Novel linkage of the Society of Thoracic Surgeons database and hospital electronic health records.

BACKGROUND: Society of Thoracic Surgeons Congenital Heart Surgery Database is the largest congenital heart surgery database worldwide but does not provide information beyond primary episode of care. Linkage to hospital electronic health records would capture complications and comorbidities along with long-term outcomes for patients with CHD surgeries. The current study explores linkage success between Society of Thoracic Surgeons Congenital Heart Surgery Database and electronic health record data in North Carolina and Georgia. METHODS: The Society of Thoracic Surgeons Congenital Heart Surgery

Prevalence of Neurodiversity in a UK High Secure Psychiatric Hospital Cohort: A Records Study.

BACKGROUND: The term neurodiversity is an umbrella term for any atypical pattern of cognitive ability, including but not confined to neurodevelopmental disorders. Research suggests that several neurodivergent conditions are overrepresented in offender populations, with a recent survey suggesting that over half of those coming into contact with the criminal justice system may have a neurodivergent condition. Considerable effort has been invested in trying to divert people with such conditions out of long-stay hospitals, but nevertheless, a few studies in secure hospitals suggest that while prev

Cancer Decedents' Hospital End-of-Life Care Documentation: A Retrospective Review of Patient Records.

Objective: International standards of end-of-life care (EOLC) intend to guide the delivery of safe and high-quality EOLC. Adequately documented care is conducive to higher quality of care, but the extent to which EOLC standards are documented in hospital medical records is unknown. Assessing which EOLC standards are documented in patients' medical records can help identify areas that are performed well and areas where improvements are needed. This study assessed cancer decedents' EOLC documentation in hospital settings. Methods: Medical records of 240 cancer decedents were retrospectively eval

Phenotypic Disease Network-Based Multimorbidity Analysis in Idiopathic Cardiomyopathy Patients with Hospital Discharge Records.

BACKGROUND: Idiopathic cardiomyopathy (ICM) is a rare disease affecting numerous physiological and biomolecular systems with multimorbidity. However, due to the small sample size of uncommon diseases, the whole spectrum of chronic disease co-occurrence, especially in developing nations, has not yet been investigated. To grasp the multimorbidity pattern, we aimed to present a multidimensional model for ICM and differences among age groups. METHODS: Hospital discharge records were collected from a rare disease centre of ICM inpatients (n = 1036) over 10 years (2012 to 2021) for this retrospecti

Underreporting of AIDS deaths in Brazil: linkage of hospital records with death certificate data.

This study investigated the underreporting of deaths due to Acquired Immunodeficiency Syndrome (AIDS) in Brazil, using the product of the linkage between the Mortality Information System (SIM) and the Hospital Information System (SIH) for the years 2008 to 2012. The hypothesis was that there were deaths recorded in the SIM that should have AIDS as the underlying cause of death, but that had been poorly classified. Many of the decedents had previous hospitalizations due to the illness. Underreported AIDS deaths were defined taking into account the coding rules of the International Classificatio

Toxic shock syndrome: incidence and geographic distribution from a hospital medical records reporting system.

A large database of hospital records maintained by the Commission on Professional and Hospital Activities Professional Activity Study (CPHA-PAS) was used to estimate the temporal incidence and geographic distribution of toxic shock syndrome (TSS). The CPHA-PAS hospital-diagnosed incidence was 3.5 times the reported TSS incidence, with a gradual decrease over the time period 1981-83. Marked differences in the regional occurrence of cases may provide clues to the etiology of this complex disease.

Precision phenomapping of pediatric dilated cardiomyopathy using clustering models based on electronic hospital records.

BACKGROUND: Pediatric dilated cardiomyopathy (PDCM) is a heterogeneous disease, and its clinical management is still considered challenging. This study aimed to establish clinically relevant PDCM subtypes to evaluate prognosis and guide its treatments. METHODS: Multidimensional data of study participants were derived from electronic hospital records based on a multicenter retrospective cohort in China. Six clustering models for heterogeneous data were adopted to identify PDCM subtypes, and multiple indices were used to select the best model. Multivariable Cox models were adopted to evaluate t…

Postpartum hemorrhage in electronic health records: risk factors at admission and in-hospital occurrence.

OBJECTIVE: Postpartum hemorrhage (PPH) is the leading cause of maternal death globally. Therefore, prevention strategies have been created. The study aimed to evaluate the occurrence of PPH and its risk factors after implementing a risk stratification at admission in a teaching hospital. METHODS: A retrospective cohort involving a database of SISMATER® electronic medical record. Classification in low, medium, or high risk for PPH was performed through data filled out by the obstetrician-assistant. PPH frequency was calculated, compared among these groups and associated with the risk factors.

Assessing the cause of in-patients pediatric diarrheal deaths: an analysis of hospital records.

Records of all the diarrheal patients up to the age of 5 years who were admitted to and died in Dr. B.C. Roy Memorial Hospital for Children, Calcutta, between January and December 1990 were analyzed. The records were reviewed to assess the relative importance of three clinical types of diarrhea (acute watery diarrhea, acute dysentery and persistent diarrhea) as the causes of mortality. Annual hospital death rates of children suffering from acute watery diarrhea, dysentery and persistent diarrhea were 13.6%, 18.2% and 25.9%, respectively. Overall death rates in dysentery (p = 0.03) and persiste

Linkage of maternity hospital episode statistics birth records to birth registration and notification records for births in England 2005-2006: quality assurance of linkage.

OBJECTIVES: The objectives of this study were to describe the methods used to assess the quality of linkage between records of babies' birth registration and hospital birth records, and to evaluate the potential bias that may be introduced because of these methods. DESIGN/SETTING: Data from the civil registration and the notification of births previously linked by the Office for National Statistics (ONS) had been further linked to birth records from the Hospital Episode Statistics (HES) for babies born in England. We developed a deterministic, six-stage algorithm to assess the quality of this

Mapping risks of hospital-recorded health conditions in people with eczema

Atopic eczema may be associated with multiple health conditions. Here, we systematically explored risks across the full health spectrum based on the International Classification of Diseases, assessing associations between eczema and 2,058 ICD-10 codes, 1,593 phecodes, and 201 Global Burden of Disease codes. In English primary care electronic health records (1997−2023) we identified cohorts of people with eczema (up to 3 million) and compared to matched comparators (individuals without eczema (by age, sex, general practice) (up to 14 million). In up to 25 years of follow-up, we captured outcome

Perspective: leveraging the electronic health record to improve sleep in the hospital.

UNLABELLED: Inpatient sleep loss can worsen health outcomes, including delirium and falls. Sleep disruptions in the hospital often originate from provider-patient interactions ordered electronically through computerized provider order entry. These orders contain clinical decision support systems with default schedules. These defaults are often around-the-clock, may not align with patients' needs, and cause iatrogenic sleep loss. Optimizing clinical decision support in the electronic health record can decrease unnecessary sleep disruptions and influence sleep-friendly decision-making. CITATION

Evaluation of the Remler M2000 blood pressure recorder. Comparison with intraarterial blood pressure recordings both at hospital and at home.

The Remler M2000 is a semiautomated device that has been used to collect epidemiological data and assess blood pressure variability. It has been subjected to limited evaluation in operation, however, and no studies of its accuracy away from the hospital or office environment have been undertaken. We recruited a group of 28 patients with essential hypertension who were undergoing intraarterial ambulatory blood pressure monitoring and compared the intraarterial recordings with those made with the Remler instrument both at home and in the hospital. The Remler recordings were also compared with si
