Oral itraconazole therapy for superficial, subcutaneous, and systemic infections. A panoramic view.

The availability of the oral triazole agents itraconazole and fluconazole has revolutionized antifungal therapy. Although there are still some limitations and treatment failures, these agents have allowed for improved efficacy, increased safety, reduced morbidity, decreased mortality from systemic fungal disease, and a shift toward increased outpatient therapy for fungal infections that are not life-threatening. The treatment of superficial infections also has been enhanced by the development of effective intermittent and short-course regimens. Itraconazole exhibits broad-spectrum in vitro act

[Cutaneous necrosis of the extremities during carbon monoxide poisoning].

INTRODUCTION: Skin lesions may occur in approximately 30 p. 100 of cases of carbon monoxide poisoning. Flushes, erythematous to violaceous blisters on pressure zones due to sweat gland necrosis can be observed in such case. CASE REPORT: We report a case of acral (fingers, ears and nose) superficial cutaneous necrosis due to carbon monoxide poisoning. DISCUSSION: Such a complication had never been reported as a cutaneous manifestation of acute carbon monoxide intoxication. Also rare, acral necrotic lesions in a comatose or obnubilated patient should suggest this diagnosis.

Acquired ichthyosis in a child with rhabdomyosarcoma.

A generalized ichthyosiform eruption developed in a 13-year-old boy three weeks before presentation with a paravertebral rhabdomyosarcoma. His skin returned to normal shortly after the onset of chemotherapy and remission of the tumor. This case is unusual because the ichthyosis developed acutely, following a generalized urticarial eruption, and also involved the palms and soles.

Surgical treatment of painful lichen planus of the hand and foot.

Lichen planus of the hands and feet, although uncommon, can be very disabling with painful hypertrophic areas or ulcers, mainly of palmar and plantar surfaces. These lesions may not respond to conventional medical treatment. A case of surgical treatment with excision and split skin grafting of ulcerative lichen planus of the soles, with a fourteen year follow up, is reported. Only three other reported cases with a longer review period could be found. A second patient is described in which excision and grafting of painful, hypertrophic, ulcerative lichen planus of the hands achieved a satisfact

Discrete palmar and plantar symptoms in children with dermatitis herpetiformis Duhring.

Unusual, discrete palmar and plantar symptoms observed in thirty of forty-seven children with dermatitis herpetiformis are described. The diagnosis was verified in every case by the demonstration of granular IgA deposits in the skin. Forty-five of the children showed villous atrophy in jejunal biopsy specimens. In four cases extensive, exudative, bullous palmar symptoms, similar mild plantar changes, and healing with desquamation were observed. At least once during treatment we found very discrete, reddish-brown spots or small blisters on the flexor surface of the fingers and on the palms in t

Pustulosis palmaris et plantaris due to tonsillar focal infections. From the analysis of long-term observed cases.

We studied the effects of tonsillectomy in patients with pustulosis palmaris et plantaris (PPP) and suspected of having focal tonsillar infections which could still be observed more than one year after tonsillectomy. The rate of healing with tonsillectomy, including 6 cases with remarkable effect, was 76%. The clinical findings from which we expect an effect of tonsillectomy are (a) late onset PPP and early tonsillectomy, (b) tonsils with grade I hypertrophy, oozing of pus, and localized dark-redness on the anterior palatine arches, (c) anamnestic tonsillitis, and (d) positive results both at

Necrotizing Fasciitis? A Mimic Produced by an Unusual Presentation of Necrotizing Neutrophilic Dermatosis of the Hand: A Case Report.

CASE: A 50-year-old woman presented to our emergency department with hand pain and a laceration. Despite administration of antibiotics, the patient progressively worsened leading to a concern for a necrotizing soft-tissue infection. She underwent multiple debridements yet continued to worsen clinically. Tissue biopsy was eventually consistent with a necrotizing neutrophilic dermatosis. The patient was then started on oral steroids with rapid improvement of her clinical status and hand examination. CONCLUSION: While neutrophilic dermatoses are rare, the orthopaedic surgeon treating soft-tissue

[Hand for the dermatologist].

Through its anatomical, topographical, and functional distinctiveness, more than all other parts of the body, the hand is the interface between humans and their environment. All types of stimulus produce cutaneous signs in the hands, notably cold, light, pressure, contact, etc. Exposure to light makes it the region where photodermatosis is expressed, but also a particular site for cutaneous carcinogenesis. The countless chemical substances that the hand encounters can create specific or particular diseases in this location (irritative dermitis, contact eczema, hyperkeratosis, atrophy, etc.). T

The epidemic of methylisothiazolinone: a European prospective study.

BACKGROUND: The use of methylisothiazolinone (MI) in cosmetic products has caused an unprecedented epidemic of MI contact allergy. Current data concerning exposures at a European level are required. OBJECTIVES: To describe demographics and MI exposures for European patients with MI contact allergy. METHODS: Eleven European dermatology departments from eight European countries prospectively collected data between 1 May and 31 October 2015 among consecutive patients who had positive patch test reactions to MI (2000 ppm aq.). RESULTS: A total of 6.0% (205/3434; range 2.6-13.0%) of patients had
