The burden of preventable hospitalizations before and after implementation of the health transformation plan in a hospital in west of Iran.

BACKGROUND: Increased number of preventable hospitalizations (PHs) for ambulatory care sensitive conditions (ACSCs) represents less efficiency and low access to outpatient and primary health care, leading to waste of health system resources. AIM: The purpose of this study is to assess the quality of outpatient and primary health care using the rate of PHs for ACSCs and to estimate the economic burden of ASCS before and after the implementation of the health transformation plan (HTP) in Iran. METHODS: This research was a before-after quasi-experimental study. The study population included all

Tool of economic development, metric of global health: Promoting planned families and economized life in Nepal.

In contemporary global health and development discourses, contraception is cast in multiple roles: an antipoverty tool at the household level, a tool of economic development at the national level, a smart investment with net gains, a means of empowering women, a way of lowering maternal mortality ratios. In order to examine such discursive uses of contraception - and their implications for women - in a concrete way, I use a compelling case of the history of the promotion of planned families in Nepal and a recent social and behavior change communication contraception campaign designed in the US

Data linkages between patient-powered research networks and health plans: a foundation for collaborative research.

OBJECTIVE: Patient-powered research networks (PPRNs) are a valuable source of patient-generated information. Diagnosis code-based algorithms developed by PPRNs can be used to query health plans' claims data to identify patients for research opportunities. Our objective was to implement privacy-preserving record linkage processes between PPRN members' and health plan enrollees' data, compare linked and nonlinked members, and measure disease-specific confirmation rates for specific health conditions. MATERIALS AND METHODS: This descriptive study identified overlapping members from 4 PPRN regist

Planned birth at or near term for improving health outcomes for pregnant women with gestational diabetes and their infants.

BACKGROUND: Gestational diabetes is a type of diabetes that occurs during pregnancy. Women with gestational diabetes are more likely to experience adverse health outcomes such as pre-eclampsia or polyhydramnios (excess amniotic fluid). Their babies are also more likely to have health complications such as macrosomia (birthweight > 4000 g) and being large-for-gestational age (birthweight above the 90th percentile for gestational age). Current clinical guidelines support elective birth, at or near term in women with gestational diabetes to minimise perinatal complications, especially those relat

Quality process measures for rheumatoid arthritis: performance from members enrolled in a national health plan.

BACKGROUND: Health care quality problems are reflected in the underuse, overuse, and misuse of health care services. There is evidence suggesting that the quality of rheumatoid arthritis (RA) patient care is suboptimal, which has spurred the development of a number of systematic quality improvement metrics. OBJECTIVE: To investigate a quality process measurement set in a sample of commercially insured RA patients. METHODS: Medical, pharmacy, and laboratory claims for members with an RA diagnosis (ICD-9-CM 714.x) during calendar years 2008 through 2012 were extracted from the Optum Clinformat

The opportunity for health plans to improve quality and reduce costs by embracing primary care medical homes.

BACKGROUND: The large and growing costs of healthcare will continue to burden all payers in the nation's healthcare system-not only the states that are struggling to meet Medicaid costs and the federal government, but also the private health plans that serve commercial, Medicare Advantage, and Medicaid beneficiaries. Cost will increasingly become a concern as millions more people become newly insured as a result of the Patient Protection and Affordable Care Act (ACA). Primary care delivery through patient-centered medical homes (PCMHs) and other coordinated-care models have improved care and r

Measuring determinants of occupational health related behavior in Flemish farmers: an application of the Theory of Planned Behavior.

PROBLEM: Preventive interventions to reduce occupational injuries and health problems in farmers require the identification of factors that contribute to unsafe and health damaging behavior. This paper describes the development and validation of a self-report questionnaire, which measures the determinants of occupational health-related behaviors in farmers. METHOD: A representative sample of 283 Flemish farmers completed a provisional 135 item questionnaire based on the Theory of Planned Behavior, measuring four behaviors related to occupational health (machinery use, animal handling, prevent

Diabetes prevention and control: National Action Plan for NCD Prevention, Control and Health Promotion in Pakistan.

More than 10% of the adult population in Pakistan suffers from diabetes. The National Action Plan for Non-communicable Disease Prevention, Control and Health Promotion in Pakistan (NAP-NCD) incorporates prevention and control of diabetes as part of a comprehensive and integrated national non-communicable disease (NCD) prevention effort. Building on existing data, the diabetes surveillance process has been integrated with a comprehensive population-based NCD surveillance system using waist circumference as a proxy indicator for the risk of diabetes in the short term; however, the surveillance s

The potential impact of incomplete radiography histories in epidemiologic studies that rely on prepaid health plan records.

Members of prepaid health plans are often considered a group for which complete histories of exposure to medically-related risk factors are known. In a population-based case-control study of radiography as a risk factor for acute myelogenous leukemia, extensive retrospective exposure data from an average of 71% of subjects' health care providers were collected. It was determined that at least 21% of prepaid health plan members received diagnostic imaging procedures outside of their plans over the 10-y period of interest and that these "outside" procedures constituted 43% of their total red bon

Update through 1985 on the incidence of toxic shock syndrome among members of a prepaid health plan.

Through hospital records of men and women aged 15-34 years who were members of the Northern California Kaiser-Permanente Medical Care Program, trends in the incidence of toxic shock syndrome (TSS) requiring hospitalization in 1984-1985 were examined, and the results were compared with those for the period 1972-1983. No definite or probable cases of TSS were found in men during either 1984 or 1985. A total of 16 cases of TSS (10 definite and six probable) were found in women in 1984-1985. Ten (71%) of 14 cases for which the menstrual status of the patient could be determined unambiguously from

The International Alliance on Brain Health (IABH): Bridging global efforts toward integrated brain and mental health plans and the promotion of local implementations.

The convergence of neurological, psychiatric, neurodevelopmental, and public health approaches to brain health is reshaping global strategies for prevention, care, and policy. However, major gaps remain in the integration and implementation of brain and mental health frameworks across healthcare systems and regions. This paper describes the foundation and early development of the International Alliance on Brain Health (IABH), established in Switzerland in 2025 to promote interdisciplinary collaboration, reciprocal innovation, and implementation-oriented exchange between the global North and gl

Health-related characteristics and preferred methods of receiving health education according to dominant language among Latinos aged 25 to 64 in a large Northern California health plan.

BACKGROUND: Latinos are a fast growing segment of the U.S. health care population. Acculturation factors, including English fluency, result in an ethnic group heterogeneous with regard to SES, health practices, and health education needs. This study examined how demographic and health-related characteristics of Spanish-dominant (SD), Bilingual (BIL), and English-dominant (ED) Latino men and women aged 25-64 differed among members of a large Northern California health plan. METHODS: This observational study was based on data from cohorts of 171 SD (requiring an interpreter), 181 BIL, and 734 E
