Giant-cell tumour of the sacrum in a child.

Giant-cell tumours rarely are seen in children before epiphyseal fusion occurs and such tumours in the spine are uncommon. The authors report the case of a giant-cell tumour of the sacrum that developed in a 12-year-old girl before epiphyseal fusion. Embolization with activated microfibrillar collagen relieved her symptoms and was carried out twice before progression of the tumour necessitated radiotherapy. Giant-cell tumours of the spine are particularly difficult to manage since the accepted treatment for such tumours at other sites is wide resection, excisional curettage or surgical curetta

Giant cell arteritis with pulmonary involvement.

Giant cell arteritis (GCA) may present as pyrexia of unknown origin with profuse night sweats, pain on mastication, headache, pain in the region of the temporal arteries, polymyalgia rheumatica, myocardial infarction or dissecting aortic aneurysm. Few cases with pulmonary involvement have been described. We report a patient with temporal arteritis preceded by pulmonary vascular disease.

Disseminated visceral giant cell arteritis.

Autopsy findings of disseminated visceral giant cell arteritis in a 78-year-old man who was in chronic renal failure for two years were presented. Arteritis had not been suspected clinically and definite diagnosis was made after the autopsy. Only small arteries and arterioles were involved. The arteritis was characterized by granulomatous lesion with exclusively multinucleated giant cells. Fibrinoid necrosis with polymorphonuclear leukocyte infiltration was found only in one arteriole of the stomach. The arteritis was distributed in the heart, lungs, kidneys, gastrointestinal tract, liver, gal…

Giant cell reparative granuloma of the hand.

A case of recurrent giant cell reparative granuloma of the hand is reported. The radiographs showed a lytic, expansile lesion that extended from the secondary ossification center proximally to the articular surface distally. Treatment consisted of excision of the involved proximal phalanx and interposition bone graft. The histopathologic findings were typical for this benign, nonneoplastic process: a cellular fibrous stroma with many giant cells. The differentiation of giant cell reparative granuloma from giant cell tumor, aneurysmal bone cyst, and brown tumor of hyperparathyroidism is reviewe…

Tonic pupil with giant cell arteritis.

A patient with giant cell arteritis and ischaemic optic neuropathy developed bilateral tonic pupils. This pupillary abnormality may result from ischaemia of the ciliary ganglia. Despite the propensity for patients with giant cell arteritis to develop vascular occlusions in the eye and orbit, tonic pupils have rarely been described, and several reasons for this are proposed. The arteries supplying the ciliary ganglia are frequently involved in giant cell arteritis, but their anastomotic pattern may confer protection from ischaemia. Tonic pupils may also be overlooked amidst the more dramatic ma

Giant cell arteritis and its variants.

In addition to cranial arteritis, the three other variants of giant cell arteritis may also lead to neurological symptoms: polymyalgia rheumatica, occult giant cell arteritis, and inflammatory aortic arch syndrome of elderly persons. This survey discusses the nosological, clinical, diagnostic, and therapeutic aspects of giant cell arteritis and its subgroups on the basis of experience with over 100 cases.

Giant cell arteritis with pulmonary nodules.

A patient presenting with polymyalgia rheumatica and biopsy-proved temporal arteritis showed response to corticosteroid therapy. Nodular pulmonary lesions developed twice, and diplopia and unilateral visual loss once, after reduction of the steroid dose. Response to reinstitution of full-dose steroids was dramatic. The clinical and pathologic findings in this patient suggest that the pulmonary lesions represented a previously undescribed manifestation of giant cell arteritis, but the possibility of limited Wegener's granulomatosis involving the temporal artery could not be ruled out.

Annular elastolytic giant cell granuloma.

Annular elastolytic giant cell granuloma (AEGCG) is a clinical and histological entity, which has been described previously in various different terms. Knowledge of AEGCG is of importance, since its misdiagnosis may lead to improper therapy; e.g. longterm tuberculostatic or corticosteroid treatment. In the present paper two patients with typical AEGCG are described and the clinical and histological differential diagnosis is discussed in detail.

Giant cell tumor associated with trauma.

Presented is a case report of a 19-year-old man who developed a giant cell tumor in the distal ulna following trauma to the carpus. Roentgenograms and bone scan revealed a fracture of the scaphoid and no other bony abnormalities. On year later, after the scaphoid fracture had healed, the same investigations demonstrated a giant cell tumor of the distal ulna. Circumstantial evidence suggests a possible role for trauma in the pathogenesis of giant cell tumor.

Maxillary giant cell reparative granuloma.

"Giant cell reparative granuloma" was introduced into medical literature by Jaffe in 1953. Prior to that time most authors considered this l … ence. These tumefactions are histologically a proliferative fibroblastic lesion with multinucleated giant cells. The histopathology of the giant cell tumor of the long bones is probably identical to the histopathology of the giant cell reparative granuloma of the jaw bones. The diagnosis of giant cell reparative granuloma must be made by physical examination, …

Ophthalmic complications of giant cell arteritis.

Temporal arteritis, or giant cell arteritis, is a common cause of blindness among older age groups. Classical and occult forms are described with particular reference to ocular symptomatology. Although the disease is self-limiting and usually subsides spontaneously within a year, prompt diagnosis and treatment are required if second eye involvement and blindness are to be prevented. Erythrocyte sedimentation rate is of major significance to diagnosis, but biopsy is necessary to prove or disprove the existence of temporal arteritis. Pathology, etiology, association with other disorders and labo

Giant cell arteritis with visceral angiitis.

We describe a case in which giant cell arteritis coexisted with microscopic polyarteritis nodosa and focal-local glomerulonephritis. We also review previous cases of renal abnormalities in giant cell arteritis. We believe that this association of inflammatory renal disease and giant cell arteritis has not been documented in the past.
