Oxytocin modulates respiratory heart rate variability through a hypothalamus-brainstem-heart neuronal pathway.

The variation in heart rate in phase with breathing is called respiratory heart rate variability (RespHRV). Relaxation and positive socio-emotional states can amplify RespHRV, yet the underlying mechanism remains largely unknown. Here we identify a hypothalamus-brainstem neuronal pathway in rodents through which oxytocin (OT) amplifies RespHRV during calming behavior. OT neurons from the caudal paraventricular nucleus in the hypothalamus regulate the activity of a subgroup of inhibitory neurons in the pre-Bötzinger complex, the brainstem nucleus that generates the inspiratory rhythm. Specifica…

The effect of beta-blockers in acute heart failure according to heart rate.

BACKGROUND/AIMS: Beta-blockers (BBs) have been shown to improve clinical outcomes in heart failure (HF) patients. We evaluated the prescribi … significantly lower in the BBs group than the no BBs group. Among these four groups, patients with heart rate (HR) ≥ 60 beats/min with no BBs group had the lowest cumulative death-free survival rate. In addition, HR ≥ 60 beats/min with BBs use was independently associated with a 31% reduced risk of all-cause death in patients with HFrEF. CONCLUSION: BBs ha …

How Heart Rate Should Be Controlled in Patients with Atherosclerosis and Heart Failure.

PURPOSE OF REVIEW: Resting heart rate is an independent risk factor for all-cause and cardiovascular mortality in patients with heart failure. The main objectives are to discuss the prognosis of heart rate, its association with coronary atherosclerosis, and the modalities of control of the heart rate in sinus rhythm and in the rhythm of atrial fibrillation in patients with chronic heart failure. RECENT FINDINGS: As a therapeutic option for control heart rate, medications such as beta-blockers, digoxin, and finally ivabradine have been studied. Non-dihydropyridine calcium channel blockers are …

Heart rate response during exercise predicts survival in adults with congenital heart disease.

OBJECTIVES: To assess the prognostic value of heart rate response to exercise in adult congenital heart disease (ACHD) patients. BACKGROUND: An abnormal heart rate response to exercise is related to autonomic dysfunction and may have prognostic implications in ACHD. METHODS: We identified 727 consecutive ACHD patients (mean age [+/- SD] 33 +/- 13 years) with varying diagnoses and without pacemakers. Peak oxygen consumption (peak VO2), resting heart rate, and the increase in heart rate from resting level to peak exercise ("heart rate reserve") were measured. We also quantified the decrease in…

[Analysis of Foetal Heart Rate Data using Complex Software: Comparison of Recurrence Plot of Foetal Heart Rate with the Course of Pregnancy -].

BACKGROUND: Is it useful to further analyse foetal heart rate to improve the prediction of pregnancy complications? The analysis of the foetal heart rate is usually based on the variability of the heart rate, i. e. the more variable the heart rate presents - except a decrease - the better the condition of the foetus is. The same concept is applied in our own analysis which differs only in the presentation of the data. MATERIAL AND METHODS: We analysed 25 non-stress-tests from unselected third trimester pregnancies using sophisticated software. The recurrence plot (RP) is able to rearrange dat…

Increased heart rate variability but normal resting metabolic rate in hypocretin/orexin-deficient human narcolepsy.

STUDY OBJECTIVES: We investigated autonomic balance and resting metabolic rate to explore their possible involvement in obesity in hypocretin/orexin-deficient narcoleptic subjects. METHODS: Resting metabolic rate (using indirect calorimetry) and variability in heart rate and blood pressure were determined in the fasted resting state. Subjects included 15 untreated, hypocretin-deficient male narcoleptics and 15 male controls matched for age and body mass index. RESULTS: Spectral power analysis revealed greater heart rate and blood pressure variability in hypocretin-deficient male narcoleptic …

Nocturnal short-term heart rate variability reflects impaired daytime vigilance better than overnight heart rate variability in suspected obstructive sleep apnea patients.

In obstructive sleep apnea (OSA), heart rate variability (HRV) decreases and performance in psychomotor vigilance task (PVT) worsens with more severe hypoxic load. Nevertheless, the association between HRV and PVT performance is poorly understood. Thus, we hypothesize that nocturnal short-term HRV is better related to daytime psychomotor vigilance compared with overnight HRV. To investigate this hypothesis, we retrospectively analyzed the electrocardiograms from polysomnographies of 546 consecutive patients with suspected OSA. We determined overnight HRV and short-term HRV in nonoverlapping 5-…

Neuromodulation of heart rate and heart rate variability in a randomized controlled trial of tDCS-augmented in vivo exposure for specific fears.

This study evaluated the prediction that tDCS-augmented in vivo exposure (IVE) for specific fears would result in durable changes in heart rate (BPM) and heart-rate variability (HRV) during and just after exposure to feared targets. In a double-blind, placebo-controlled trial, participants with contamination- and animal phobia (N = 49) were randomized to active tDCS (1.7 mA, 20 min; n = 27), or sham tDCS (1.7 mA, 30 s; n = 22), followed by a single session of 30 min of IVE. Active tDCS targeted excitation of the left mPFC and inhibition of the right dlPFC. BPM and HRV were acquired during beha…

Cardiac autonomic nervous dysfunction detected by both heart rate variability and heart rate turbulence in prediabetic patients with isolated impaired fasting glucose.

OBJECTIVE: Cardiac autonomic nervous dysfunction (CAND), a severe complication of diabetes, has also been shown to affect prediabetic patients. The role of isolated impaired fasting plasma glucose (IFG), a subtype of prediabetes, is not clear in the pathogenesis of CAND. The aim of this study was to examine the relationship between isolated IFG and cardiac autonomic function using heart rate variability (HRV) and heart rate turbulence (HRT) indices derived from 24-h Holter electrocardiogram recordings. METHODS: This observational, prospective, cross-sectional study examined 400 consecutive su…

Time- and state-dependent analysis of autonomic control in narcolepsy: higher heart rate with normal heart rate variability independent of sleep fragmentation.

Narcolepsy with hypocretin deficiency is known to alter cardiovascular control during sleep, but its aetiology is disputed. As cardiovascular control differs between sleep states, and narcolepsy affects sleep architecture, controlling for both duration and transitions of sleep states is necessary. This study therefore aimed to assess heart rate and its variability in narcolepsy during sleep taking these factors into account. The study included 12 medication-naïve patients with narcolepsy with cataplexy and hypocretin deficiency (11 male, 16-53 years old), and 12 sex- and age-matched healthy co…

Aging and Postural Changes Influence the Agreement Between Pulse Rate Variability and Heart Rate Variability.

OBJECTIVE: Pulse rate variability (PRV) derived from photoplethysmography is often used as a surrogate for heart rate variability (HRV) in daily monitoring. However, their interchangeability in older adults and under postural change remains uncertain, potentially leading to misinterpretation in practical usage. Moreover, the mechanisms underlying HRV-PRV differences are incompletely understood. APPROACH: We evaluated HRV-PRV interchangeability across 18 features during four kinds of orthostatic tests in 35 younger and 42 older adults. Bland-Altman analysis assessed the interchangeability in d…

Circadian patterns of heart rate, respiratory rate and skin temperature in hospitalized COVID-19 patients.

RATIONALE: Vital signs follow circadian patterns in both healthy volunteers and critically ill patients, which seem to be influenced by disease severity in the latter. In this study we explored the existence of circadian patterns in heart rate, respiratory rate and skin temperature of hospitalized COVID-19 patients, and aimed to explore differences in circadian rhythm amplitude during patient deterioration. METHODS: We performed a retrospective study of COVID-19 patients admitted to the general ward of a tertiary hospital between April 2020 and March 2021. Patients were continuously monitored…

Isoflurane-Dependent Heart Rate Modulation by Cardiac Focused Ultrasound.

OBJECTIVE: Focused ultrasound (FUS) is an emerging noninvasive neuromodulation technique, yet its effects vary across studies due to multiple factors, including subject physiology, targeting accuracy, FUS exposure conditions and anesthetic state. This study investigated whether isoflurane-induced anesthetic depth modulates heart rate (HR) responses to cardiac FUS (cFUS) in mice. METHODS: The acoustic field of a FUS transducer was characterized via hydrophone measurements and numerical simulations. Transthoracic cFUS was applied to adult C57BL/6 mice under 1% or 2% isoflurane, with continuous …

Reliability of resting heart rate measurements in atrial fibrillation.

BACKGROUND: Heart rate (HR) control in atrial fibrillation (AF) is commonly guided by resting HR measurements, usually with 12‑lead ECG or in-office pulse palpation. OBJECTIVES: We aimed to assess the reliability of resting HR measurements in AF patients with long-term ambulatory ECG and activity monitoring. METHODS: We included patients who had recorded a full-disclosure ambulatory ECG with a mobile cardiac telemetry device equipped with an accelerometer (PocketECG, MEDICALgorithmics, Poland), with at least 95% AF. Resting HR samples (≤20 per individual/day) were collected as 10-s daytime (…

Heart rate optimizer: a novel bio-inspired metaheuristic algorithm.

The increasing complexity of real-world optimization problems demands robust and adaptive metaheuristic approaches. Despite notable progress, many existing algorithms suffer from premature convergence, imbalanced exploration-exploitation trade-offs, and sensitivity to high-dimensional search spaces. Motivated by the adaptive regulation mechanisms of the human cardiovascular system, this paper proposes a novel bio-inspired metaheuristic termed the Heart Rate Optimizer (HRO). The algorithm models heart rate variability and autonomic nervous system dynamics to enhance optimization performance, in…

Heart rate response to cannabis in active older adults.

PURPOSE: Cannabis use is increasing among older adults. Given reported associations between delta-9-tetrahydrocannabinol (THC) and increased acute heart rate, there is concern regarding the impact of cannabis on cardiovascular health in older adults. Engaging in regular physical activity (PA) is associated with reduced heart rate and cardiovascular disease risk, but it has yet to be explored whether PA status influences the acute effects of THC on heart rate in older adults. METHODS: This data is from a larger observational study of older adults using cannabis for sleep, pain, or mood problems…

Heart rate variability helps classify phenotype in systemic sclerosis.

We aimed to develop a systemic sclerosis (SSc) subtypes classifier tool to be used at the patient's bedside. We compared the heart rate variability (HRV) at rest (5-min) and in response to orthostatism (5-min) of patients (n = 58) having diffuse (n = 16, dcSSc) and limited (n = 38, lcSSc) cutaneous forms. The HRV was evaluated from the beat-to-beat RR intervals in time-, frequency-, and nonlinear-domains. The dcSSc group differed from the lcSSc group mainly by a higher heart rate (HR) and a lower HRV, in decubitus and orthostatism conditions. Stand-up maneuver lowered HR standard deviation (sd…
