Peritoneal dialysis-associated peritonitis due to Nocardia farcinica: A case report.

Nocardia spp. are Gram-positive aerobic actinomycetes that are widely found in everyday environments, such as soil, vegetables, and water. They are known to cause infections in the lungs, central nervous system, and skin. However, peritonitis caused by Nocardia spp. is rare. We report a case of peritoneal dialysis-associated peritonitis caused by Nocardia farcinica in a 53-year-old woman. She had been undergoing peritoneal dialysis (PD) for approximately two years because of diabetic nephropathy. Gram staining of the PD effluent revealed the presence of Gram-positive rods. Because species-leve…

Polymicrobial bacteremia including Ignatzschineria indica caused by myiasis in a female patient with carcinoma of unknown primary.

A 70-year-old woman with a 6-month history of poor hygiene presented with a right occipital mass, ulceration, and neck swelling. The right occipital region was infested with approximately 100 fly maggots, and the mass contained a foul-smelling abscess. Maggots were removed, and the mass was drained, irrigated, and dressed with padding. Ceftriaxone 1 g/day was administered for 6 days. The patient was transferred on hospital day 32. Blood cultures obtained at admission were positive at 17.3 hours. Gram stain revealed both Gram-positive cocci and Gram-negative rods. However, only Providencia stua

[Severe phimosis as cause of urosepsis with Actinobaculum schaalii].

Actinobaculum schaalii is a small, Gram-positive, facultative anaerobic, CO2-requiring rod. It is now an acknowledged uropathogen, but often overlooked because of its slow growth. It is part of the normal bacterial flora in the urogenital area, but can be the cause of both local and invasive infections. We present a case of A. schaalii urosepsis in a 68-year-old male with phimosis caused by a severe lichen sclerosus et atrophicus. The only bacteria found in blood and urine was A. schaalii.

Rhodococcus equi pneumonia.

Corynebacterium equi is a pleomorphic gram-positive rod that was first isolated in 1923 by Magnusson, and is the cause of suppurative broncho-pneumonia in foals. The organism, now know as Rhodococcus equi, is ubiquitous in nature and is increasingly recognized as pathogenic, particularly in the immunocompromised population. Cavitary pneumonia clinically resembling Mycobacterium tuberculosis is the most common manifestation of R equi disease, but extrapulmonary infections are seen as well. Long term treatment with intracellularly active antimicrobial agents is required.

Prevalence of Extended-Spectrum Beta-Lactamase Producer Gram-Negative Rods and Associated Factors Among Patients With Wound Infection at University of Gondar Comprehensive Specialized Hospital, Northwest Ethiopia.

Background: Multidrug-resistant (MDR) bacteria have significantly affected the management and treatment of wound infections globally. Data on the prevalence of MDR bacterial profiles that cause wound infections in Ethiopia are scarce. Therefore, this study aimed to determine MDR as well as extended-spectrum beta-lactamase production profiles of Gram-negative rods that are difficult to treat with conventional antibiotics and that cause wound infections. Objective: The aim of the study was to determine the prevalence of extended-spectrum beta-lactamase producer Gram-negative rods and associated

Comparison of hemodynamic changes resulting from toxic shock syndrome toxin-1-producing Staphylococcus aureus sepsis and endotoxin-producing gram-negative rod sepsis in patients with severe burns.

Five patients with burns and toxic shock syndrome toxin-1 (TSST-1)-producing Staphylococcus aureus sepsis (TSS group) were treated in a 5-year period at Kyorin University Hospital's Traumatology and Critical Care Center Burn Unit. Hemodynamic and metabolic differences in these patients were compared retrospectively with those in another five patients who were matched by burn index and age and in whom endotoxin-producing gram-negative rod sepsis developed (End group). Both groups showed hypermetabolic and hyperdynamic changes at the point sepsis developed. There were no significant differences

Clinical evaluation of the BacT/Alert and isolator aerobic blood culture systems.

The BacT/Alert (BTA) (Organon Teknika, Durham, NC) and Isolator 10 (ISO) (Wampole Laboratories, Cranbury, NJ) blood culture systems were evaluated for their ability to detect aerobic and facultatively anaerobic microorganisms in blood of adult patients. For each culture 8 mL of blood was inoculated into both the aerobic standard BTA bottle and the ISO tube. Of 7,259 paired culture sets, 1,168 organisms were recovered, and 667 (57.1%) of these were considered clinically significant. This represented 540 clinically significant positive cultures from 266 patients. Of the significant isolates, 410

Quinine and Quinidine Derivatives as Photosensitizers for Photodynamic Inactivation of Bacterial Pathogens.

This study reports the synthesis and characterization of 5-azoniabenzo[no]tetraphene derivatives of Cinchona alkaloids with potential applications in antimicrobial photodynamic therapy (aPDT). These compounds exhibit absorption maxima at 430-441 nm, as well as in the UV-A (340-342 nm) and UV-C (270-275 nm) regions, with fluorescence emission peaks ranging from 519 to 534 nm. At nontoxic concentrations of 2 μg mL-1 and upon irradiation with blue light (418 nm), these compounds demonstrated potent bactericidal activity depending on the light dose. Staphylococcus aureus was eradicated after 5 min

Actinotignum schaalii Caught for the Second Time in Fournier's Gangrene!

Actinotignum schaalii, a Gram-positive rod residing in the urinary tract, is responsible for urinary tract infections and their complications including but not limited to bacteremia and sepsis. A. schaalii is increasingly being detected in body fluid specimens owing to advancements in PCR techniques. This report describes an interesting case of an adult diabetic patient managed for Fournier's gangrene. A. schaalii was detected in the PCR of his wound cultures. To the best of our knowledge, this is the second case of A. schaalii as the causative agent for Fournier's gangrene.

A case of Clostridium difficile bacteremia in a patient with loop ileostomy.

Clostridium difficile, an anaerobic, spore-forming, gram-positive, rod-shaped bacterium, is the most common nosocomial pathogen causing pseudomembranous colitis. C. difficile is not intrinsically invasive and rarely infects extraintestinal sites. The bacterium, therefore, is not commonly detected in blood cultures. Here, we report a case of C. difficile bacteremia in a patient who had underwent loop ileostomy because of rectal obstruction following metastatic colon cancer originated from prostate cancer.

Fatal septicemia in 2 South American camelids with caudal C3-pyloric-duodenal adenocarcinoma.

Gastrointestinal adenocarcinomas are often reported in South American camelids (SAC). We describe here cases of gastroduodenal adenocarcinoma in an adult alpaca (Vicugna pacos) and a llama (Llama glama); both SACs were anorectic and lethargic before death. At autopsy, a prominent and firm caudal C3-pyloric-duodenal junction with stricture and ulceration was present in both animals, as were hemorrhages in various organs and hydrothorax. Microscopically, scattered nests, cords, and tortuous acini of neoplastic epithelial cells were embedded in desmoplastic stroma and invaded the submucosa and mu

The Multi-Component Causes of Late Neonatal Sepsis-Can We Regulate Them?

Elucidating the mechanisms of bacterial translocation is crucial for the prevention and treatment of neonatal sepsis. In the present study, we aimed to evaluate the potential of lactoferrin to inhibit the development of late-onset blood infection in neonates. Our investigation evaluates the role of key stress factors leading to the translocation of intestinal bacteria into the bloodstream and, consequently, the development of life-threatening sepsis. Three stress factors, namely weaning, intraperitoneal administration of Gram-positive cocci and oral intake of Gram-negative rods, were found to

A case of feline gastrointestinal eosinophilic sclerosing fibroplasia associated with phycomycetes.

Feline gastrointestinal eosinophilic sclerosing fibroplasia (FGESF) is a recently described inflammatory condition of domestic cats with unknown aetiology. A proportion of cases of FGESF are associated with bacteria, but antibiotic treatment is ineffective. It has been hypothesized that genetically predisposed cats may develop FGESF in response to the introduction of bacteria or other antigens into the intestinal wall. A 9- month-old male Persian cat presented with a history of marked acute haematemesis. A mass (10 cm diameter) was detected within the pylorus and proximal duodenum and this was

Necrotizing fasciitis in patients with liver cirrhosis: predominance of monomicrobial Gram-negative bacillary infections.

Necrotizing fasciitis (NF), a rare but severe infection, usually occurs in individuals with underlying chronic illness, but its clinical presentation among cirrhotic patients is infrequently discussed. Forty-two cirrhotic patients with 47 episodes of NF between 1995 and 2006 were analyzed. Their mean age was 55.6 years, with male preponderance (34 patients, 81%). Lower extremities were mainly involved (70%). Of 42 episodes with identified pathogens, 41 (97%) were monomicrobial infections and were caused mainly by Gram-negative rods (GNBs) (32, 76%), including Vibrio (15, 36%), Klebsiella (9, 2
