Randomized, clinical trial on diathermy and scalpel incisions in elective general surgery.

BackgroundSince a long time skin incisions have routinely been made with scalpels. Now a day there is a shift in trend from this method to electrosurgical skin incisions. However, fear of bad scars and improper wound healing has prevented its wide spread use. This Study aimed to compare both methods of skin incisions for different variables.ObjectivesThe aim of this study was to examine incisional time, blood loss during incision and postoperative wound complications and pain with both methods of skin incision.Patients and methodsA prospective, comparative and randomized study was conducted at

Postoperative QT interval prolongation in patients undergoing noncardiac surgery under general anesthesia.

BACKGROUND: Abnormal cardiac repolarization, indicated by a prolongation of the QT interval, increases the risk for torsades de pointes, a potentially life-threatening arrhythmia. Many perioperatively administered drugs and conditions prolong the QT interval. Despite several reports of perioperative torsades de pointes, systematic evidence regarding perioperative QT interval prolongation is limited. METHODS: Serial postoperative 12-lead electrocardiograms were obtained from 469 adult patients undergoing major noncardiac surgery under general anesthesia. Heart rate corrected QT-interval durati

General surgery in patients with a bleeding diathesis: how we do it.

BACKGROUND: This study was undertaken to assess perioperative management, postoperative complications, and the adequacy of perioperative plasma factor levels in a regional hemophilia center. METHODS: A total of 113 consecutive patients (75 men, 38 women; median age 48 years, range 18-86 years) with bleeding disorders undergoing general surgical and endoscopic procedures (144 procedures: 15 urgent, 129 elective) were reviewed. The episodes were identified from a prospectively collected database at a regional hemophilia center from 1998 to the end of 2008. In all, 46% of the surgical patients h

Preventing infection in general surgery: improvements through education of surgeons by surgeons.

Surgical patients are at particular risk of healthcare-associated infection (HCAI) due to the presence of a surgical site leading to surgical site infection (SSI), and because of the need for intravascular access resulting in catheter-related bloodstream infection (CRBSI). A two-year initiative commenced with an initial audit of surgical practice; this was used to inform the development of a targeted educational initiative by surgeons specifically for surgical trainees. Parameters assessed during the initial audit and a further audit after the educational initiative were related to intra- and

Postoperative psychiatric disorders in general thoracic surgery: incidence, risk factors and outcomes.

OBJECTIVE: Postoperative psychiatric disorders (PPDs) may complicate the post-surgical outcome. We analysed the types, incidences, risk factors and outcomes of the PPDs in non-cardiac thoracic surgery patients. METHODS: All patients (n=100) undergoing major non-cardiac thoracic surgery from January 2004 to March 2005 were investigated prospectively. The diagnosis of PPD was made based on the Diagnosis and Statistical Manual of Mental Disorders. The patients were grouped into two according to the presence (group I) or absence (group II) of PPD. Data on pre-, per- and postoperative factors, and

Minimally invasive video-assisted parathyroidectomy. Initial experience in a General Surgery Department.

BACKGROUND: The aim of this study is to analyze our preliminary results from minimally invasive video-assisted parathyroidectomy (MIVAP) and demonstrate the feasibility of MIVAP also in non-referral centers. MATERIAL AND METHODS: During a period from June 2005 to January 2008, in the General Surgery Department of University of Trieste, we operated on 39 patients with primary hyperparathyroidism (pHPT). MIVAP by an anterior approach was proposed for 23 (59%) patients with sporadic pHPT and one unequivocally enlarged parathyroid gland on pre-operative ultrasound and 99mTc-SestaMIBI scintigraphy…

Helium insufflation with sevoflurane general anesthesia and spontaneous ventilation during airway surgery.

PURPOSE: The therapeutic benefits and clinical applications of helium-oxygen gas mixtures were first reported over 70 years ago. Since then, helium has been used in the treatment of obstructive processes of both the upper and lower airways. We report a unique case regarding helium insufflation with sevoflurane anesthesia and spontaneous ventilation for airway surgery. CLINICAL FEATURES: A helium-oxygen mixture was administered during airway surgery in an 8-year-old boy with obstruction of the upper airway. Following the introduction of a helium-oxygen mixture during general anesthesia with sp

Paediatric surgery carried out by general surgeons: a rural New Zealand experience.

BACKGROUND: There are increasing moves towards centralization in paediatric surgery. With only four paediatric tertiary centres in New Zealand, many general surgeons still routinely carry out paediatric surgery. We present an audit of paediatric surgical patients admitted to our general surgical unit. METHODS: Data were prospectively recorded using a standardized pro forma on all children aged 15 years and below, who presented to general surgery between 11 December 2005 and 11 December 2006. RESULTS: There were 209 admissions (194 children); the median age was 8 years (range 6 weeks to 15 ye…

[Case of coronary spasm during thoracic surgery under combined epidural-general anesthesia].

A 70-year-old woman, who had neither complication nor risk factors of ischemic heart disease, was scheduled for a partial resection of the left lung. After an epidural catheter was placed at the T7-8 interspace, general anesthesia was induced and maintained with propofol and fentanyl. Once stable vital signs had been confirmed, 3 ml of 0.5% ropivacaine was given through the epidural catheter. Bradycardia with hypotension progressively developed and continued despite the intravenous administration of phenylephrine, ephedrine, and atropine. Fourteen minutes after the injection of ropivacaine, th

[Malignant hyperthermia--complication of general anesthesia in the clinic of maxillofacial surgery].

Malignant hyperthermia (MH) is a rare genetic disease that manifests itself more frequently in the use of general anesthetics that are triggers of MH and results in death when specific treatment is not performed. The authors have observed a case of MH during anesthesia at maxillofacial surgery. The patient could not be saved irrespective of the attempts being made.

Why do residents leave general surgery? The hidden problem in today's programs.

OBJECTIVE: Much has been written and discussed about the reasons for reduced interest in surgery, but few institutions have chosen to examine the loss or attrition of general surgery residents from their own programs. In preparation for an upcoming Residency Review Committee analysis of our program, we took the opportunity to examine the reasons for attrition in our own institution. DESIGN, SETTING, AND PARTICIPANTS: During the years 1990 to 2003, 120 categorical residents were admitted into our general surgery residency program. Residents who matched into preliminary positions or non-5-year …

[Interdisciplinary surgery of the thorax-from the general surgeon's point of view].

Interdisciplinary professional management is the most important basic principle for successfully mastering thoracic operations including the thoracic wall and adjacent regions such as neck, axilla, mediastinal vessels, upper limb, and spine. Extended oncological resection in advanced malignant diseases, side-effects of radiotherapy and trauma explain the diversity of possible operative procedures. For technical success, the necessity of vascular grafting, reconstruction of the brachial plexus, spine surgery, cardiac surgery, plastic thoracic wall reconstruction, stabilization of the thoracic w

Epidural versus general anesthesia for twin-twin transfusion syndrome requiring fetal surgery.

OBJECTIVES: This paper describes our experiences with epidural and general anesthesia for selective fetoscopic laser photocoagulation (SFLP) of placental vascular anastomoses in patients with severe twin-twin transfusion syndrome (TTTS). METHODS: Retrospective review of 29 anesthetic and surgical operative records of 28 women undergoing SFLP at The Children's Hospital of Philadelphia from July 1996 to June 2001. RESULTS: Patients with anterior placentas were more likely to receive a general anesthetic. Patients receiving an epidural anesthetic received significantly more intravenous (i.v.) c

Parathyroid surgery using monitored anesthesia care as an alternative to general anesthesia.

BACKGROUND: Although there have been several reports in the literature describing a renewed interest in performing thyroid surgery under local anesthesia (LA), there has been little information regarding parathyroid surgery under local anesthesia. METHODS: We retrospectively reviewed our experience of 49 LA parathyroid patients over a 9-year period at a single institution. A bilateral cervical block (C2-C3) was administered by a single surgeon using lidocaine and bupivacaine. RESULTS: The study included 39 females and 10 males with an average age of 62 years (range, 35-89 years). Every surge

[Surgery in childhood at the hospital for basic and routine general management].

Pediatric surgery in general surgical hospitals involves treatment of blunt thoracic trauma, blunt abdominal trauma and acute abdominal and inguinal surgery. Special surgical techniques are: opening of the bursa omentalis to detect pancreas ruptures; partial liver resections in deep organ ruptures instead of wound readaptation due to postoperative bleeding, gall bladder fistulae and rising infections; organ-protecting spleen resections. Appendectomy is indicated in children in cases of non-inflammatory, congenital chronic abdominal pain (dissociated visceral growth); side-to-side anastomosis i

Morbid obesity: a comparison between a general population and obesity surgery patients.

Characteristics of morbidity obese adults in the general population and morbidly obese adults presenting for obesity surgery were compared. Black morbidly obese women in the general population were less obese than black women obesity surgery patients, showed less emotional distress, and had fewer abnormal eating behaviours; age and socio-economic status were similar. A greater proportion of morbidly obese surgery patients were white women than would be expected either on the basis of the prevalence of morbid obesity in the general population or on census data. It was hypothesized that black wo

Spinal anaesthesia or general anaesthesia for emergency hip surgery in elderly patients.

One hundred and thirty-two elderly patients undergoing emergency hip surgery were randomly allocated to receive subarachnoid block (SAB) or general anaesthesia (GA). Using the 125.I fibrinogen uptake test, deep vein thrombosis was found to occur in 17 of 37 patients in the SAB group and 30 of 39 patients in the GA group (P 0.05). Blood loss was 513 ml (+/- SEM 44) in the SAB group and 714 ml (+/- SEM 67) in the GA group (P less than 0.01). Hypoxaemia was present preoperatively (mean PaO2 9.2 kPa). Immediately following general anaesthesia, the mean fall in PaO2 was 0.86 kPa compared with preop
