Opportunities for prevention of alcohol-related death in primary care: results from a population-based cross-sectional study.

The mortality rate from alcohol-related conditions has risen sharply in the United Kingdom and it is not known whether opportunities for preventive interventions could be improved. The purpose of our study was to identify opportunities to detect, assess, and manage alcohol problems in primary care according to evidence-based guidelines. We carried out a cross-sectional study on patients who died from alcohol-related conditions in the calendar year 2003 within National Health Service Greater Glasgow Health Board area, Scotland (population 920,000). We described patient characteristics and care

Impact of digital versus faxed e-referrals on triage decisions in a medical retina clinic.

CLINICAL RELEVANCE: Efficient triage is essential for managing increasing demand in resource-constrained ophthalmic services. Referral systems that enable structured clinical information and high-quality ocular imaging may support prioritisation according to clinical need and improve service efficiency. BACKGROUND: This study compared triage decisions made using traditional faxed referrals to those made using digital referrals with high-resolution retinal imaging in an Australian public hospital medical retina outpatient clinic. METHODS: Two ophthalmologists independently reviewed referrals

A Personalized, Symptom-Based Approach to Boost Research Participation in Hospitalized Older Adults.

BACKGROUND: Delirium is common among hospitalized older adults on many clinical services and associated with poor outcomes. Given delirium's fluctuations, wearable devices are promising continuous monitors. While recruiting for a wearable electroencephalography (EEG) delirium study, we initially experienced low enrollment rates among older adults and patients on non-neurologic services. Our aim was to understand patient and community perspectives on inpatient, wearable research to adapt recruitment protocols and increase enrollment. METHODS: We approached patients admitted to an academic medi

The orthogeriatric comanagement improves clinical outcomes of hip fracture in older adults.

UNLABELLED: Treatment of older adults with hip fracture is a healthcare challenge. Orthogeriatric comanagement that is an integrated model of care with shared responsibility improves time to surgery and reduces the length of hospital stay and mortality compared with orthopedic care with geriatric consultation service and usual orthopedic care, respectively. INTRODUCTION: Treatment of fractures in older adults is a clinical challenge due partly to the presence of comorbidity and polypharmacy. The goal of orthogeriatric models of care is to improve clinical outcomes among older people with hip

Post-Exposure Hepatitis B-Treatment in Newborns: A Qualitative Study on Interprofessional Practices in Norway.

AIM: Children born to mothers with chronic Hepatitis B virus (HBV) infection are at substantial risk of developing chronic HBV-infection without appropriate perinatal post-exposure treatment. This study aimed to explore midwives' and public health nurses' (PHNs) experiences with HBV-post-exposure treatment for infants and identify factors influencing the timeliness of follow-up care. METHODS: Data were collected through individual and focus group interviews with midwives and PHNs in hospitals and catchment-area municipalities and analysed with thematic analysis. RESULTS: The study revealed s

Factors affecting short- and long-term prognosis among 1069 patients with out-of-hospital cardiac arrest and pulseless electrical activity.

AIMS: To describe the epidemiology, resuscitation factors and prognosis among a consecutive population of patients suffering from out-of-hospital cardiac arrest (OHCA) where pulseless electrical activity (PEA) was the first arrhythmia recorded on emergency medical services (EMS) arrival. METHODS: Analysis of data collected prospectively on all patients regardless of age succumbing to OHCA during 1980-1997 in the community of Gothenburg where EMS initiated resuscitative measures. RESULTS: 4662 patients with OHCA were attended by the EMS during the study period. Of these, 1069 (23%) were judge

Needlestick and sharps injuries and its preventive implications among cleaners in tertiary public hospitals, Ethiopia.

BACKGROUND: Hospital cleaners frequently get needle stick and sharps injuries (NSSIs), which are described as unintentional injuries brought on by various sharp objects, needles, scalpels, and glass slides, cutting through the skin while providing cleaning and managing medical care. However, there are fewer studies among these marginalized working groups than among other healthcare staffs. OBJECTIVE: The study aimed to assess NSSIs, associated factors among hospital cleaners and preventive implications in eight selected tertiary public hospitals of eastern part of Ethiopia. METHODS: A hospital

Importing pancreata for transplantation: a single-center experience across evolving allocation eras.

Recent changes to allocation systems have increased the geographic distribution of pancreas offers, often originating from outside a transplant center's donor service area or region. The impact of this wider sharing on outcomes remains uncertain. This study analyzed outcomes of primary pancreas transplants (2000-2018) at a large transplant center, stratified retrospectively on the nautical miles distance from the donor hospital. Primary endpoints were death-censored graft survival (DC-GS), patient survival, and graft thrombosis at different time points. No significant differences were found in

Practice Expense, Site of Service, and the Survival of the Independent Interventional Pain Practices: With Medicare Policies Leading the Way.

BACKGROUND: Practice expense (PE) accounts for approximately 45% of relative value units (RVUs) under the Medicare Physician Fee Schedule (PFS), yet it receives substantially less analytic attention than physician work. For interventional pain management (IPM), a specialty in which physicians frequently own the procedural suite, imaging equipment, and clinical infrastructure and employ the clinical and administrative staff, or perform surgical interventions in an Ambulatory Surgery Center (ASC) or hospital setting, PE is not merely an accounting abstraction but a major determinant of practice

Supporting Aboriginal Community Controlled Health Services to deliver alcohol care: protocol for a cluster randomised controlled trial.

INTRODUCTION: Indigenous peoples who have experienced colonisation or oppression can have a higher prevalence of alcohol-related harms. In Australia, Aboriginal Community Controlled Health Services (ACCHSs) offer culturally accessible care to Aboriginal and Torres Strait Islander (Indigenous) peoples. However there are many competing health, socioeconomic and cultural client needs. METHODS AND ANALYSIS: A randomised cluster wait-control trial will test the effectiveness of a model of tailored and collaborative support for ACCHSs in increasing use of alcohol screening (with Alcohol Use Disorde

Record fragmentation due to transfusion at multiple health care facilities: a risk factor for delayed hemolytic transfusion reactions.

BACKGROUND: Patients transfused at more than one health care facility face safety risks, because their transfusion record is fragmented. Blood group antibodies documented at one facility may be unknown to others. Because many antibodies are evanescent, access to prior antibody records is important for preventing incompatible transfusions and delayed hemolytic reactions. The study goal was to quantify multisite transfusion activity and its impact on antibody record accuracy. STUDY DESIGN AND METHODS: Patients (n = 100) undergoing hospital transfusion testing were surveyed to determine the loca

Comprehensive Management of the Patient With Traumatic Cardiac Injury.

Cardiac injuries are rare but potentially life-threatening, with a significant proportion of victims dying before arrival at the hospital. The in-hospital mortality among patients who arrive in-hospital alive also remains significantly high, despite major advancements in trauma care including the continuous updating of the Advanced Trauma Life Support (ATLS) program. Stab and gunshot wounds due to assault or self-inflicted injuries are the common causes of penetrating cardiac injuries, while motor vehicular accidents and fall from height are attributable causes of blunt cardiac injury. Rapid t

Thiazide-associated hyponatremia in arterial hypertension patients: A nationwide population-based cohort study.

OBJECTIVE: Thiazides are the first-line treatment for hypertension, however, they have been associated with hospitalizations for thiazide-associated hyponatremia (TAH). The aim of this study was to evaluate the risk of TAH and other drug-associated hyponatremia in a Korean population. METHODS: The study used big data from the National Health Insurance Sharing Service of 1,943,345 adults treated for hypertension from January 2014 to December 2016. The participants were divided into two groups based on the use of thiazides. Cox proportional hazard models were used to identify independent risk f

Unintentional Injuries Are Associated with Self-Reported Child Maltreatment among Swedish Adolescents.

Injuries constitute a large share of childhood morbidity and mortality. This study examines whether adolescents with self-reported experiences of different types of child maltreatment more frequently reported unintentional injury events requiring health- or dental care during the last year and/or hospitalization at any time during childhood. Cross-sectional data from a Swedish national representative school survey (2016) including 4741 adolescents were used (78.5% response rate). Data were analyzed with univariate tests and multiple logistic regression. Statistically significant associations b

Women's experiences of marital rape and sexual violence within marriage in India: evidence from service records.

Sexual violence within marriage is common and manifests in various forms, including marital rape. It has serious physical and mental health consequences and is a violation of women's sexual and reproductive health rights. Marital rape, reproductive coercion, inserting objects in the vagina or anus, and withholding sexual pleasure are forms of violence routinely experienced by women. Based on service records of survivors coming to public hospitals in an Indian city, this paper presents their pathways to disclosure and institutional responses such as hospitals and police. The findings highlight

Health USA. A national health program for the United States.

The Health USA Act of 1991 addresses two fundamental health services financing problems: the more than 30 million uninsured persons and the rising costs for health care and for health insurance. Health USA would provide coverage of the entire resident population for comprehensive medical and preventive health and long-term care services through a universal tax-funded financing system. The federal government would contribute an average of 87% of program costs to each state, which would establish, under federal guidelines, a state health program. Each individual or family may enroll in any healt
