showed a pH of 7.

A man in his late twenties collapsed shortly after intravenously injecting himself with methamphetamine (MA). He slipped into a deep coma and remained in this condition for 9 days, until his death. Autopsy revealed severe brain edema and localized subarachnoid hemorrhages in the cerebrum and cerebellum. Histopathological examination revealed myocardial necrosis in the left ventricle, rhabdomyolysis and bronchopneumonia. Blood derived from the cadaver was found to have high levels of blood urea nitrogen and creatinine, suggesting he experienced acute renal failure probably due to rhabdomyolysis

BACKGROUND: Immunochromatography-based urine drug screening (UDS) can have false-positive or false-negative results; thus, interpretation of results requires careful evaluation. A UDS-negative result simply indicates that target drugs were not detected in urine and does not rule out acute poisoning. This study aimed to determine the presence of drugs in the blood and blood drug concentrations in suspected cases of acute poisoning with UDS-negative results. METHODS: In this single-center, retrospective, observational study, we included 501 patients who attended Tokai University Hospital Advance
